4 GENERAL BUILDING INFORMATION BUILDING ACCESS The building is open: Monday Friday... 8am 6pm Saturday.... 9am 1pm Sunday and Holidays... Closed MAILBOX LOCATION The building s mailboxes are located in the main lobby next to the elevator. Your property manager will provide you keys and your mailbox number. JANITORIAL Janitorial service is provided by Performance Building Services. Service is five nights per week, Monday Friday. A Day Porter services the building one hour per day; from 9am 10am. BUILDING RULES AND REGULATIONS A complete list of rules and regulations can be found in your lease. For questions or concerns please contact the management office. ORATE TENANT REFERENCE GUIDE 2
5 RENT Rent payments are due on the first day of each month. Please make rent checks payable to; 41 Corporate Park, LLC and mail to: 41 Corporate Park, LLC P.O. Box Los Angeles, CA As a courtesy, rent statements are ed to the tenants on or about the 20th of the month. However, failure to receive a statement does not relieve tenants of the obligation to pay rent on the first of the month. ORATE TENANT REFERENCE GUIDE 3
6 ACCESS FOBS & PARKING ACCESS FOBS Building access after regular building hours is controlled by an access system. Please complete the access fob request form (located in your Welcome Folder) for each employee who needs an access fob and return to your property manager. Each tenant is allowed four (4) free access fobs for every 1,000 sf of leased space. Access fobs in excess of the allotted number can be purchased for a cost of $50.00 per fob. Additionally, please return the access fobs when individuals leave employment with your company so that we may de-activate them. Access fobs must be returned at the end of lease or a charge will be incurred. ORATE TENANT REFERENCE GUIDE 4
7 BUILDING MAINTENANCE & ANGUS ANYWHERE ANGUS ANYWHERE To make any service or maintenance requests please utilize our secure tenant services system called Angus Anywhere. This password-protected system will allow you to submit Service Requests, view Request Status Reports and ensure that the information that you enter remains confidential. You can access the system by pointing your web browser to: TO SET-UP A NEW EMPLOYEE Please complete the Angus Anywhere Employee Username Request Form (located in your Welcome Folder) and return to property manager. One or more of the employees from your office will also need to be authorized to approve any items that are above standard and billable. 8 SECURITY SUITE SECURITY If you have installed a security system in your suite please include the security alarm code and the security alarm company name on the Emergency/Tenant Contact Form (located in your Welcome Folder). BUILDING SECURITY Vandalism, theft, or other incidences should be reported first to the Police Department and then to the management office at (949) Tenants are required to insure their space for losses as described in the lease, and provide Mark IV Capital, Inc. with proof of insurance (See Tenant Insurance, page. 10). 10 MOVE IN/OUT PROCEDURES Our management staff and building employees make many preparations for your move. To help facilitate the process, please make sure we have been informed of your move-in/out date and moving company information as soon as possible. MOVE IN/OUT PROCEDURES Moving must be completed after 6pm Monday through Friday or on Saturday and Sunday. Please refer to the attached moving checklist. If you choose to utilize the elevator, the elevator must be blanketed. Do not roll anything over the lobby floor without protecting it first and lobby doors must not be propped open. Also, do not dump any furniture or electrical equipment in the trash bins. MOVE IN/OUT INSURANCE REQUIREMENTS If you are using a moving company, they will need to provide a certificate of insurance prior to move-in (See Vendor Insurance, page 10). 11 MOVING CHECKLIST Notify Property Manager of moving schedule Contract with utility vendor Contract with cabling vendor *NO CUTTING OF CABLES ALLOWED Contract with IT and Phone/Internet vendor Send vendor insurance to property manager Contact management to blanket elevator Discuss lobby floor protection with moving vendor Coordinate to have any large trash items disposed of off-site ORATE TENANT REFERENCE GUIDE 9
12 INSURANCE REQUIREMENTS TENANT INSURANCE Pursuant to Section XI of the Lease, Tenant is required to maintain certain insurance and provide evidence of that insurance to Landlord. Satisfactory evidence is a Certificate of Insurance AND an Additional Insured Endorsement to your policy that names the building and Mark IV Capital, Inc., as the additional insured. Workers compensation is also required. Please refer to your lease for insurance requirements. VENDOR INSURANCE In addition to Tenant Insurance requirements, any vendors completing work in the building, including moving companies, are required to also provide a current certificate of insurance naming the building and Mark IV Capital, Inc. as additional insured. For your convenience, the correct wording for the Certificate Holder and Additional Insured Endorsement is listed below. Please refer to your lease for insurance requirements.
