1 General Price List Prices effective September 1, 2010 It is our goal to provide superior services at a cost acceptable to the public. Our General Price List presents an itemized listing of our service and merchandise offerings. It also lists several service packages which provide a simplified way of selecting complete services at a single, discounted price. 2 General Price List Prices effective September 1, 2010 Credit Policy of Amundson Funeral Home Arrangements for payment of our charges must be made prior to services being rendered. Reimbursement for non-funeral home costs (also known as cash advances) paid on your behalf and 25% of our charges for the services and merchandise you select, or $2,500.00, whichever is greater is due before the day of the visitation or disposition, with the balance due within 30 days. For approved county funded arrangements, the full balance for additional items is due at the time of arrangements. We accept cash, checks, visa or master card, and verified assignment of life insurance policies to satisfy payment requirements. Those individuals holding prepaid funeral trust accounts verified by our staff to be current shall be exempt from the down payment provision of our policy. A late penalty of 1% per month (12% per year) will be assessed on the unpaid balance. Accounts with remaining balances after 90 days will be turned into the hands of another for collection. Funeral Service Value Packages Traditional funeral services package - Our traditional funeral services package includes: Services of funeral director and staff, transfer of remains to funeral home (within 50 miles), embalming, other preparations of the body, use of facilities and staff for afternoon or evening visitation and funeral ceremony or services in other facility, funeral coach, committal or other disposition service, service vehicle, thank you cards, standard service bulletin paper, register book, and temporary marker when necessary. Standard Printed Memorial Package (see below), temporary grave marker, and Pallbearer car items are provided as needed with the package at no added cost. Traditional graveside funeral services package - Our traditional graveside funeral services package includes: Services of funeral director and staff, transfer of remains to funeral home (within 50 miles), embalming, other preparations of the body, funeral coach to cemetery,service vehicle, register book, standard service bulletin paper, thank you cards, temporary marker when necessary. Afternoon or evening visitation is available as an option with this plan. Standard Printed Memorial Package (see below), and temporary grave marker items are provided as needed with the package at no added cost. CREMATION FUNERAL SERVICE (with services and/or viewing prior to cremation) Local removal of remains, embalming, dressing and other care of remains, services of funeral director and staff, facilities and staff for evening visitation (where included below) and funeral ceremony, transportation to nearest crematory, transportation of cremated remains to local cemetery for interment, funeral director an/or staff for weekday committal service at a later date (where indicated), $50.00 Urn selection credit (used only toward urn purchase from Amundson Inventory.) STANDARD DIRECT CREMATION SERVICE (with no formal service, or assistance with interment of cremated remains) Local removal of remains, other care of remains, one hour private family viewing of unembalmed remains during regular business hours, services of funeral director and staff, minimum cremation container acceptable to the crematory transportation to nearest crematory. $ SHIP IN MEMORIAL SERVICES (where cremation has already been cared for out of town) Services of funeral director and staff and necessary equipment to arrange and direct memorial service at our funeral home or other facility in local area. Memorial Service and evening visitation on day prior to service with graveside - $ Memorial Service with graveside (no visitation on day prior) - $ Memorial Service only (no visitation day prior or graveside) - $ Graveside committal only - $ *Standard Printed Memorial Package - includes one register book, standard service bulletins with standard inhouse printing of up to 250 bulletins, up to 100 thank you cards, one Personalized Memorial Candle, and a temporary grave marker where needed. $ Custom Printed Memorial Package - Same as in standard package but with custom designed bulletins using up to 4 color photos printed on two sides 8 1/2 x 11 standard paper (up to 250 bulletins) $ Upgrade Standard Memorial Package to the Custom Printed Memorial Package - $75.00 (only available when using one of our service packages) Temporary Grave Marker - $25.00 Personalized Memorial Candles (discounted pricing available for quantity purchases) - $20.00 Accommodation charges such as crematory fees, cemetery fees, flowers, honoraria for clergy, musicians, obituary charges by newspapers, will be added to any arrangements. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Services of Funeral Director and Staff Our fee for the services of funeral director and staff includes, but is not limited to staff to respond to the initial request for service, arrangement conference with family or responsible party, arrangement of funeral, preparation and filing of necessary authorizations and permits, recording vital statistics, preparation and placement of obituary notices, staff assistance prior to, during and following the funeral, including coordination with those providing other portions of the funeral, e.g. cemetery, crematory and others. Also included in this charge are overhead expenses, secretarial and administrative costs, and equipment and inventory expenses. $ This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) Embalming Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation, or immediate burial. $ Other Preparation of the Body Hair Dresser, dressing, cosmetics, and casketing remains $ Page 6

Our services include coordinating the funeral arrangements, supervision of funeral, and staff to attend funeral ceremony. $ C. Use of facilities and staff services for memorial service (without body). Our services include coordinating the memorial service arrangements, supervision of the memorial service, and staff to attend the service.$ D. Equipment and staff for graveside services. Forwarding Remains to Another Funeral Home This charge includes removal of remains, necessary services of staff, arrangements with family, embalming, necessary authorizations, and local transportation to airport. This charge does not include visitation, rites or ceremonies prior to forwarding of the body A. With Minimum shipping container $ B. Direct Cremations (plus crematory charges) $2100-$6475 range of price Our charge for a direct cremation without any attendant rites or ceremonies, includes removal of remains, local transportation to crematory, necessary services of staff and authorizations. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fibreboard or composition materials (with or without and outside covering). The containers we provide are cardboard base and top. A. Direct Cremation with container provided by purchaser $ B. Direct Cremation with alternative container $ C. Direct Cremation with casket selected from our funeral home $ ( in addition to the cost of the casket selected) Anatomical Donations Local transportation, necessary prepatation of remains as per UND School of Medicine regulations, professional service of staff including filing necessary permits, etc. Does not include a visitation, viewing, rites, or ceremonies. (UND provides a stipend of $300 preparation fee directly to the funeral home.) $ Miscellaneous Items Additional fee for arrangements and transportation to ship or receive via AMTRACK $ Additional fee for arrangement with Foreign Consulate for shipping remains $ One hour private viewing during regular business hours $75.00 Visitation or service on New Years Day, Easter, Thanksgiving, or Christmas Day (when available) $ Receptions (two hour maximum) $ For receptions held in the funeral home. Includes staff assistance with setup, serving, and cleanup. Funeral home will provide styrofoam cups, paper plates, napkins, and coffee. Family will provide all food items which need to be purchased from a retailer and not homemade. Food items will be limited to sandwiches, meat and cheese trays, chips, fruit trays, and cookies or bars. We are unable to allow the service of hot food items. An additional per half hour will be added to the reception fee for time that exceeds the two hour time allotment. Page 9

However, many cemeteries require that you have such a container so that the grave will not sink in.
