Resident Births: Number of infants born to women who were Florida residents. 2. Resident Birth Rate per 1,000 Population (Only Available for Year): Number of resident births per 1,000 population. 3. Resident Fertility Rate per 1,000 Females (Only Available for Year): Number of resident births per 1,000 females who are years old. 4. Consented to Screen: Number of infants screened for Healthy Start. 5. Percent Consenting to Screen: Number of infants screened for Healthy Start divided by births to Florida residents. 6. Screen Consent is Yes: Number of infants who consented to screening for the Healthy Start Program. 7. Percent Screen Consent is Yes: Number of infants who consented to screening for the Healthy Start Program divided by births to Florida residents. 8. Resident Neonatal Deaths: Number of infants less than 28 days old who died in Florida. 53. Resident Neonatal Death Rate: Number of infants born to mothers who were Florida residents divided by the number of infants who were Florida residents and less than 28 days old when they died. 54. Resident Postneonatal Deaths: Number of infants 28 days old or older but less than 1 year old who died in Florida. 55. Resident Postneonatal Death Rate: Number of infants born to mothers who were Florida residents divided by the number of infants who were Florida residents and 28 days old or older but less than 1 year old when they died. Rows, Columns, and Filters: 1. Fiscal Year (Use with Counts): The fiscal year starts July 1 st of each year and ends on June 30 th of each year. This dimension will provide counts for the entire fiscal year or it can be expanded to show counts for each quarter, month, or day. The Fiscal Year dimension cannot be used with the Resident Birth Rate per 1,000 Population or Resident Fertility Rate per 1,000 Females measures. When the Fiscal Year is selected, then the Calendar Year, Contract Year, and Year dimensions are disabled. 2. Calendar Year (Use with Counts): The calendar year starts January 1 st of each year and ends on December 31 st of each year. This dimension will provide counts for the entire calendar year or it can be expanded to show counts for each quarter, month, or day. The Calendar Year dimension cannot be used with the Resident Birth Rate per 1,000 Population or Resident Fertility Rate per 1,000 Females measures. When the Calendar Year is selected, then the Fiscal Year, Contract Year, and Year dimensions are disabled. 3. Contract Year (Use with Counts): The contract year starts October 1 st of each year and ends on September 30 th of each year. This dimension will provide counts for the entire contract year or it can be expanded to show counts for each quarter, month, or day. The Contract Year dimension cannot be used with the Resident Birth Rate per 1,000 Population or Resident Fertility Rate per 1,000 Females measures. When the Contract Year is selected, then the Fiscal Year, Calendar Year, and Year dimensions are disabled. 4. Year (Use with Counts or Rates): The year starts January 1 st of each year and ends on December 31 st of each year. This dimension will provide counts or rates for the entire calendar year. The Year dimension can be used to display any count or rate in the Healthy Start Infant Screening Ad Hoc Reporting Tool. 3

4 When the Year is selected, then the Fiscal Year, Calendar Year, and Contract Year dimensions are disabled. 5. Healthy Start Coalition: This dimension displays infant screening reports by Healthy Start Coalition. For each record the Healthy Start Coalition is based on where the infant s mother resides. 6. Residence County : This dimension displays infant screening reports by county of residence. For each record the county of residence is based on where the infant s mother resides. 7. Residence Zip Code: Starting in 2004, this dimension displays infant screening reports by mother s zip code of residence. Before 2004, this dimension displays infant screening reports by mother s mailing zip code. 8. Residence Census Tract: Starting in 2004, this dimension displays infant screening reports by mother s residence census tract. Before 2004, this dimension displays infant screening reports by mother s mailing census tract. 9. Occurrence County: This dimension displays infant screening reports by the county where the birth occurred, regardless of mother s residence address. 10. Birth Facility Type: This dimension displays the type of facility where the birth occurred. The facility types are: Hospital Freestanding birth center Clinic or doctor office Home birth En route or other place 11. Birth Facility: This dimension displays the specific birth facility by name where the birth occurred. 12. Birth Hospital is Level III: This dimension indicates if births occurred in a hospital with a Neonatal Intensive Care Unit with Level III beds. Yes No 13. Birth Attendant: This dimension displays the credentials of the birth attendant. The birth attendant types are: MD DO Certified Nurse Midwife (CNM) Licensed Midwife (LM) Other 14. Healthy Start Screen Consent: This dimension indicates if the parents/guardians of infants consented to the Healthy Start infant screen. The responses for this dimension are: Yes No Not Applicable 15. Healthy Start Program Consent: This dimension indicates if the parents/guardians of infants consented to the Healthy Start program. The responses for this dimension are: 4

5 Yes No Not Applicable 16. Healthy Start Score: This dimension shows the Healthy Start score. Scores of 4 or more show that an infant is at-risk of experiencing postneonatal death. <4 (Expands to show individual scores of 0, 1, 2, 3, and 4) 4 or more (Expands to show individual scores of 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 15, 16, 17, and 18) Declined HS Screen Not Applicable 17. Mother s Age: This dimension shows the mother s age by age groups.
