3 IMPORTANT DATES Applications must be returned to the Administration Building, Office of Special Services, no later than 3:00pm, FRIDAY, MAY 1, 2015 Documents Required: Original Birth Certificate with raised seal Proof of Residency Lottery Drawing Monday, May 4, 2015, at 3:00 P.M. at the Administration Building Screenings to begin - THURSDAY, MAY 7, 2015, through FRIDAY, MAY 8, 2015 Successful candidates will be notified by TUESDAY, MAY 12, 2015 Confirmation of attendance by parent to school TUESDAY, MAY 26, 2015

4 REMEMBER THAT Morning sessions will be available for 3-year-old children who qualify at Ocean Township Elementary School and Wayside School. An afternoon session will be be provided for 4-yearold children who qualify at Wayside Elementary School. These sessions will be held five days per week for 2 ½ hours per day by a fully certified preschool teacher. Parents will be responsible for all transportation of the nondisabled preschool students. No tuition is currently charged.

5 MORE INFORMATION Upon confirming attendance into the program, it is the parent s responsibility to also contact the Central Office to schedule an appointment to officially register your student for school and the program. Assistant Superintendent of Special Services Tel Ext Jacqui Castronovo Supervisor of Special Education PS Ext April, 2015 Attention Ocean Twp Parents of Preschool Age Children The Ocean Twp Public School District is pleased to announce the eleventh year of our Preschool Program beginning September, Special needs and nondisabled preschoolers will come together in an integrated environment; research has shown that all students benefit from this type of program. Preschool sessions will be held five days per week for 2 ½ hours per day by a fully-certified preschool teacher. Parents will be responsible for all transportation of the nondisabled preschool students. No tuition is currently charged. Absences exceeding 20 days will result in your child being dropped from the program. Applications for the program are available at the Twp. of Ocean Administrative Office at 163 Monmouth Road, Oakhurst. They will also be available in your local school s main office, by calling (732) x1040, and online at Applications must be returned to the Twp. of Ocean Administration Building, Office of Special Services, no later than 3:00 PM on Friday, May 1, An original birth certificate with a raised seal and proof of residency are required to participate in the lottery and must be presented when the application is submitted. To qualify: A child must be three (3) years old by October 15, 2015, and be at age level on a preschool readiness screening administered by the District. Children will be screened in the order of their names randomly chosen in a lottery. The Lottery drawing will be held on Monday, May 4, 2015, at the Township of Ocean Administration Building at 3:00 PM. Parent attendance at the lottery is not required. The Preschool Readiness Screening will be administered to children in the order of their lottery selection between Thursday, May 7 and Friday, May 8, Parents will be required to bring their child to their designated elementary school for this assessment at a designated time. The program will be offered to the first fourteen 3-year-old children and the first two 4-year-old children who score at age level in pre-academic, language and motor skills on the Preschool Readiness Screening conducted by the District. The first fourteen 3-year-olds who qualify in the order that they were screened will be offered enrollment in the 3-year-old morning program at OTES or at Wayside School, and the first two 4-year-olds who qualify in the order that they were screened will be offered enrollment in the afternoon 4-year-old program at OTES. The remaining children will be placed on a waiting list. Only those parents of children who were found eligible for the program will be notified no later than Tuesday, May 12, If your child is offered placement, confirmation of attendance is required by Wednesday, May 26, Following this confirmation you will need to call the Central Office at x 3116 to schedule an official registration. (Hospital certificates are not acceptable for Proof of Age). RESIDENCY: PLEASE NOTE: TWO PROOFS OF RESIDENCY ARE REQUIRED: You MUST reside in Ocean Township in order to register your child. You must present the original documents and a copy for our records. Post Office Boxes are NOT considered legal documents for Proof of Residency. You must provide one item from Group A and two items from Group B. Group A (select one) Property tax bill Current lease agreement Mortgage statement Sewer tax bill Group B (select two) Current utility bill Bank Statement Current insurance statement Vehicle registration MEDICAL RECORD registration. Immunization record (most current) must be presented at CENTRAL REGISTRATION x3116 If the above documents are not presented at the time of registering your child, the registration will not be processed. If you are unable to submit the proofs of residency noted above because you have recently moved into Ocean Township, if you are living with family or friends who are Ocean Township residents, or the child lives with an Ocean Township resident other than the parent or guardian, you must contact Mrs. Amato in the Superintendent s Office at (732) , ext to obtain Supplementary Enrollment Forms and submit proof of residency for the person who actually owns or leases the home or apartment in which the child lives. If you are unable to produce the above documentation and consider yourself to be homeless, please let Mrs. Amato know and she will direct you to our District Homeless Liaison.
