1 BAND are pleased to be a partner in Support First, a project run by Urban Outreach and funded from The Big Lottery Fund Help through Crisis Programme. The Project will provide holistic, intensive community-based support to individuals who have a range of complex social and lifestyle needs which are resulting in a disproportionate use/demand on emergency services. Other partners involved in Support First are Bolton Hospital NHS Foundation Trust, Greater Manchester West Mental Health NHS Foundation Trust, North West Ambulance Service and Greater Manchester Police. BAND will be offering a volunteer supported befriending and mentoring service for beneficiaries of the project who have a mental health condition as part of their presentation. BAND is looking for a person to deliver the befriending and mentoring service. The successful candidate will have the knowledge, skills and experience to build mutually respectful professional relationships with people who are referred to the service, all of whom will have multiple and/or complex needs. He or she will have experience of supporting volunteers and will have a commitment to and passion for supporting people to make meaningful and sustainable changes to their lives. 2 JOB DESCRIPTION Post Title: Grade: Hours of work: Reports to: Service setting: Job Purpose: Support First Mental Health Befriending and Mentoring Worker Based on NJC SCALE 5 point 22 ( per hour) 21 hours per week some early mornings, late evenings and weekend working may be required Operations Manager Based within the voluntary sector at the BAND head office As part of Urban Outreach s Support First Project, to deliver a Recovery Focussed Befriending and Mentoring Service to adults with multiple needs, one of which will be lived experience of a mental health condition, and who live in the Borough of Bolton. Key Tasks: To work in partnership with Urban Outreach, in particular with their Support Workers, to receive referrals into the Befriending and Mentoring Service. To ensure the delivery of a person-centred service to Support First beneficiaries, to include an assessment which listens to and fully understands the needs, challenges and barriers for people living with multiple needs presentations.

3 To work with the beneficiary to develop and regularly review a personalised action plan which is specific to their needs. Using a flexible approach, offer and encourage the uptake of brief interventions, such node link mapping, CBT based life skills learning and problem solving to support, motivate and sustain movement towards agreed outcomes. To promote and recruit to volunteer peer befriender and mentor opportunities to work within the project to work alongside beneficiaries to offer practical help and support to put their plan into action. To participate in the delivery of BAND s in-house volunteering training package, and to arrange for the delivery of additional external training as required. To provide line management support and supervision to volunteers engaged in the project. To liaise directly with other services, organisations and teams that beneficiaries may be known to and to work collaboratively with them to develop a joint response as part of the planning process To keep clear records in line with the organisations data collection, monitoring, evaluating and recording processes. General Duties To ensure that the project is delivered in line with all organisational Policies and Procedures including but not limited to Boundaries, Confidentiality, Safeguarding Policies, Health and Safety and Data Protection. To manage the workload and to bring to the attention of the line manager any instances where difficulties are encountered
