3 Hot Weather Drink lots of water Avoid accessing tank during hottest part of day
4 Cold Weather Wear layered clothing Seek shelter from wind as much as possible
5 Ladder Safety Devices Gather any slides or sleeves available for use with ladder safety device
6 Attendant Have an attendant or ground person on crew Never climb a tank alone Establish communication procedures
7 Emergency Information Gather emergency information such as: Law enforcement, fire department, and EMT phone numbers Directions to the nearest hospital Review emergency information with crew Post emergency contacts and procedures
8 Lockout/Tagout Prior to access, lockout or tagout: Tank valves Cathodic protection boxes Electrical equipment According to employer procedures
9 Safety Considerations 1. Prior to Access 2. Personal Protective Equipment (PPE) and Personal Fall Arrest System (PFAS):
10 Hard Hats Wear if overhead or bump hazards could occur. Inspect outer shell and suspension prior to use. Replace when cracks are noted in outer shell and/or suspension is no longer taut.
11 Eye Protection Wear to protect eyes from airborne objects, dusts, or debris. Inspect prior to use. Replace when scratched or damaged.
12 Harness and Lanyard Wear at all times when accessing tank ladders and elevated platforms.

18 Lanyards Must limit free fall to 6 ft or less. Inspect prior to EACH USE: Hooks Straps Stitching If lanyard has been used to arrest a fall, dispose of and replace. DO NOT USE IF DEFECTS ARE NOTED.

39 Platform Hazards Trip hazards. Safety rails are not OSHA compliant. No access through safety railing. Unprotected access openings. Cellular antennas that could not be de- energized. Corrosion/potential structural damage.

41 Lead-Based Paints If intact, lead paint poses little health hazard. If damaged, lead paint chips can be inhaled or ingested and pose potential health problems.
42 Lead-Based Paint Precautions Have designated site clothing and gloves. Wear if chipping, grinding, etc., any LBP. Remove before leaving site. Launder separately from other clothing. Wash hands with soap and clean potable water before smoking, eating, or drinking.
43 Asbestos Asbestos-containing material can be present in insulation materials.
44 Asbestos Intact asbestos-containing materials present little hazard.
45 Asbestos If disturbed or damaged, asbestos-containing materials release fibers which can be inhaled.
46 Asbestos Precautions If disturbing any asbestos-containing insulation or working around any friable asbestos-containing material, wet the material and wear a respirator with HEPA filters.* * Only if received medical clearance, had respirator training, and had a respirator fit test in accordance with employer s respirator plan.

51 Confined Spaces Permit Required (PRCS) Non-Permit Required (NPRCS)
52 Permit Required Confined Spaces Have a hazardous atmosphere. Contain materials that can engulf entrant. Have an internal configuration that could trap entrant. Possess any other recognized safety or health hazard. Have more OSHA requirements than NPRCS. Require use of a permit detailing specific information.
53 Non-Hazardous Atmosphere Parameter Limit Oxygen Level 19.5% % Lower Explosive Limit (LEL) < 10% Any Other Recognized Hazardous Material < PEL, REL, TLV, etc.
54 Non-Permit Required Confined Space Does NOT contain, or have the potential to contain, any hazard capable of causing death or serious injury.
55 Reclassifying a PRCS as a NPRCS Per OSHA (c)(7): If the space poses no actual or potential atmospheric hazards If all hazards within the space are eliminated without entry into the space
