2 Summary About the service Glyn Cynffig is registered to provide a rehabilitation service for up to 16 people who have mental health problems or a dual diagnosis of mental health and substance or alcohol misuse. The service aims to maximise independence and community integration, prevent unnecessary admissions to hospital and to provide a respite service. Glyn Cynffig is located in Kenfig Hill near to community facilities. The accommodation consists of a single person flat, bedsits and shared flats. The registered provider is Bridgend County Borough Council. The responsible individual is Susan Cooper and the registered manager is Christopher Mogg. What type of inspection was carried out? We, Care and Social Services Inspectorate Wales (CSSIW) carried out a scheduled, unannounced focused inspection on 15th January The focus of the inspection was on the quality of life for people using the service. During the inspection we carried out a brief visual tour of the premises to see how the environment impacts on the quality of life of people receiving support from Glyn Cynffig. We looked at a sample of documentation and care files, chosen at random, spoke with service users, the registered manager and a visiting professional. We did not consider using the short observational framework for inspection (SOFI). This is because people were able to tell us how they felt about the service provided by Glyn Cynffig. What does the service do well? The agency is well run and people receive a good level of support during their rehabilitation. What has improved since the last inspection? This inspection identified that there were no significant areas of improvement since the last inspection. What needs to be done to improve the service? There were no areas of non compliance. Page 2

3 Quality Of Life Overall, we found that people who use the service receive good and consistent care provided by an agency whose staff are committed to the provision of high quality care. The registered person is committed to continuous improvement via consultation with service users and other relevant individuals. Customer service satisfaction questionnaires had been completed as part of an annual review of the quality of the service. The latest quality of care report was available and very detailed. We examined the records of three of the people who use the service. We found that care plans are reviewed at least monthly or as often as necessary to reflect an up to date overview of assessed care needs. This enables care staff to provide care in the most appropriate way, taking into account people s likes and preferences. Service delivery plans were detailed and person centred and we found evidence of people using the service being involved in care planning and reviews. The files also contained risk assessments and the documents had been reviewed regularly. The support plans incorporated personal choice and we saw that people were supported to maintain links with other healthcare professionals and support groups. The support plans encouraged independence and assisted people to reach their potential. We spoke with a community psychiatric nurse who was visiting the service and we were told you can t fault the staff and that there is always a good level of communication and that they are kept fully informed of any changes in a persons needs. Overall we found that people using the service were satisfied with the support that they were receiving and felt they were treated with dignity and respect by the staff. We saw that staff knew people using the service well and were able to provide appropriate and responsive care and understood their individual needs and preferences. One person told us that staff recognise when they are feeling low and that they always provide support at any time. We saw that rehabilitation was integral to the support people using the service received. We saw that individuals were encouraged to establish or re-establish their social support networks. People were supported to undertake educational and recreational activities which included educational courses, voluntary work, sports and music events. We also saw that people were supported to spend time away from the hostel with their family and friends to assist in maintaining relationships. We saw that people were supported to manage their financial affairs. We did not look in detail at medication issues at this inspection but we have had no concerns raised with us about medication procedures and we saw that medication was stored securely and appropriately and medication administration records were completed correctly. We also saw evidence that staff had undertaken medication competencies. People were encouraged to make healthy choices with regards to diet and staff assisted Page 3

4 individuals to improve their cookery skills. People are also supported in all aspects of independent living skills including shopping, finances and domestic chores. We saw that staff were kind and caring, and provided emotional support as well as physical support. We spoke with several people residing at the hostel and they were complimentary about the service and the staff, and about the support they had received to assist them with their recovery. We were informed that a number of ex-service users maintain contact with the hostel and regularly visit, also using Glyn Cynffig as a point of contact during crises. We were told by people that use the service:- until I came here I have never felt loved or needed The staff have a caring heart and they actually care about you It is 100% amazing here We are treated like adults and there is a mutual respect haven t got a bad word to say about anyone Overall the outcome for the people using the service was that they received good quality care from dedicated and competent staff. We found that the agency provided a high standard of care which effectively assisted people to maintain and, where possible enhance their level of independence or wellbeing. The service as a whole demonstrated that it is reliable and dependable. Page 4

5 Quality Of Staffing This inspection focussed only on the Quality of Life. CSSIW did not consider it necessary to look at the Quality of Staffing in detail on this occasion. This theme will be considered in more detail in future inspections. However, we noted that people using the service are supported by a motivated staff team with a low turnover of staff, which means that there is good continuity of care. People enjoy being cared for by staff who are appreciated and want to make a difference to people s lives. People we spoke to talked of how friendly and helpful staff were. On our visit we could see that there were sufficient staff to be able to support people and we saw evidence of really positive relationships between people using the service and the staff caring for them. 6 Quality Of Leadership and Management This inspection focussed only on the Quality of Life. CSSIW did not consider it necessary to look at the Quality of Leadership and Management in detail on this occasion. This theme will be considered in more detail in future inspections. However, we noted that a range of information is available to people considering using the service. There is a statement of purpose, which is a document that sets out information about the agency combined with a service users guide. The guide was easy to read and informative. This document ensures that people who are seeking care from the agency are clear in what the agency can and cannot provide. Page 6

7 Quality Of The Environment The quality of the environment is not relevant in relation to domiciliary care other than that there are appropriate office facilities. We can confirm that the registered office is appropriate for the needs of the service and that there are lockable cabinets for the safe and confidential storage of care files and staff information. 8 How we inspect and report on services We conduct two types of inspection; baseline and focussed. Both consider the experience of people using services. Baseline inspections assess whether the registration of a service is justified and whether the conditions of registration are appropriate. For most services, we carry out these inspections every three years. Exceptions are registered child minders, out of school care, sessional care, crèches and open access provision, which are every four years. At these inspections we check whether the service has a clear, effective Statement of Purpose and whether the service delivers on the commitments set out in its Statement of Purpose. In assessing whether registration is justified inspectors check that the service can demonstrate a history of compliance with regulations. Focused inspections consider the experience of people using services and we will look at compliance with regulations when poor outcomes for people using services are identified. We carry out these inspections in between baseline inspections. Focussed inspections will always consider the quality of life of people using services and may look at other areas. Baseline and focused inspections may be scheduled or carried out in response to concerns. Inspectors use a variety of methods to gather information during inspections. These may include; Talking with people who use services and their representatives Talking to staff and the manager Looking at documentation Observation of staff interactions with people and of the environment Comments made within questionnaires returned from people who use services, staff and health and social care professionals We inspect and report our findings under Quality Themes.
