2 Service descriptin Older Persns and Rehabilitatin services include: Cmbined Strke service Prvides acute care fr patients with strke and thse strke patients requiring assessment, treatment and rehabilitatin. Orthpaedic rehabilitatin Wrks in cllabratin with acute rthpaedics t ensure a seamless apprach between pst-perative r acute care f lder peple with fractures and active and intensive rehabilitatin. General assessment, treatment and rehabilitatin Fr peple 65 years + r thse clse in age, interests and characteristics. Intensive rehabilitatin Fr peple 16 years + wh meet set criteria. Older Persns and Rehabilitatin clinics Specialist-led clinics that take a multidisciplinary apprach and address a wide range f issues and cnditins f lder patients referred by GPs r hspital services. Disability Supprt Link (DSL) Prmte a persn s quality f life and create an envirnment that enables cmmunity participatin and maximum independence. It identifies and crdinates hme supprt services fr peple with disabilities in the Waikat, King Cuntry, Thames Valley and Crmandel Peninsula (except thse with shrt-term illnesses that will require supprt services fr less than six mnths and peple will ACC-related cnditins). START (Supprted Transfer and Accelerated Rehabilitatin Team) Prvides intensive in-hme rehabilitatin t thse discharged frm hspital, fcusing n achieving persnal, meaningful gals identified by the patient and their family. Services are delivered at multiple DHB sites acrss the District Health Hard bundaries Purpse f the psitin T be respnsible fr prviding academic and clinical leadership in relevant research and schlarship, teaching and learning and innvatin fr the benefit f BUPA and Waikat DHB Older Persns and Rehabilitatin Service. T prvide cnsultant services within the Older Persns and Rehabilitatin Services. Medical Specialist means any medical practitiner wh is vcatinally registered by the Medical Cuncil under the Health Practitiners Cmpetence Assurance Act 2003 in ne f the apprved branches f medicine and wh is emplyed in either that branch f medicine r in a similar capacity with minimal versight. Senir Medical Officers and Dental Officers Cllective Agreement Is required t undertake their clinical respnsibilities and t cnduct themselves in all matters relating t their emplyment in accrdance with best practice and relevant ethical and prfessinal standards and guidelines, as determined frm time t time by: the New Zealand Medical Assciatin s cde f ethics the practitiner s relevant medical cllege(s) and/r prfessinal assciatins the New Zealand Medical (r Dental) Cuncil the Health and Disability Cmmissiner the emplyer s plicies and prcedures except t the extent that they may be incnsistent with any ther prvisin f the Senir Medical Officers and Dental Officers Cllective Agreement Registratin with Medical Cuncil f New Zealand, therefre inclusive f scpe f practice f specialist.
