It outlines our programs and services, summarizes the major policies and procedures of relevance to the student. The information contained in this catalog is not necessarily all-inclusive. 4 President s Message: I would like to extend a warm welcome to the students of Hawaii Scuba University. I recall the excitement that I felt as I pursued my own professional diving credentials. When I realized that I could turn a sport that I enjoyed immensely into a profession it seemed a little bit like a dream come true. I was going to get paid to scuba dive I couldn't believe my luck! Over the years it has been my pleasure to introduce thousands of people to the underwater world through my vocation as a professional diver and that has been an incredibly rewarding experience. Scuba diving can be a transformational experience, and to be able to bring that experience to others is truly a worthwhile goal. It will certainly enrich your own life. Hawaii Scuba University's programs will assist you in making your goals a reality by providing you with professional diving credentials through the world s leading professional diving organization, PADI. Our network of professional staff, retail locations, pool training facilities, marina facilities and USCG inspected dive vessels will make it possible for you to become fully prepared to enter the professional diving field. 5 Recreational diving courses leading to the professional rating of Divemaster, Assistant Instructor, Open Water Scuba Instructor, Master Scuba Diver Trainer and Instructor Development Course Staff Instructor. HSU is not and accredited institution. Individual PADI courses are ACE accredited. HSU is a Professional Association of Diving Instructors (PADI) 5 Star Instructor Development Center (IDC). College credit is available for some HSU/PADI courses. In addition, at least one Instructor Trainer is on staff. A current list of instructors and their PADI teaching qualifications can be found in Appendix 5. Admission and Enrollment Eligibility Requirement Policy: Applicants should complete and return the Enrollment Agreement (See Appendix 3) and payment FIVE BUSINESS DAYS before the start date of a course or program. Tuition Assistance (TA) and Veterans Affairs (VA) students must submit certificate of eligibility to Hawaii Scuba University FIVE BUSINESS DAYS before the start day of the course or program. Exceptions may be granted by registrar at his/her descretion. Applicants must be 18 years of age or older. Students must be able to swim 400 yards in twelve minutes. Students must complete a HSU/PADI Medical Statement (Appendix 2) signed by physicians before beginning dive training. All students must show evidence that they are free from active tuberculosis prior to enrollment. 4

6 Description of HSU Facility: Hawaii Scuba University is located at 435 Ulrich Way, Building 2110, Schofield Barracks, HI HSU is a Professional Association of Diving Instructors (PADI) 5 Star Instructor Development Center. HSU classrooms are located on the first floor of building 2110 and can accommodate up to 12 students in each of two classrooms. The both classrooms are equipped with a complete multimedia educational system including interactive computer programs, audio-visual learning lesson guides and whiteboard. There is a dividing wall between the two classrooms that can be removed for larger gatherings. The dive center itself has a retail sales floor with full equipment required for professional divers. A wide variety of national equipment brands are for sale and rental for customers and students. Our facility also consists of a men s and women s bathroom, showers and lockers available at all time for our students. Pool facilities are located approximately 500 yards away at the Richardson pool. The largest pool in the state of Hawaii and the largest in the Morale Welfare and Recreation system of the US Army, there is ample room for training, including a diving well that allows for training depths of up to 15 ft. HSU has a fleet of vans for client transportation to and from classroom, pool and ocean training. Additional classroom space is available at 556 Heard Ave, Schofield Barracks, HI Liability: HSU takes every reasonable precaution to prevent students from sustaining injuries while on the premises and in training. HSU, however, assumes no responsibility for injuries that are sustained on HSU property or at HSU sponsored activities. All students are required to sign a liability waiver prior to participating in training. Tuitions and Fees: The Tuition, Activity Fees and Class Material Fees listed below are due before commencement of training. Class Material and Activity Fees are mandatory for each candidate to fulfill 5

7 certification requirements established by PADI (Professional Association of Dive Instructors) and include professional manuals, and necessary boat charters. All issued equipment and class materials are nonrefundable. HSU may offer courses that involve extra costs (for materials, facilities, transportation, etc.) other than the ones listed in the appendix these courses are outside of core programs and not required for school progression, and in case of students using GI Bill benefits, cannot be certified to the VA. See class schedule for program dates. Required equipment consists of: buoyancy compensation device, regulator, octopus, dive computer, wetsuit, fins, mask, boots, snorkel, gear bag, regulator bag, gear pockets on dive shorts for accessories, surface marker, spool, dive knife, compass, dive weights, dive light and scuba cylinders. Equipment will be issued prior to the commencement of HSU4000 (Divemaster) as required by the standards of the training organization PADI. See Appendix 4 for details of charges. Students should be aware that the PADI organization charges certain fees for membership and application processing, these fees are collected directly by the PADI organization and are therefore not under the control of the University. See Appendix 1 for a listing fees. 6

Rental fee for HSU9000 includes use of portable regulator tester for duration of course, the rental fee of $50 will not be reimbursed by the VA for students using GI Bill benefits and the fee will be an out of pocket cost.

9 WITHDRAWAL AND READMISSION: Students must notify HSU in writing of their intent to withdraw. Letters of intent may be delivered to HSU via US Mail, , or hand delivered. The last date of attendance will e considered the date of withdrawal. A student who stops attending class without an official withdrawal will receive a pro rata refund based on the last date of attendance. Pro-Rated Refund policy will apply to tuition paid when a student withdraws. PRO RATA REFUND POLICY: Students withdrawing prior to the first day of class will not be charged tuition. However, the student is obligated for the cost of books and equipment received if not returned in unused and un opened condition. Withdrawal following the commencement of class results in a refund equal to the unused portion of tuition. Pro rata calculation of tuition owed will be calculated using course hours completed divided by total course hours. The difference between this amount and the tuition amount paid will be refunded to the student. A refund will also be given following withdrawal, for any un-issued books, which were paid for as part of the course. Activity Fees are nonrefundable fees incurred for reserving boat dive charters during the course of study. Professional Diver Fees (Appendix 1) paid directly to PADI are due near completion of each class. One these fees are paid, they are non-refundable. Required equipment, once issued, is a non-refundable fee. Refunds will be sent to the student within 21 days after their withdrawal notice have been received. A veteran using GI Bill benefits may apply the following Veteran refund policy if it is more beneficial to the veteran. Veterans Refund Policy: Hawaii Scuba University Veterans Refund Policy complies with CFR In the event the veteran or eligible person fails to enter the course, withdraws, or is dismissed at any time prior to completion, any unused portion of tuition, fees, and other charges is refunded. Any amount in excess of $10 of the application/registration fee is subject to pro-ration. The amount charged will not exceed the exact pro-rata portion of total charges. The length of the completed portion of the course will be prorated over its total length, and the exact pro-ration will be determined by the ratio of the number of days of instruction completed by the student, to the total number 8

10 of instructional days in the course. Refunds are made within 40 days of the last date of the student s attendance. For reference the provisions of CFR regarding refund policies for non-accredited programs are as follows: 1 Registration Fee: An established registration fee in an amount not to exceed $10 need not be subject to prorating. Where the established registration fee is more than $10, the amount in excess of $10 will be subject to pro ration. 2. Breakage Fee: Where the school has a breakage fee, it may provide for the retention of only the exact amount of the breakage, with the remaining part, if any, to be refunded. 3.Consumable Instructional Supplies: Where the school makes a separate charge for consumable instructional supplies, as distinguished from laboratory fees, the exact amount of the charges for supplies consumed may be retained but any remaining part must be refunded. 4. Books, Supplies and Equipment: Where the veteran or eligible person purchases his books, supplies, and equipment from a bookstore or other source, and the cost of such items is separate and independent from the charge made by the school for tuition and fees, he may retain or dispose of such items at his own discretion. Where the school furnishes the books, supplies, and equipment, with the cost thereof included in the total charge payable to the school for the course, and the veteran or eligible person withdraws or is discontinued prior to the completion of the course, refund will be made in full for the amount of the charge for the unissued books, supplies, and equipment. Issued items may be disposed of at the discretion of the veteran or eligible person. 5. Tuition and Other Charges: Where the school either has or adopts an established policy for the refund of the unused portion of tuition, fees, and other charges subject to pro-ration, which is more favorable to the veteran or eligible person than the approximate prorata basis described above, such established policy will be applicable. Otherwise, the school may charge a sum, which does not vary more than 10% from the exact prorata portion of such tuition, fees, and other charges that, the length of the completed portion of the course bears to its total length. The exact pro-ration will be determined on the ratio of the number of instructional days in the course. 6. Prompt Refund: In the event that the veteran, spouse, surviving spouse or child fails to enter the course or withdraws or is discontinued at any time prior to the completion of the course, the unused portion of the tuition, fees, and other charges paid by the individual shall be refunded promptly. Any institution that fails to forward any refund due within 40 days after such a change in status, shall be deemed, prima facie, to have failed to make a prompt refund as required by this paragraph. Outstanding Financial Obligation Students who have outstanding financial obligations to HSU (tuition, fees, loans past due, rental fees, etc.) may be denied certification card, of an equivalent class rating, from an agency recognized by the Recreational Scuba Diving Training Council. The (RSDTC) is an internationally recognized agency that acts as a liaison for the SCUBA training agencies. Upon verification of certification, the Training Director will notify students and credit will be given. Academic Calendar While calendar is time based, student must meet performance requirements to achieve satisfactory progress. A schedule of courses is published prior to each registration periods and is available at HSU. Note: Due to the nature of our curriculum, the university does observe a 9

11 holiday schedule with the exception of a one week break that occurs during the week of Dec 25 th. In 2015 that week is Dec 21 st o Dec 27 th. The full academic calendar is published at the end of this catalog. The academic calendar lists Mondays as start dates. Even numbered terms (2, 4, etc.) meet on Monday and Wednesday from 6-9pm and Saturday and Sunday for 8 hours each day. Weekend diving schedule is typically 7am to 3pm, but can change due to tides, weather and boat availability. Odd numbered terms meet on Tuesday and Thursday 6pm to 9pm and weekends. Minimum Courses performance Requirements: Possible grades include P for Passing or F for Fail. Students are required to have a passing grade prior to progressing to the next course level. Passing Grade: In order to obtain a passing grade, students must successfully complete all performance requirements for the course of study. Attendance, completion of assignments, quiz and exam scores of 75% or higher, and satisfactory performance of all diving skills and tasks are necessary for a Passing Grade. Failing Grade: Student will fail the course if he does not meet the requirements to obtain a Passing Grade. Absences: Students are responsible to make arrangement with their instructor to make up missed classes. Programs are progressive, and while 2 absences are allowed, any absence that results in 3 or more absences or missed 20% or more of a class will result in failing the class. 3 or more tardies will be considered one absence. No absence excuses a student from making up a PADI course performance requirement. For students using GI Bill benefits students must arrange to make up all contact hours due to an absence before the end of the term. Illnesses: Students must make arrangements with the instructor to make up missed classes. If a student misses 3 class days or more than 20% of a class, he or she is encouraged to withdraw from the class and complete it in the next session, as any student missing 3 or more classes or more than 20% of the class cannot be given a passing grade. No absence excuses a student from making up a PADI course performance requirement. Leave: Students who need to take leave should withdraw from the class and complete it at a later session. Leaves of absence duty to activity duty commitment are common at Hawaii Scuba University. The registrar has leave of absence forms that become part of the student s 10

12 permanent file. Proper use of the Leave of Absence form prevents charges to students utilizing GI Bill benefits. Unsatisfactory Progress: Students who are making unsatisfactory progress due to failing grades, absences, or illness will be notified in writing that they are entering a one week probationary period, the student may choose to withdraw at this point. This document will be placed in the student file. During that week the student will be responsible for obtaining satisfactory grades or making up missed classes. At the end of the week the instructor for the course will determine if the student is able to continue the course of study. If the instructor determines that the student may not continue the course of study, they will be withdrawn and issued a refund based on the Pro Rata Refund Policy (see page 7). Students that withdraw due to unsatisfactory progress may reenter the program at the same level in a subsequent term (Hawaii Scuba University repeats the complete programs of study every 5 weeks with identical schedules). If a student was forced to withdraw due to difficulty with passing grades, they are encouraged to repeat portions of the curriculum. There are no tuition charges for repeating any portion of the curriculum within one year of withdrawal, activity fees may apply. Unsatisfactory conduct: Includes criminal activity during class time, endangering of fellow students or excessive disruptive behavior during class which prohibits others from learning. These behaviors will result in a failing grade and dismissal from the university. Students may appeal to the president of the university. The President's finding is final. Some training is conducted under the supervision of senior staff instructors and involves direct interaction with the scuba diving public. Students are required at all times to conduct themselves in a respectful, courteous and professional manner when interacting with students of their own. Inability to interact with students in an industry acceptable manner will be considered Unsatisfactory Conduct and will result in dismissal from the university. Program Graduation Requirements: To complete a program of study at HSU, students must pass all required classes within the program of study, pass all necessary exams and submit all required paperwork. In addition, students must clear of indebtedness to HSU and have paid appropriate fees (Appendix 1) to HSU. Within 21 days of successful completion Candidates will receive an internationally recognized Certification Document from PADI (Professional Association of Dive Instructors) for the course level completed. Prior Credit: Hawaii Scuba University utilizes the PADI system of continuing education. Students often enter the program with some level of diving experience. Prior credit will be issued to the student for certification earned before enrollment in Hawaii Scuba University, resulting in a reduction of 11

13 both length of program of study and a reduction in tuition and fees according to the fee schedule. Students will document their prior education using the Hawaii Scuba University Enrollment Agreement and that form will be retained in the student permanent record. See Appendix 3. Student Records: Student records and progress documentation will be maintained by HSU in their Student Record Folder. Records include Transcript, Timesheet, Prior Credit Records, Applications, Discipline Reports, Medical Statements, Knowledge Review, Quizzes, Exams and Evaluations. Student Record Folders and their contents are the property of HSU and copies can be obtained by the student via a transcript request. Students are required to log and track each of their dives in their personal log books. Transcripts: Students may request that official transcripts be sent to a third party or that they be given an unofficial transcript, upon presentation of a photo I.D. Students must pay the transcript fee of $25.00 and settle any other financial obligations with HSU before a transcript may be released. Job Placement: HSU staff is available to help students identify their career goals and provide leads, contacts, and search strategies for job placement. The student must actively engage in the search process to secure employment. Programs of Study HSU adheres to strict instructional standards as established by PADI. All prerequisites must be met by each individual candidate prior to enrolling in any HSU course. Prerequisites can be found in the class descriptions as published by PADI. In conjunction with successful completion of all required exams and instructional courses, certain Professional Levels have additional logged dive and PADI fee requirements for certification indicated below. For a description of vocational qualifications of each professional level, see the course descriptions. Students using GI Bill benefits should be aware that preparation courses, and other courses not listed above are not certifiable to the VA and the VA will not pay for them. 14

16 Course Catalog HSU 1000 OPENWATER DIVER COURSE (22 hrs: equally divided between theory and application (in the water) and conducted over a 1-2 week period). The Open Water Diver course teaches student divers the foundational knowledge and skills they need to dive with a buddy, independent of supervision. Pre: Age 12 HSU 2000 ADVENTURES IN DIVING COURSE (22 hrs: equally divided between theory and application (in the water) and conducted over a 2-3 day period). The Adventures in Diving program provides means to explore special diving interests and gain dive experience. The program allows student divers to broaden their awareness of the environment and their capabilities as divers. Pre: Minimum age of 12 and HSU 1000 or equivalent. HSU 2100 EMERGENCY FIRST RESPONSE COURSE (22 hrs equally divided between theory and application and conducted over a 1-2 day period). An introduction to cardiopulmonary resuscitation and emergency care from the perspective of the layperson. The course covers primary and secondary care and treatments including rescue breathing. CPR, choking management, bleeding management, shock management, and illness and injury assessment. Pre: None. HSU 2200 RESCUE DIVER COURSE (22 hrs equally divided between theory and application (in the water) and conducted over a one week period) this course is designed to develop the necessary knowledge and skills to effectively perform diver rescues and assists, manage diving-accident situations, and render proper first aid. Topics include self-rescue: diver stress; diving first aid: emergency management, and equipment considerations. Pre: Minimum age of 15 (12 Junior Rescue Diver Course) and HSU 2000 or equivalent, have completed a sanctioned course in CPR within the past two years, and have 20 logged dives. HSU 3030 ENRICHED AIR DIVER SPECIALTY COURSE (22 hrs equally divided between theory and application (in the water) and conducted over a 2-3 day period). This course is designed to qualify recreational divers to use enriched air nitrox for no decompression stop recreational diving. The program addresses the use of enriched air with 22 percent to 40 percent oxygen, with emphasis on enriched air with 32 percent and 36 percent oxygen. Topics include planning, organization procedures, techniques, problems, and the hazards of diving with enriched air, equipment and maintenance, considerations, and use of diving tables. Pre: Minimum age of 15 and HSU 1000 or equivalent. Required for MSDT. 15

17 HSU 3100 DIVEMASTER PREP COURSE [110 hrs approximately 30 hours theory and 80 hours of application (in the water) and conducted over a 5 week period] Course is designed to provide supervision for the 40 required dives that are prerequisite for entering the Divemaster program. This course will provide supervised scuba dives to reach that prerequisite. Important: Prior credit will be issued for this course for any student with proof of 40 logged dives. Professional Level Diver Course HSU 4000 DIVEMASTER COURSE [154 hrs approximately 40 hours of theory and 114 hours of application (in the water) and conducted over a 6-14 week period] This course includes an overview of the standards for the course, watermanship and skill assessment, knowledge development, and practical application. Topics include: dive planning; dive management and control; supervising students in training physics; physiology; equipment; skills and the environment, recreational dive planners: underwater mapmaking, and emergency plans. Pre: minimum age of 18; HSU 2000, or equivalent; HSU 2100, or equivalent, completion of HSU 2100 or equivalent within the past two years: have completed and logged at least 40 open-water dives, and be in good physical condition for diving and submit an approved medical-exam form for a diving medical examination conducted within the past year. This course provides the student with the skills, training and entry-level PADI certification that will allow the student to work in the recreational scuba diving industry leading certified divers in recreational dive experiences. HSU 4100 INSTRUCTOR PREPARATION COURSE [110 hrs approximately 30 hours theory and 80 hours of application (in the water) and conducted over a 5 week period] Course is designed to provide supervision for the 80 required dives that are prerequisite for the OWSI program. This course will provide supervised scuba dives to reach that prerequisite. Important: Prior credit will be issued for this course for any student with proof of 80 logged dives. Topics include: the diving industry; general standards and procedures; risk management; how to organize, prepare, and conduct SCUBA training; how to teach; continuing education, and the business of diving. HSU 6395 IDC STAFF INSTRUCTOR PREPARATION COURSE [330 hours approximately 90 hour theory and 240 hours application of over a 15 week period] Required for students with the Master Scuba Diver Training rating that have not certified 25 students in PADI programs. This course will provide the student with the minimum of 25 certifications required to advance to the IDC Staff program of study. Students will be directly supervised by senior Hawaii Scuba University staff while teaching PADI programs. In addition to directly issuing PADI certifications, the student will be introduced to other areas of dive shops operations, including social media marketing, point of sale operations with the dive center specific computer software, continuing education sales training, and equipment sales training all essential elements of success in the vocation of recreational scuba diving industry. Student to instructor ratio is limited and senior Hawaii Scuba University staff must dedicate significant temporal resources to this almost 4 month training program, therefore enrollment is extremely limited. Students intending to pursue this course of study should declare their intentions to the registrar well in advance of course start date. Important: Prior credit for this course may be obtained by showing proof of 25 PADI certifications issued. HSU 6400 IDC STAFF INSTRUCTOR COURSE [154 hrs with approximately 64 hours of theory and 64 hours of application (in the water) and conducted over a 7 week period]. This course is designed to train students to assist with the teaching of the HSU 5001 course, and to independently teach the HSU 5000 course. Topics include dive theory, the HSU 5000 and 5001 curriculum, the psychology of evaluation and counseling, and evaluation methods. Pre: Minimum age 18, and be certified as a PADI Instructor with the Master SCUBA Diver Trainer rating. Have issued 25 PADI Certifications. HSU 7000 EQUIPMENT REPAIR TECHNICIAN [110 hrs with approximately 22 hours theory, 68 hours practical application (lab bench) and 20 hours diving (in water). This course is designed to give the student a practical experience in the maintenance of scuba equipment in preparation for a working with a full service scuba repair center. Major topics will include: visual inspection and certification of scuba cylinders; full service of scuba regulators, BCDs, and computers; basic compressor maintenance and troubleshooting; overview of high pressure systems and tools required for an effective service center. 20

22 HSU 8000 REBREATHER INSTRUCTOR [264 hrs with approximately 84 hours theory, 180 hours practical application and dive training. This course is designed to teach the student to safely dive a rebreather unit, become proficient at its use in a professional setting, and finally to earn the instructor level rating for the user level on that unit. Finally, the student will teach that course under the direct supervision of an instructor trainer as the final requirement for the full instructor level rating on that unit. Every attempt is made to issue the student all reasonably anticipated equipment necessary to participate in this course, including rebreather consumables (see appendix 6). However, the student must acknowledge that rebreather diving is at the cutting edge of the diving industry, and that unanticipated expenses and repairs to the rebreather unit itself, and well as additional required consumables may be necessary to complete the course. For students using GI Bill benefits, additional charges are not reimbursable by the VA. Pre: Minimum age 18, and be certified as a PADI Master Scuba Diver Trainer with Deep Diver Specialty Instructor, HSU6315, and with HSU6300 Enriched Air Diver Specialty Instructor, and have a PADI Medical statement signed by a physician within the last six months. HSU 9000 SCUBA EQUIPMENT TECHNICIAN [110 hrs with approximately 22 hours theory, 88 hours practical application]. This course will enable the student to effectively engage in the repair and service of commonly used scuba equipment in the recreational dive industry. Though both knowledge development and practical application, the student will leave the course with the necessary skills and understanding to repair and service 1 st and 2 nd stage regulators, BCDs and high pressure valves. In addition the student will be trained to conduct visual inspection of scuba cylinders, as well and be introduced to the care and maintenance of a high pressure compressors. All of these skills are fundamental to the smooth operation of a recreational dive center and will well position the graduate for employment. Pre: 18 years of age. 21
