1 Job Description POSITION: PHYSICIAN RESPONSIBLE TO: CLINICAL DIRECTOR, MEDICAL SERVICES PRINCIPAL OBJECTIVES: 1. To provide medical services to the community served by the South Canterbury District Health Board. 2. To provide appropriate and timely advice and assistance to the Acute Care Services. 3. To instruct, monitor and advise junior medical officers and other health professionals as appropriate. 4. To work constructively with other Health Professionals to attain high standards of care in their area of clinical expertise. 2 -2- DUTIES AND RESPONSIBILITIES KEY TASKS The assessment, treatment and care of medical patients within South Canterbury District Health Board Health Services on an ongoing basis during regular working hours and on a rostered basis outside regular hours including weekends and public holidays. PERFORMANCE TARGETS The Physician will care for patients admitted to medical beds and will carry out and/or supervise the assessment, investigation and treatment of patients under his care. Share duties with other Physicians so that one is on call and available at all times for the hospital. Medical admissions to hospital will be seen as soon as practicable, consistent with good patient management and no later than 24 hours after admission. The Physician will conduct sufficient planned ward rounds (a minimum of 3 rounds per week) during regular hours to monitor care, check progress and allow early discharge consistent with best practice thereby maintaining a high standard of patient care. While on call the Physician will be available within 10 minutes by phone and will be able to return to the hospital within 20 minutes. The Physician will appropriately manage patients on an outpatient basis, admitting patients when necessary. Patient stay in hospital shall be kept at a minimum level, appropriate to a high standard of care. The Physician will conduct sufficient specialist outpatient clinics to meet contract target and service specifications, while conforming to best practice, subject to availability of support staff and facilities. New patients referred by General Practitioners or other specialists will be seen expeditiously. Urgent patients will be seen within 2 weeks, and non urgent patients within 6 months at most. Follow up patients from Accident and Emergency Department, wards and elsewhere will be seen as

3 required. The Physician will discuss patients with the physician providing medical cover prior to any planned absence from work.

4 -3- DUTIES AND RESPONSIBILITIES KEY TASKS Length of waiting time for initial appointments will be kept to a minimum as availability of staff and facilities permit. PERFORMANCE TARGETS Outpatients will be seen with priority being allocated according to clinical need in accordance with the contract target and service specifications. The Physician will advise on any patients referred to him in relation to medical care. See and advise as soon as possible on patients referred by Hospital specialist colleagues for a medical opinion. Referrals will be admitted to the medical ward for investigation, and treatment if appropriate. Medical care will conform to best practice and meet contract target and service specifications. The referring doctor will be kept informed of inpatient and outpatient progress. A preliminary discharge summary will be issued to each patient prior to them leaving hospital, in accordance with service specifications. The Physician is to discuss planned care with the patient and other professionals as appropriate. Make appropriate use of other departments (e.g. Audiology, Physiotherapy, Occupational Therapy, Laboratory, X-Ray, Dental, etc), or refer to other hospitals regarding patient diagnosis and rehabilitation in accordance with best clinical practice. Maintain effective interpersonal relationships with all staff and patients. Ensure that patients are kept fully informed of and understand all procedures and treatments to be undertaken. Informed Consent will be obtained from patients in accordance with South Canterbury District Health Board Be conversant with, and adhere to the patients Code of Rights and Obligations, and be aware of cultural differences when dealing with patients.

5 Should the physician receive or become aware of complaints relating to the standard of care and/or the treatment of patients, or professional attitudes toward staff, these shall be referred to the Clinical Director of Medicine, or discussed with the Medical Adviser.

6 --4- DUTIES AND RESPONSIBILITIES KEY TASKS The Physician will take an active role in health promotion and disease prevention. PERFORMANCE TARGETS Provide advice to patients regarding healthy lifestyles and specific measures that may help the patient avoid a recurrence of ill health. The Physician is to maintain adequate records. Ensure that comprehensive hospital medical records for all in and outpatients are accurately maintained. Selected medical records, patient casenotes, and discharge letters will be evaluated at peer review. Each referring doctor will receive documentation of the progress and treatment of the patient. Liaise on behalf of any patient or the organisation with outside agencies, (e.g. Accident Compensation Corporation, Social Welfare Department. Insurance companies etc) where this is warranted. To provide educational and clinical experience for junior medical staff. The Physician shall instruct, guide and supervise the work of the Medical House Officer. The House Officer s work performance shall be discussed with him/her during and at the end of the rostered time with the Medical Service. The House Officer will be given clear guidelines and expectations of the Medical Service and will be encouraged to be aware of relevant departmental, hospital and government policies. A written report relating to the House Officer s performance will be given to the Intern Supervisor every three months. The Clinical Director Medical Services will also be informed of any unsatisfactory performance, as soon as it comes to the physician s attention.

7 To contribute to teaching sessions for other hospital staff as appropriate. Participate in teaching sessions of consultant colleagues on a regular basis and provide regular teaching sessions for attached medical officers and/or students.

8 DUTIES AND RESPONSIBILITIES KEY TASKS -5- PERFORMANCE TARGETS MANAGEMENT AND ADMINISTRATION To ensure the efficient use of resources in medical services. - Business Plan - Performance Target - Contract volume Length of stay New patient follow up - Efficient use of diagnostic and pharmacy resources. As a major user of finite and limited resources will have a responsibility together with colleagues and other health professionals to use these efficiently and effectively in the day to day management of the Medical Service, taking cognisance of information provided by advisory groups and management. As the physician must endeavour to provide medical services within budget, the physician will consult with the Clinical Director and the Clinical Nurse Manager regularly, as to the use of medical, nursing and/or hospital resources. To provide reports and contribute to the planning, budgeting and development of the medical service. May be required to participate in the forward planning and budgeting of the medical service. To provide technical support to the Clinical Director on request. Endeavour to suggest new initiatives to aid the efficient provision of Medical Services. PERSONAL EDUCATION AND RESEARCH The physician will continue to keep abreast with trends, developments and the appropriate utilisation of technology in the field of medical care on a regular and ongoing basis. Participate in available educational sessions on topics such as clinical budgeting, medical audit, performance appraisal and quality control. The physician will participate in journal review with colleagues and junior staff.

9 Assures clinical practice is current by availing themselves of opportunities to attend relevant educational programmes and report back to colleagues on any programmes attended.

10 -6- DUTIES AND RESPONSIBILITIES KEY TASKS PERFORMANCE TARGETS Studies medical literature and other resource material on a regular basis. Records evidence of their own educational and developmental initiatives. This will form an integral part of the annual performance review. Attendance at local post-graduate meetings is expected. QUALITY ASSURANCE Develop, implement and maintain a Quality Assurance Programme for the Medical Service. Assistance may be available from a Quality Improvement co-ordinator. This programme and the Audit associated with it may need to be co-ordinated with other programmes of other Health and Hospital Services. The quality assurance system must incorporate review mechanisms of patient care, allow regular review of patient deaths and of critical incidents that occur. Review will be based on recognised standards of practice for medical services. If such a programme is instituted it will consume between two and four hours per month and adjustment to hours of work may be recognised as necessary. GENERAL The physician may also be expected to carry out other duties from time to time as agreed with the Clinical Director of Medicine. It is accepted that all targets in this Job Description will be reviewed on a regular basis (at least annually). The Job Description may be amended from time to time by the Clinical Director, in consultation with the physician.
