2 Eligible / Target Organization. This program is designed for the department of the central and the local governments in charge of communicable diseases control. Course Objective. From the prospective of prevention and health education in communicable diseases, participants are expected to recognize current problems, and an improvement plan will be formulated and proposed in their own organization. Expected Module Output and Contents. This program consists of the following components. Details on each component are given below: (1) Preliminary Phase in a participant s home country (July to August 2015) Participating organizations make required preparation for the Program in the respective country. Modules Job and Country Report Activities Formulation of the Job Reports on participant s present job and the outline of administration on communicable disease control in participant s country in PPT format. (2) Core Phase in Japan (August 19, 2015 to September 9,2015) Participants dispatched by the organizations attend the Program implemented in Japan. Modules Subjects/Agendas Methodology To share information about present situation and challenge of communicable disease control among participants. Explanation and discussion on Job Reports (Situation analysis in participants duty and/or country/region) - Participant s presentation - Discussion

3 To understand historical background, policy and measures, administrative system and current situation for communicable disease control in Japan, and especially taking the case of Nagasaki Prefecture. To find out the issue of emerging and re- emerging infectious diseases Neglected Tropical Diseases(NTDS) Utilize the knowledge and techniques gained during the training to be able to create draft plans for health service improvement plans that are effective and practical. - Administration on communicable disease control in Japan. - International cooperation in the field of communicable disease control - Health administration system and historical background on Japan - Recent trends in communicable diseases - Communicable disease surveillance system - Environmental health - Communicable disease control at the Hospital - Tuberculosis Program - Malaria and other insect vector borne viral communicable diseases - Preventive control of Neglected Tropical Diseases - Study visit at healthcare Participants will develop an effective and practical plan to improve communicable disease control - Lecture - Participants presentation - Facilities Observation/ Visit Lecture Discussion (3) Finalization Phase in a participant s home country (September 2015 to March 2016) Participating organizations produce final outputs by making use of results brought back by participants. This phase marks the end of the Program. Modules Activities Implementation of the Action Plan Application and implementation of the action plan black in the participant s country and submission of its final report by March Nominee Qualifications. Applying Organizations are expected to select nominees who meet the following qualifications.

4 (1) Essential Qualifications. 1) Current Duties: - Be a governmental official engaged in the planning and operation of communicable diseases control - Working at national level or at least regional level 2) Experience in the relevant field: - Have more than 3 years experience in the field of public health - Will continue to work in the field of communicable diseases control at least 3 years 3) Language: have a competent command of spoken and written English 4) Health: must be in good health, both physically and mentally, to participate in the Program in Japan 5) Has not participated in this program in the past 6) Not to be serving any form of military services Procedures for Application and Selection. (1) Submission of the Application Documents: Closing date for applications, the JICA office (or the Embassy of Japan) will send them to the JICA Center in Japan by JUNE 10, 2015 Travel to Japan. (1) Air Ticket: The cost of a round-trip ticket between an international airport designated by JICA and Japan will be borne by JICA. (2) Travel Insurance: Coverage is from time of arrival up to departure in Japan. Thus traveling time outside Japan will not be covered. Expenses. The following expenses will be provided for the participants by JICA: 1) Allowances for accommodation, meals, living expenses, outfit, and shipping. 2) Expenses for study tours (basically in the form if train tickets.) 3) Free medical care of participants who become ill after arriving in Japan (costs related to pre-existing illness, pregnancy, or dental treatment are not included) 4) Expenses for program implementation, including materials

The Regional Office on the exterior of Panama, have to present the entire necessary document 3 days before the closing date in the Office Panama- Central. IMPORTANT REMARKS For better filling of application form for scholarships JICA ok with missing or filled forms will be received. 1. The form to be in a predetermined format is written first draft (red letter) before printing the candidate must activate the command to finish editing the format for the letter change to black. A printed format can not receive draft. 2. The first signature box on the form page which indicates the name of the applicant is to sign the highest authority of the institution not the applicant. 3. The last picture is only for institutions that depend on another agency or ministry, so that the signature of both authorities is required.

The medical history and all the data requested in the application form are important when selecting the candidate in Japan so it must be completed in its entirety without excluding any. 7.
