1 Job Description Deleted: Role Profile Job Title Transitions Project Manager Barnet Band and Scale range SCP ,070-40,716 Reports to Head of Service, Learning Disabilities Service area Adult Social Care and Health Number of staff responsible for Budget responsibility ( ) Context and Purpose of the job Context The London Borough of Barnet has developed a Youth to Adulthood Transitions Strategy working across the council and with partners to support young people who are significantly disabled or have high level complex needs to progress to adulthood. Adult Social Care and Health and Children s Services will work together with partners to ensure there is a straightforward transition process in place to support young people and adults with complex needs. The Transitions project manager will lead the implementation of a new Transitions action plan which includes our transition commissioning strategy, joint working across children s and adults services and developing an improved pathway that provides an excellent service and high quality information for service users, carers and stakeholders about the transition process. To implement this action plan and progress joint working successfully, we require a Project Manager to manage change and ensure that all stakeholders are involved with, and benefit from it. These stakeholders include: Young people and adults with complex needs and their families and carers the council s public, private and voluntary sector partners; Statutory Services including Children s Services and Health partners Purpose of the Job The role of the Project Manager Transitions is central to our successful implementation of the Transitions action plan. He / she will draw on previous experience of project management to successfully run high profile transitions work to deliver both our financial savings and strategic aims. To lead this project successfully, the Project Manager Transitions will employ project management, business analysis and change management expertise that he / she will have developed through previous work. Through his / her work, the Project Manager will deliver change in a manner that: is controlled and sustainable; is sensitive to the needs to stakeholders; engages and benefits stakeholders. 1

2 The Project Manager will play an important role in developing the department s strategic plans for transition. This will require a strong understanding of the personalisation agenda for social care and the wider political, financial and social environment within which local government works. This will also require the ability to advice, challenge and work closely with senior managers from the Council and its partner organisations. Accountabilities: Key Responsibilities: Develop the main project plan and other associated plans for the project, working with relevant staff in the Council and beyond as appropriate. Manage delivery of the project and work with the Head of Service for LD and others as appropriate to ensure sufficient resources for the project Ensure the project delivers the required benefits on time and on target, meeting quality requirements Support and administer the project board. Responsible for the definition, documentation and successful completion of work within a project-management framework. The Project Manager will manage whole projects to ensure transitions processes are developed and sustained. Develop and maintain communication at all levels across the organisation to facilitate smooth and effective delivery of projects. Design, organise and facilitate change activity with staff at all levels and external stakeholders, developing changes that will meet their requirements and deliver the desired outcomes of the department. Promote new ways of working and facilitate change by ensuring buy-in and support at all levels of the service. Creatively apply business and technical knowledge to enable, support and review all aspects of change in the service. Change and Improvement: To deliver projects to a high standard: using corporate project management standards; developing realistic project, quality and risk plans; reviewing project plans regularly and capturing lessons learnt; maintaining stringent programme controls, in line with Prince 2 principles and with effective forecasting, planning and reporting processes. To lead change activity with stakeholders both within and outside of the service, as is specified above. Financial and Asset Responsibilities The Project Manager Transitions will have no direct financial or budgetary responsibilities. 2

3 Staff Responsibilities The Project Manager Transitions will have no direct line management responsibility. The role will also co-ordinate work across partner organisations and service managers Promotion of Corporate Values It is expected that the post holder will work in a way which furthers the values of the organisation, with particular reference to customer care, equal access and opportunity and quality of service. Flexibility In order to deliver the service effectively, a degree of flexibility is needed and the post holder may be required to perform work not specifically referred to above. Such duties, however, will fall within the scope of the post, at the appropriate grade.
