Blank forms are available at asfr.epc.org/blankacr. Once data has been gathered, it should be entered on asfr.epc.org/annual church report. Click Instructions on that page for a video on how to log in and use the online form. Please complete the report by March 31. That will give us adequate time to prepare reports for publication to the 2016 General Assembly meeting in June. We depend on you to send us your reports on time so we can give the Assembly an accurate snapshot of our family of churches. Information entered last year is pre printed on the following pages. 2. When you complete the worksheet, go to the EPC Annual Church Report page asfr.epc.org/annual church report. Click <Instructions> for an introductory video. Click <Login to start> and use the login information for your church provided below or in the cover letter: Login information was mailed to Pastors and Clerks of Session in mid January To obtain or reset your church s user ID and password, kim.avis@epc.org, with a copy to your Pastor, Clerk of Session, or church administrator. Your church leadership is responsible for controlling access to this User ID and Password. 3. If your church entered a Report for 2014, you will be able to view it. If your church did not file a 2014 report, you may open it and submit 2014 data (either complete or partial) if you wish. If you sent in corrected information after you submitted the 2014 report, it will not be visible in the online report, but is in our database and will be reflected in the pre printed data in this worksheet. If you want to make few corrections to your 2014 report, please those to kim.avis@epc.org. If there are numerous corrections, please file a new 2014 report. 4. As you are working on your 2015 Report, you may click the <save as draft> button at any time, then come back to resume and/or make changes. 5. CLICKING SUBMIT IS FINAL. 9 FINANCES Note to churches on a fiscal year other than January December: All financial reporting to the Assembly is on a calendar year basis. Churches are asked to use figures in all categories that correspond to the calendar year, even if that means a reasoned estimation. Questions on the Finances section?
