Working alongside and supporting other carers to deliver a high standard of continuing care and respite to families who have children and young people with long-term and/or complex health care needs in their own homes, other community settings and hospital if required.

2 The post holder will be required to administer a restricted range of care to these children on a named child basis following a period of orientation, training and assessment resulting in verification of competency. KEY JOB RESPONSIBILITES 1.1 Under direction, initiate appropriate action should the condition of the child or young person change from the agreed parameters previously set by the multi-disciplinary team, and be competent to undertake support measures following training and assessment on a named child basis. 1.2 With support, deal sensitively with families who are coming to terms with their child s complex needs. 1.3 With support, work with the children s family as a whole respecting their needs and wishes whilst providing the best care for the child. 1.4 Work alongside a multi-disciplinary team to enable children reach their full potential. 1.5 With support, act as the child s advocate ensuring the child s best interests come first. 1.6 With support, ensure correct paperwork is in the homes and inform community nurses of any changes to care required. 1.7 Provide written and verbal reports of care provided including, communication sheets, play communication and medication parental permission forms. 1.8 Assist colleagues in completing care/play reports when required to assist multi-disciplinary team in providing the correct care package 1.9 Undertake regular carrying, moving and handling as required to provide appropriate care and offer therapeutic play as agreed with a team of therapists With support, assist families to cope with their child s changing needs and possible life limiting condition/s as well as bereavement Use a range of verbal and non-verbal communication tools to communicate effectively with children and their families to progress rehabilitation and treatment programmes. This will include children who have difficulties in understanding or communicating e.g. children with neurological difficulties such as profound developmental delay, hypersensitivity, sensory defensiveness, visual and hearing problems Under direction, provide health, play and recreational education and activities to children, young people and their families to ensure a healthy and stimulating environment at all times Contribute towards maintaining an environment conductive to learning and development, by offering activities tailored to the individual child or young person, whilst considering, developmental level and age appropriateness Ensure that available play and therapeutic equipment is functioning safely and correctly within the working environment

3 2. 0 Education and Training 2.1 To undertake mandatory training including cardio-pulmonary resuscitation, child protection, moving and handling, fire safety and food handling Through training and assessment become competent in the assessment of children with complex needs and be able to undertake some essential clinical skills under supervision on a named child basis. 3.0 Research and Audit 3.1. To participate in the monitoring and evaluation of the service e.g. through audit, as directed by the Team Leader To participate in audits that underpin practice. 3.3 To undertake any other duties that may reasonably be required. GENERAL DUTIES In addition to the key job responsibilities detailed in this job description all employees at Nottingham University Hospitals NHS Trust are expected to comply with the general duties detailed below. Infection Control To maintain a clean, safe environment, ensuring adherence to the Trust s standards of cleanliness, hygiene and infection control. Health and Safety To take reasonable care to prevent injury to themselves or others who may be affected by their acts or omissions To co-operate fully in discharging the Trust policies and procedures with regard to health and safety matters To immediately report to their manager any shortcomings in health and safety procedures and practice To report any accidents or dangerous incidents to their immediate manager and safety representative as early as possible and submit a completed accident/incident form. To use protective clothing and equipment where provided Whilst the aim of the Trust is to promote a co-operative and constructive view of health and safety concerns in the organisation, all staff must be aware that a wilful or irresponsible disregard for safety matters may give rise to disciplinary proceedings. All staff and volunteers are therefore expected to behave in such a way that supports this commitment.

4 Information Governance All staff has an individual responsibility for creating accurate records of their work and for making entries into and managing all NHS records effectively in line with the Health Record Keeping Policy and other Health Records and Corporate Records Management policies and procedures in order to meet the Trust s legal, regulatory and accountability requirements. Uniform Policy You are expected to adhere to the Trust dress code as laid out in The Trust Uniform Policy. This includes: Wearing soft soled shoes to reduce noise Keep fingernails short, clean and free from nail polish. Do not have false nails or nail extensions. Be bare below the elbow roll-up long sleeves and do not wear a watch Do not wear stoned rings (a single band is permitted) Governance To actively participate in governance activities to ensure that the highest standards of care and business conduct are achieved General Policies Procedures and Practices To comply with all Trust policies, procedures and practices and to be responsible for keeping up to date with any changes to these. WORKING CONDITIONS The post holder s normal working conditions will include: moving and handling of physically disabled children; travelling in and around Nottinghamshire; working in homes in the community; exposure to bodily fluids and to work as a lone worker. ACCESS TO PATIENTS In undertaking the duties outlined above the post holder will have access to patients. This means that the post is exempt from the Rehabilitation of Offenders Act 1974 and all post holders must disclose any criminal conviction including those considered as spent under the Act. Post holders appointed to this will be required to consent to a check through the Criminal Records Bureau. REGISTERED HEALTH PROFESSIONALS All persons appointed to the post are required to hold registration with their appropriate professional Regulatory Body and to comply with their professional code of conduct.
