2 The Kidney Stone Prevention Clinic: helps you and your family learn about kidney stone formation, diagnoses, treatment and prevention evaluates the client s history and provides medical management and education to prevent or decrease kidney stone formation offers specific recommendations regarding lifestyle changes, diet and medications to help minimize the risk of future stone formation or recurrences. Stone recurrence is a common problem for kidney stone patients. In Case of an Emergency If you think you are passing a stone, symptoms indicating that you should seek immediate medical attention include: severe intolerable pain that begins in the lower back and may move to the side or groin unable to void or produce urine renal colic - severe pain described above plus blocked urine flow presence of fever and unusual tiredness that occur with above symptoms indicate true emergency, and may require urgent surgical drainage (ureteral stent or nephrostomy tube placement) persistent nausea and vomiting other symptoms of a urinary tract infection with a stone, including burning during urination, cloudy urine or foul-smelling urine Certain medical conditions can make passing a stone potentially more dangerous, including those with only 1 kidney, those with diabetes, or those with decreased kidney function. Call your family health care provider. If you are unable to contact your family health care provider and you live in Regina, go to the Emergency Department at the Regina General Hospital. If you live in rural Saskatchewan, go to your nearest local health centre. Kidney Stone Prevention Clinic Team Members Client You are the most important part of the team. You and your team work together to develop your plan of care. Your role is to provide accurate health information to team members and to stay informed about your condition. It is your responsibility to actively participate and follow your plan of care. 1

3 Nephrologist (Kidney Specialist) Assesses and provides medical management to decrease and prevent kidney stone development. Sees you in the clinic at regularly scheduled appointments. Program Coordinator Coordinates client and family care. Acts as a resource person for all health care professionals working in the clinic, providing supervision and direction. Provides nursing care, assessment and education to clients. Develops and provides constant evaluation of the standards of care received by clients in the clinic. Investigates concerns identified by staff, clients, and their families. Ensures the clinic runs smoothly on a day-to-day basis. Registered Nurse Provides ongoing teaching, care and assessment. Reviews your blood work and urine results and contacts your kidney specialist as needed. Works with clinic team members to coordinate your care. Dietitian Conducts ongoing nutrition assessments. Provides diet education for you and your family. Office Assistant Sends out appointment letters to you and your health care provider(s). Confirms your appointment with you by phone. Prepares your chart for your appointments. Answers the phone and performs related office duties. 2

4 What Happens during Your Appointments Your first appointment is a group education class that lasts about 2 hours. It provides you with general information about kidney stone formation, types of kidney stones, and the causes of kidney stones. You receive diet information to help prevent kidney stone formation. Your second appointment is a one-on-one assessment and lasts about 1-1/2 to 2 hours. At this appointment, you see a registered nurse, dietitian, and nephrologist. Your blood work and urine results are reviewed with you. Your complete kidney stone history is reviewed by the team. Your follow-up appointments vary in length and are about 1 hour long, depending on your learning needs. If you prefer, you may bring a family member and/or a close friend with you to any of your appointments. Bring Your Medications List to the Clinic Bring your complete medication list with the dose and time taken, including any herbal and over-the-counter medications. If you do not have a list, you may bring all your current medications, but a list is preferred. Your medications are reviewed and discussed with your nephrologist. What Happens if the Staff Changes We keep each other well informed about your care. All of our team members know your treatment plan. The Kidney Stone Prevention Clinic Staff talks to Your Health Care Provider Every time you come to our clinic, test results, current medication list, doctor s orders, and a team report are faxed to your kidney specialist and your health care provider(s). 3

5 Blood and Urine Tests Your blood and urine tests need to be done 3 to 4 weeks before your appointment so the results can be discussed with you at our clinic. Keep all your blood work and urine requisitions in a place where you can easily see them to remind you when the tests are due. Use your Kidney Stone Prevention Clinic requisitions when you have blood and urine tests. This ensures your healthcare provider(s), kidney specialist, and this clinic receive copies of your results.
