The Medisize Barr-Vent filter contains a hydrophobic filter which acts mechanically and electrostatic. This group contains artificial noses that effectively warm and moisten the air to be inhaled. They can, in consultation with a physician, replace an active humidifier. 11 Filtration and Medisize Humidification Filters Medisize HME s for Tracheotomy The Medisize HME s for tracheotomy are named Climatrach. This group contains heat and moisture exchangers which take over the function of the upper respiratory tract in patients with a tracheotomy. The Medisize Climatrachs are intended for spontaneously breathing patients. It effectively warms and humidifies the inhaled air by means of the HME medium. The Medisize Climatrach is available with two options: a safety valve and/or oxygen port. The safety valve opens when the pressure within the Medisize Climatrach temporarily becomes to high, for instance during coughing. When the pressure reaches normal values, the valve closes again. The oxygen port is positioned in such a way that the oxygen to be inhaled is also humidified and heated. Application: Humidification of air to be inhaled (passive humidification). 13 Filtration and Humidification Medisize HME Booster for active humidification Passive humidification of an HME is transformed into active humidification with the use of the HME-Booster. Where passive humidification is not enough, the HME Booster can add extra heat & moisture to the output of an HME. How does it work? An aluminium grid in the T-Piece transfers heat from the heating element through the membrane. This results in the evaporation of the administered water, allowing vapours to go through the membrane to heat and humidify the gasses passing through the ventilatory circuit. The physical properties of the Gore-Tex membrane only allows vapour to pass, and not liquid. See also the illustration. The HME-Booster thus enhances the humidification to the critically ill in a simple, safe, efficient and cost-effective way as an alternative to expensive and cumbersome hot water bath humidifiers. How it Works The HME-Booster The HME-Booster is a T-shaped device equipped with a hydrophobic membrane, allowing water to pass only when it evaporates. The purified water saturates the hydrophobic membrane. The water is then heated to the vaporization point and passes through the membrane and enters the delivered gas stream. Dry & Cool air becomes warm & moist Water enters the HME-Booster by way of a luer lock connection (250mls of water will last 3 days). The moisture in the saturated membrane is heated by the Positive Temperature control element. The HME-Booster improves absolute humidity, relative humidity and the temperature delivered by the HME. Tracheal temperature also increases. Not only does the HME-Booster delivers heat and moisture directly to the patient with every inspiration, during expiration it also increases the performance of the HME for the next respiratory cycle. We can guarantee the good qualities of the HME Booster when it is used in combination with the Medisize Hygrovent or the Hygrovent S. 15 Note: for an optimal quality we recommend to use the HME Booster for a maximum of 3 days.

27 Breathing Therapy: CPAP CPAP Continuous Positive Airway Pressure. A mode of breathing support that helps patients to keep open certain parts of the airways. Patients suffering from sleep apnoea (caused by collapse of parts of the upper airways) benefit greatly by CPAP. Medisize CPAP systems are not used for this indication, they are used in hospital settings. Description Indication for applying CPAP in hospitals for patients with poor gas exchange. Causes for a decrease of gas exchange are liquid in the lungs, inflammations and thickening of the alveoli-wall. for patients who haven t been able to breathe on themselves for a longer period of time. These patients have to learn to breathe again. Therapies The concentration of oxygen in the lungs has to be increased. With the CPAP method the alveoli maintain a greater inflation volumes. An important physiological feature of CPAP is that as alveli are recruited the Fi0 2 needed to maintain an adequate PaO 2 will decrease there fore oxygenation becomes more efficient. The bulky designs of existing expiration valves has trigged Medisize to start the development of the Compact Expiration Valve; a small product with improved functional characteristics.
