1 Guidelines to Specification Sheet for Lifts for Persons with Physical Disabilities Form ED General Guidelines 1. Application of the form. This form may be used for: (a) Elevating devices classes (as defined in the Regulation): (i) Stair-chair lift (ii) Vertical platform lift Type C (iii) Vertical platform lift Type D (iv) Stair platform lift Type C (v) Stair platform lift Type D; and for Notes: (b) Types of design submission: (i) New installation ( not based on a previously registered standard design), (ii) Major alteration ( not based on a previously registered standard design), or (iii) Standard design -For classes of elevating devices other than those specified in (a) above, request other forms. -For a design submission of the following types: (i) New installation or major alteration based on a standard design, (ii) Revision to a design submission for a new installation or a major alteration, (iii) Revision to a registered standard design, (iv) Minor type A alteration, use form ED Specification Sheet for all Elevating devices (Abridged form). -For a Minor type B alteration use form ED Notification for Minor Type B Alteration. 2. Design Submission. A design submission, when submitted to the Ministry for registration, must include: (a) Application on form No. ED (b) This specification sheet (c) Drawings prepared in accordance with items of these guidelines, (d) Supplementary report on form No. ED 09088, and (e) Other documents if requested by the technical Standards and Safety Authority. All documents, forming a design submission, must be signed, dated and sealed by a professional engineer registered in the Province of Ontario, except that the application may be signed by and officer of the submitter s company. Any submitter interested in using an electronic means to substitute the use of the forms when applying for the registration of a design submission, should contact this office. 3. Copies. All documents composing a design submission must be submitted in four copies, or in the case of a standard design submission, in two copies. Photo-copies are acceptable, but at least in 1 set of documents all requested signatures and stamps must be original and at least two sets must include colour-coded forms. 2 4. Entries and Blanks in the Form. All blanks must be filled-in, except that in the case of stair lifts shaded spaces may remain blank and also in the case of standard design spaces specified in guideline 8 may remain blank. Entry N/A shall be used where an item is not applicable. Where the provided blanks cannot accommodate all data, additional pages may be enclosed, bearing reference numbers of respective blanks. The entries in this specification are legally binding and shall prevail in the case of discrepancy with any other documents in the submission. Please note that it is an offence to knowingly, make a false statement in any document required by Ontario s Elevating Devices Act and the Regulation. 5. Units & Abbreviation in the Form. Numbers in brackets, e.g. (3.3.3) or (T1) mean a clause or table in CSA standard B3555 to which a designer should refer for clarification of the terminology used, or of the expected entry. All entries shall be expressed in metric units in accordance with CSA Standard CAN3-Z324, 1-7_ Canadian Metric Practice Guide. Where model of a part is required to be specified, it will mean any designation: e.g. name, number, etc., which is specific to that particular model and which enables any person and the original manufacturer to identify the part. 6. Additional Specification Data. This specification requires only data on a few common components and characteristics for an elevating device. However, the submitting engineer should, if he/she considers it necessary, forward additional specification data to the manufacturer or submitter in order to ensure that the balance for the parts and features of the elevating device type, mandatorily required by the code, will be fulfilled. 7. Major Alteration. In the case of design submission for major alteration, the specification and drawings may be limited to the scope of the proposed alteration so that whole sections may be omitted. 8. Standard Design. For Standard design Submissions, related details and procedures, please refer appropriate Director s Ruling. This ruling also contains instructions for new submissions or alterations based on standard design. 9. Lift location and fire protection. The engineer who signs this specification must assure himself/herself that the design of the runway enclosure and obstruction created by stair chair or platform, as the case may be, are acceptable to the authorities governing fire resistivity and egresss. 3 Guidelines to the Form ED Three possible types of the design submission, for which this form may be used, are listed in general guideline 1(b), above. Where a Major Alteration is proposed, the installation number for the lift being altered must also be indicated in line 11, e.g. major alteration of Installation # Submitter s specification number may be job or contract number or any manufacturer s or submitter s number unique to that particular installation. 13. State one of the classes listed in guideline 1 (a). 14. State one of the following: one or two chairs, standing platform, wheelchair platform, wheelchairand attendant platform, special adaptation (see clause 3.4 and Section 6). 16/ Specify rated load, and number of persons and wheel chairs, that the device is designed for. State N/A unless lift manufacturer has a specific designation for the lift model as covered in this specification. 21. The address of the building or premises in which the proposed lift will be located. If the full address is not know, describe the location as exactly as possible (i.e. SW corner of A Street and B Avenue intersection). At least the first three digits of the postal code should be provided in the full postal code is not available at the time of application. 23. Number of levels served by the lift. 26. Travel means the vertical distance between the bottom and the top limits of travel. 30. E.g. suspension rope and drum, or rack and pinion, or chain and chain sprocket, or screw and nut or hydraulic direct plunger, or hydraulic with suspension rope, or rope chain and rope sprocket, or (see 5.1) 31. Specify details; e.g. driving drum or sheave or sprocket diameter, number, diameter/size and breaking strength of ropes or chains; number of teeth on sprockets; type of gearing; brake type; maximum allowable pressure; relief valve setting, etc. If you need more room to describe everything, refer to box #189 or to specific dwg. No 36. Give brief description; e.g., hand-winding wheel at the motor; or lowering valve etc., or state not required if not required by the code. 37/ For entries in these boxes use the same designations as marked in the attached electric schematics This item refers to the portion of the runway enclosure between the bottom and top landing, but excluding landing doors and the top landing enclosure. Specify fully if that portion of the runway is fully enclosed ( type C Vertical Platform Lift), or partially if enclosed in accordance with clauses ( type D Vertical) or ( type C stair platform lift), or none if no runway enclosure is provided (e.g., on type D stair platform lift). 41. This item refers to top landing enclosure, but excluding the top landing gate. Specify fully if the enclosure extends from the landing floor to the building ceiling or partially if not fully enclosed, but the enclosure conforms to ( permitted on any lift) or none if no enclosure is provided ( e.g., on type D stair platform lift or on type D vertical platform lift having travel less that 500mm). 42. Clause (b) is applicable to type C vertical platform lifts only. For that lift type, state Yes or No. for other lift classes, state N/A. 43. Give a general description; e.g., painted concrete blocks, or aluminum and glass, or painted plastered bricks, etc. 44. State Yes or No on the basis of requirements in CL / Landing doors or gates are required on every type C vertical or stair platform lifts, and on type 51 vertical platform lifts, if travel exceeds 500mm. For other devices where no doors are provided, state N/A / Examples for door material: Metal, perforated metal with toe board, glass and metal 51.3 frame, etc. 50.4/ Fire rating for landing doors must comply with table 3.5.3A of Ontario building code. If fire 51.4 resistive construction not require, state N/A. 52. Only doors of type C vertical platform lifts located more than 2.5m above the bottom landing must have interlocks. Entry may be: all landings, or the top landing, or None or N/A where no doors required. 53. E.g. all landings, or bottom landing, or none, or N/A where no doors are required. 5 52.2/ Specify file number allocated by the CSA testing laboratory at the time the interlock or 53.2 mechanical lock and electric contact, as specified in 52.1 or 53.1, was certified in accordance with clause or B Indicate material, number and size of guide rails or N/A where the guide rails are not required the design or clause For any stair lift, specify provided or not required. For vertical platform lifts, enter N/A. 62. Specify Zero where no pit is provided. 63. Give a general description; e.g. a foldable steel channel affixed to platform.or state not required (see clause 5.4). 70. For stair-chair lifts and for type D platform lifts, state Yes or No depending whether the carriage as a whole can be folded. ( This item does not refer to parts such as armrests, handgrips, footrests ). For other lifts, state N/A. 71. Specify location; e.g. at footrest (in the case of stair-chair lift), or under the platform ( for a type d platform lift), or not provided ( if not required). For type C vertical platform lift, state N/A. 72. On type C vertical platform lifts with travel over 2.5m, the entry may be type A-governor operated where type A will be as defined in CSA standard B44. For direct plunger hydraulic lifts, the entry may be not required. If alternative devices are provided, give details to justify the stopping & holding capabilities of the alternate devices, which must provide and equivalent degree of safety as specified in clause State Yes or No. If Yes, submit the certificate produced in accordance with appendix A (of B355) and stamped and signed by a professional engineer registered in the Province of Ontario. Entering No will mean that the safety device is so designed that it will be possible to test it (with overload under overspeeeding conditions per clause ) during the inspection of the lift, in this case, include field procedure. 81. State Yes or No. 82. Specify minimum clearance between the platform and building structure, or between the platform and platform ceiling if provided. 83. State Yes or No. 84. Describe method of conformance to clause for type C vertical platform lifts. For all lifts with travel of 500mm or less, state N/A. In column (a) indicate the height of the platform surface above the floor level; e.g. 150mm, or Zero if the platform is designed to stop level with the floor level (see for standing platforms for wheelchair platform, for wheelchair-and attendant platforms). In column (b) indicate ramp inclination; e.g., 1:4, or 1:6 (per ). for standing platform, no ramp is required, so the entry may be N/A. For any platform the entry will be N/A if the vertical clearance in column 9(a) is Zero. (Ramps are required in and 7.7.4). In column (c) the entry will be typically 15mm or less (see 7.5.2)for standing platform, for wheelchair platform 7.7.4(b) & (c) for wheelchair-and-attendant platform. However, it may be more than 15mm (see ). In column (d) state Yes or No. the flap must be provided on each platform, if the clearance in column (c) is more than 15mm ( ). 90. E.g., platform only, or landings only, or chair only 90.1 Enter CP for continuous pressure type or CP DF for continuous pressure with pre-determined floor stops. 91. E.g., platform and landings, or platform only, or chair only, or landings only. 92. State provided or not required. 93. State provided or N/A if provided, leveling zone must be marked, and a visual indicator provided. For detail requirement associated with automatic leveling, see * Measure and enter the time elapsed between the time that the landing door is closed and the time that a landing operating device can be activated. Verify conformance to Clause Measure and enter the time elapsed between the time that the carriage operating device is turned to the OFF position and the time that the landing operating device can be activated. Verify conformance to Clause Use specific legend for each individual switch as marked in the attached schematic diagram or 111 state N/A where the switch is not provided (a) In column critical components identify each magnetically operated switch, contactor and relay, failure of which would: (i) render an electrical protective device (listed above) ineffective, or (ii) permit the car to move beyond the leveling with open door. (b) In column redundant critical components identify means provided to eliminate the above hazards should the critical components fail. (c) In column Testing method describe method of verifying in the field the provided means of protection in the case of failures, of critical and redundant critical component, one failure at a time (a) In column critical components identify each static control device, failure of which would cause hazards (i) and (ii) mentioned in guideline 112.1(a). (b) In column redundant critical components identify the means provided to eliminate the above hazard should the solid state component fail. 7 (c) See guideline (c) (a) In column critical components identify each leveling switch, failure of which would cause hazard mentioned in guideline 112.1, 2 (ii) (b) In column redundant critical components identify the means provided to eliminate the above hazard should the leveling switch fail. (c) See guideline (c) In column redundant critical component identify means provided to eliminate hazard mentioned in guideline in case of the occurrence of a single accidental ground. In column Testing method describe method of verifying of the provided protection against accidental ground that may be used at the time of inspection By entering Yes, confirm that the control circuits incorporating all critical and back-up components are so arranged that the lift will not be permitted to restart normal operation after the failure of any single component or accidental ground has occurred This space is provided either for description of special features of the device, or for outlining the scope of a major alteration. 190 Where a variance from applicable codes and regulations is proposed, it must be delineated in this section. The variance proposal must include the following key components: -identification of issue and relevant code/regulation clause; -outline of the reason of necessary deviance; -to recommend alternate safety compensation if deemed appropriate; -professional assessment on safety of the installation attributable to this variance Otherwise, state NO VARIANCE PROPOSED 191 The list may include drawing numbers only. 192 Under and 2, specify the edition or latest supplement which is enforced at the time of signing of this design submission. All other standards referenced in the codes mentioned in this item are considered to have been applied in this specification. Any additional code and /or Director s ruling(s), if applied, shall be specified in item In making this statement, the professional engineer may rely on the opinion of or information obtained from other professional engineer or an architect, and he/she may indicate, in the specification and drawings, in what respects he/she is relying. If and engineer wishes to indicate so, he/she will answer item 194 with yes and attach an amendment to the Professional Engineer Statement (on a blank paper) indicating in what respects he/she is so relying on another engineer or an architect Refer to guideline 193. 8 Guidelines to Drawings 201. General. Drawings composing a design submission referred to in guidelines shall; 1. Be identified by a number or other designation unique to that drawing; 2. Bear a date (of completion, last revision or approval); 3. Set out the address of the building or premises where the lift will be installed ( see item 21 in this form); 4. Set out the elevating device class, rated load and rated speed (see items 13,16 and 18 in the form); 5. Include layout, plan and elevation views of the elevating device and/or parts thereof, showing all pertinent information, as specified in guideline 202, and other information necessary to demonstrate conformance with Regulations and applied codes; 6. Include electric schematic diagrams, as specified in guideline 203; 7. Bear the signature and seal with date of the professional engineer referred to in guidelines 2 and 193; 8. Include a legend or reference to a relevant code for all symbols used in the drawings; 9. Be prepared in accordance with good engineering and drafting practices, and 10. Be accurate and complete General Layout, Plan and Elevation Views. General layout, plan and elevation view of the lift shall include but shall not be limited to, the following; 1. General location of the lift in relation to the adjacent building components (stairs, corridors, entrances, walls, etc.) Also see guideline Driving machine: location of all major machine components including control cabinet; enclosure; access to the machine; and where applicable, machine room enclosures, access door, clearances around machinery, etc. 3. Runway: location and dimensions of enclosure, entrances, landings; also equipment in the runway, such as guiderails, ceiling intersection guards, runway pit, platform supporting device counterweight, cylinder, sheaves, mechanical and electrical limits, etc. 4. Carriage: location and dimensions of the carriage (chair or platform), its enclosure, guards, barriers, gates, safety flaps, foldable seats, ceiling, etc. 5. Clearances: vertically, above the carriage or above the platform ceiling, and below the carriage; horizontally, between the platform sill and landing sills or runway enclosure, between sides of the carriage and the runway enclosure or any obstruction in the runway. 6. 9 203. Electric Schematic Diagrams shall include, but not be limited to, the following; 1. All protective contacts and switches referred to in items 100 to 111, with sequence of their operation identified. 2. Control of power supply to the drive machine and its brake by means of protective contacts and switches; 3. Accidental ground protection.
