1 1 OFFICIAL REQUEST FOR APPROVAL FOR FUNDRAISING ACTIVITY IAW AND Instructions: Complete this form and provide all necessary attachments to 366 FSS/FSK for approval. Your request will be coordinated with appropriate staffing agencies and a written response will be given to you. It is necessary that you submit your request at least 2 weeks prior to your proposed event to gain approval. Remember, you may not begin advertising or fundraising until you have written approval from the 366th Force Support Squadron Commander/Deputy Director. MEMORANDUM FOR 366 FSS/FSK Date From: (Name of Private Organization/Unofficial Activity or Person Requesting Approval) Choose type of organization: This entity is a Private Organization. I have the following up-to-date items on file with the Private Organization Monitor: Constitution and by-laws Insurance statement or insurance waiver Letter of understanding. All members understand the personal liabilities associated with this organization. Choose type(s) of request: This is for a Fundraiser This is for an Item Drive (food, clothing, etc) Note: on-base money solicitations are against the AFI This is for a Solicitation Financial statement Annual budget Current list of elected officers Quarterly meeting minutes This entity is an Unofficial Activity/Organization I have attached a copy of the organization s financial history for the last 6 months which includes any monies it has received in the last 90 days. We would like permission to conduct a Type of Request Purpose (Example: Bake Sale fundraiser, Specific Food Drive, Holiday Party Solicitation) on from -- at (Date Range) (Time) (Place) The date, time, and place must not say to be determined. Note: fundraisers can occur for up to 7 consecutive days and take place only twice per quarter. You must obtain approval from the facility manager for the location of your event (chapel, gym, golf course, commissary, downtown facility, etc) Facility Manager (Print) (Sign) Approved/Disapproved (Circle one) (Date) 2. Please answer each of the following questions with as much detail as possible. What items will be sold? (Include cost of item) If not selling items, how will you be raising funds? (example: entry fee, items donated, etc) For what will the proceeds be used? Who will be volunteering to work at your fundraiser? (example: military, civilians, spouses) How will you advertise? (ATTACH proposed flyers, s, or solicitation letters) Who will be invited? Will participants from off-base attend?
3 3 3. As the individual submitting this request for the entity named above, I certify the following to be true: (You Must Initial Each Line) a. This proposed fundraiser is NOT part of a continuous resale activity. The dates of our last fundraising activities were,, and. b. This proposed fundraiser does not involve the sale of alcohol or any form of gambling activity. The only exception to conducting raffles is Private Organizations that hold 501C status from the Internal Revenue Service. c. Military members will not be in uniform during the fundraising event. (See footnote) d. Military members and DOD employees will be in a non-duty status during the event. (See footnote) e. The use of government resources will NOT be used. For example, government resources include phones and equipment. Government resources do not include the building where the fundraiser is held, water or lights. f. This fundraiser will NOT involve the one-on-one solicitation of junior ranking members by higher-ranking participants in the fundraising effort. g. The participants of this fundraiser will not solicit downtown businesses by asking or obtaining donations of any kind without prior written approval of the Force Support Squadron Commander. Additionally, organizations are prohibited from conducting raffles if the proceeds are purely for social, recreational, or entertainment purposes of the organization members. AFI , Paragraph h. Event approval extends only to the information provided. We understand that if we change our event plans we will be required to obtain approval for any change. i. The fundraiser will not involve any door-to-door or mailing solicitation on base. j. If the fundraising event involves the sale of food, a temporary food handlers permit application was submitted to 366th MDG Public Health and was approved on (Date). k.. I have reviewed the attached checklist for all media advertising. All advertising is compliant and attached.
4 4 4. Checklist for Advertising Attached documents from part two question five must comply: a. Message informs the audience about an authorized PO event. The PO must be authorized to operate on Mountain Home AFB and the fundraiser (if any) must be authorized by the 366 th Force Support Squadron. b. The fundraiser is not purely for commercial or personal financial gain. PO s are voluntary associations of private persons and do not conduct official business. c. Message must include the following disclaimer in the same font as the message and without edit: THIS IS A PRIVATE ORGANIZATION. IT IS NOT A PART OF THE DEPARTMENT OF DEFENSE OR ANY OF ITS COMPONENTS AND IT HAS NO GOVERNMENTAL STATUS (AFI , paragraph ) For Unofficial Activities: THIS IS A NON-FEDERAL ENTITY. IT IS NOT A PART OF THE DEPARTMENT OF DEFENSE OR ANY OF ITS COMPONENTS AND IT HAS NO GOVERNMENTAL STATUS (DoDI Enclosure 2 (1)(b)(3).). d. Message does not direct the audience to use other official communications media or equipment to transact fundraising activity (do not use duty office phone number or on flyer or in content). e. Message does not direct or imply the use of duty-time for fundraising activity (on lunch hour is okay). f. For s: Message will not be sent as a Base-Wide . s can only be sent to your organization s active members or your first sergeant. g. For s: Message must not include any attachment, image or media file greater than 10 megabytes, or link to any non-.mil or.gov webpage due to risk of overloading the system and causing network slow-down or failure. h. For s: Official cannot be used for advertising if the fundraising activity is off-base.

Private Organizations must not engage in activities that duplicate or compete with AAFES or FSS activities of the NAFI (Non-appropriated Fund Instrumentalists).
