It doesn t come with complex regulations. You don t need medical certificates. And you can use it at any provider of your choice. The Active Lifestyle Package simply gives you refunds on popular services you probably already use. So you can afford to do more, or try something new. If you re looking for a motivational boost you can use your gym membership refunds towards new sports gear. And the kicker is, you can get money back for that too. Alternatively, use the money you get back on running that marathon and put it towards paying for new cycling equipment. And yes, there are refunds for that as well. So regardless of what s driving you whether it s losing weight, gaining muscle or beating a personal best you can cool down safe in the knowledge your Active Lifestyle Package is good for your wallet too. 4 6 7 GYM MEMBERSHIPS AND ENTRY FEES FAST TRACK YOUR FITNESS Get up to $160 back. We will pay for: the entry fee or membership fee to a registered gym or fitness centre in Australia. The most we pay is: 50% of the cost, with a total 12 month limit of $160. Conditions To be eligible, the gym or fitness centre must offer fitness related machines, weights or group classes, and be a registered business with a national fitness association. A refund will be based on the date of purchase of a membership or entry fee. To apply for a refund, detailed accounts or invoices from the gym or fitness centre must be supplied and clearly show your name and the business s ABN. PERSONAL TRAINING PUSH YOUR BOUNDARIES Get up to $210 back. GROUP EXERCISE CLASSES USE THE EXTRA PUSH TO GET YOU THERE Get up to $180 back. We will pay for: fitness and physical activity classes conducted by a qualified instructor where there are three or more participants. The most we pay is: up to $15 per class, with up to $90 per six month period, and with a total 12 month limit of $180. We will not pay for: classes which may be included or claimed under Gym membership and entry fees or Personal training. Conditions To apply for a refund, detailed accounts or invoices, outlining the business s ABN for an instructor lead fitness session such as group training, aerobics and yoga is required. Eligible services include (but are not limited to) yoga, Pilates, swimming classes, cycling classes, dance classes, rowing and other related activities with the primary purpose to enhance health and fitness. We will pay for: fitness and physical activity sessions provided by a qualified instructor where no more than two participants attend per session. The most we pay is: up to $17.50 per session, with a total 12 month limit of $210. We will not pay for: sessions which may be included or refunded as a membership fee under Gym membership and entry fees or Group exercise classes. Conditions To apply for a refund, detailed accounts or invoices outlining the business s ABN and your exercise professional s name are required. The exercise professional must hold a recognised certification in group fitness and be registered with a national fitness association.

5 8 9 ENTRY FEES TOWARDS SPORTS AND FITNESS EVENTS BEAT YOUR PERSONAL BEST Get up to $160 back. We will pay for: competitor participation entry fees for a sporting or fitness event held in Australia, such as triathlons, marathons, cycling or specialised competitor events. The most we pay is: 50% of the cost, with a total 12 month limit of $160. We will not pay for: competitor entry fees for events paid by another source (i.e. corporate sponsorships). Conditions To apply for a refund, detailed accounts or invoices from the event organiser detailing the competitor s registration are required. Participation in the event must occur during the Agreement term. Your participation in the event must satisfy all rules and regulations set by event organisers. SPORTS GEAR AND EQUIPMENT LOOK THE PART Get up to $50 back. CYCLING EQUIPMENT AND REPAIRS REFUSE TO BE SIDELINED Get up to $250 back. We will pay for: bicycles and components purchased through an Australian registered sporting goods store or bicycle retail store (including online); and repairs and/or maintenance required to a bicycle provided through a bicycle retail store. The most we pay is: 50% of the cost of the bicycle, equipment or repairs, with a total 12 month limit of $250. We will not pay for: equipment which can or has already been paid under Sports gear and equipment. non-core items such as bike accessories or apparel. Conditions A Qualifying Period of 6 months applies. To apply for a refund, an itemised receipt detailing the business s ABN is required. To be eligible for a refund, a bicycle must be non-motorised, two-wheeled and used for sports or leisure activities. Maintenance and repairs relate to core bicycle components only, such as frame, chains and wheels. repairs relate to core bicycle components only (such as wheels). We will pay for: sports apparel or equipment purchased through an Australian registered sporting goods store or specialised sporting retail business (including online). The most we pay is: 50% of the cost, with a total 12 month limit of $50. We will not pay for: equipment which can or has already been paid under Cycling equipment and repairs. Conditions To apply for a refund, an itemised receipt detailing the business s ABN is required. Apparel and equipment must be designed for the specific purpose of undertaking a sport or physical activity.

6 10 SERVICES AND SUPPORT HOW TO GET A REFUND HAVE YOUR REFUND APPLICATION PROCESSED QUICKLY USING ONE OF THESE OPTIONS: Online Services Request a fast refund by uploading and sending your receipts. To login to Online Services, visit australianunity.com.au/onlineservices Once logged in, follow the options under the Claims menu. Apps and devices The Australian Unity refund app also helps you fast track your refund applications. If you have an iphone or ipad, simply download our refund app from the itunes app store. Android users can download the app from the Google Play Store. Once downloaded, you can submit refund applications by photographing and uploading your receipts to us. Send your refund application request by Send a scanned copy of your Refund Application Form and your receipts to us via to lifestylepackages@australianunity.com.au Send your refund application request by post Simply complete, date and sign a Refund Application Form and attach your receipts. Remember to keep a copy for your records. A refund is invalid if it is: made outside the Agreement or received 14 days after the end of your Agreement. made when the Agreement has terminated, or lapsed due to non-payment. a service fee, delivery charge, including a tax or impost, that is not a direct cost for the treatment received or goods purchased. A refund will not be paid when: the product or service has been purchased or received outside Australia. the product or service was purchased prior to the date of joining. you are entitled or eligible to get money back under a government benefit scheme such as Medicare or the Pharmaceutical Benefits Scheme (PBS). you are eligible to either get money back against any third party, including a private health insurer or receive payment under a scheme of insurance or compensation provided for by the Commonwealth or a state or territory law (i.e. workers compensation insurance). they relate to products and services provided or rendered by a Provider in the treatment of themselves as an individual (or customer) or to the Provider s partner or dependants; or business partner, or the partner or dependants of the Provider s business partner. you fail to make full and complete disclosure of all matters relied upon by us in support of, or relevant to, assessing a refund application. you provide false, inaccurate, misleading or fraudulent information regardless of whether the information is contained in the Refund Application Form. you fail to provide any information which may be reasonably required by us to assess your refund application. 11 Download additional Refund Application Forms from: australianunity.com.au/downloads Once your refund application is approved, we will deposit your refund directly into your nominated bank account. 7 12 13 MORE ABOUT YOUR EXCLUSIVE LIFESTYLE PACKAGE What is the price that I ll pay? The price of the Exclusive Lifestyle Package is set out in your lifestyle package summary and in this guide. The price of the Exclusive Lifestyle Package may vary in different states and territories. From time to time we may offer discounts on the Exclusive Lifestyle Package at our discretion. How can I pay for my Exclusive Lifestyle Package? There are two ways to pay for your Exclusive Lifestyle Package: 1. Pay the full price for your Agreement period upfront; or 2. Pay the equivalent of the first monthly instalment by credit card, followed by 11 equal monthly direct debit payments. We will inform you at the time of purchase, which date of the month your instalments are due. To pay by direct debit, you need to complete a Direct Debit Service Agreement. Our customer service team will assist you with this. Simply give us a call on ENDING AND RENEWING THE AGREEMENT How to end your Exclusive Lifestyle Package Agreement You can terminate this Agreement in writing within 30 days of the Commencement Date ( Cooling Off Period ), and get your money back if you have not applied for any refunds. This Cooling Off Period also applies where the Agreement has rolled into a new term. You may terminate your Agreement at any time after the Commencement Date in writing and before the expiry of the Fixed Term, but you may have to pay us an Early Termination Fee, which will not exceed three months of instalments, and may be directly deducted from the account you nominated in your Direct Debit Service Agreement. You can contact us at customerservice@australianunity.com.au How does the Exclusive Lifestyle Package Agreement get renewed? We will contact you in writing at least 30 days prior to the expiry of your Agreement, to give you the right to opt out of the Agreement being renewed. When can Australian Unity terminate or end my Exclusive Lifestyle Package Agreement? We may terminate this Agreement immediately in writing and charge you an Early Termination Fee if we believe, on reasonable grounds, that you: have not met the conditions required to enter into this Agreement. have breached this Agreement by acting fraudulently in either applying for the Exclusive Lifestyle Package or in receiving a refund on an Exclusive Lifestyle Package service. are more than 60 days late in making your payment. Where you have entered into a Direct Debit Service Agreement with us, you will have authorised us to debit any early termination fee payable by you. We reserve the right to end the Agreement at any time at our discretion, subject to providing reasonable notice to you, ensuring any monies paid in advance of the termination date are refunded, and any valid refund applications made prior to termination are paid. For the avoidance of doubt, nothing in this Agreement affects your rights to cancel or terminate this Agreement when any of the consumer guarantee provisions apply under the Australian Consumer Law.

9 16 17 IMPORTANT INFORMATION Services overview Subject to the terms of this Agreement, we agree to pay you a refund in relation to the costs you incur purchasing goods or receiving services up to the limits shown on pages five through nine. The term of the Agreement The Agreement starts on the Commencement Date and continues for a fixed term of 12 months unless cancelled, renewed or terminated in accordance with this Agreement. Conditions of purchase You must meet the following conditions to be eligible to purchase the Exclusive Lifestyle Package: you must consider and accept the Exclusive Lifestyle Package advertised terms, including any limited offers and price; you must be a resident of Australia; and you must be 18 years or older. When you apply to purchase or continue to receive the Exclusive Lifestyle Package services, you acknowledge you have met the above conditions. A customer may purchase an Exclusive Lifestyle Package up to a maximum of one of the same package per person and two of the same Exclusive Lifestyle Packages per household. A customer may only purchase one of the same Exclusive Lifestyle Packages, per person for any given 12 month period from the Commencement Date. The benefits of an Exclusive Lifestyle Package are limited to one person per package, therefore refunds can only be applied for and received by the person originally nominated as the beneficiary of the Exclusive Lifestyle Package services at the time of purchase. Limits are not transferable between household members. Refund criteria Only the person who has been nominated as the beneficiary of the services is able to seek a refund. This may be the person who has entered the Agreement and paid for it, or the person who is nominated as the beneficiary of the services. Refunds are limited to the amounts stated on pages five through nine or the actual amounts charged by the Provider, whichever is the lesser. You can apply for a refund up to 14 days after the end of the Agreement for goods or services purchased during the Agreement. We are not under any obligation to pay refunds we receive more than 14 days after the end of the Agreement. Receipts that you use to apply for a refund must be dated accordingly and represent the actual product or service you purchased within the Agreement. Refund sub-limits not reached during the period, set out on pages five through nine, will automatically expire and not be carried forward. Refund applications can only be made if they relate to goods and services purchased and received within Australia and supplied by an approved Provider. We do not represent or provide any warranties for a Provider s products, services, qualifications, training or skills. The information we provide in relation to these Exclusive Lifestyle Packages and their services is offered solely for your convenience. The information does not constitute or imply an endorsement, sponsorship, recommendation, or affiliation with the Provider s products and services. We disclaim all warranties that relate to the Provider s products and services, express or implied, to the full extent permitted by law. Limits apply per person, per fixed term of your Agreement. Your rights under the Australian Consumer Law Our Exclusive Lifestyle Package services come with nonexcludable guarantees under The Australian Consumer Law ( Consumer Guarantees ), including that services will be provided with due care and skill. You are entitled, at your option, to various remedies under The Australian Consumer Law if these guarantees are not complied with, such as the resupply of services and you may also be entitled to compensation. Nothing in this Agreement excludes, restricts or modifies any of the Consumer Guarantee.

10 18 19 Your rights under the Australian Consumer Law (continued) These guarantees include the resupply of services. You may also be entitled to compensation. Nothing in this Agreement excludes, restricts or modifies any of the Consumer Guarantees. To the maximum extent permitted by law, our liability in connection with this Agreement (including liability for negligence) is limited to: the lesser of the replacement or repair cost of the goods supplied in connection with this Agreement; and in the case of services provided in connection with this Agreement, the cost of supplying the services again. For all other claims or liabilities not related to any breach of a Consumer Guarantee, and to the extent permitted by law, our maximum liability to you arising out of or in connection with this Agreement is capped at the average monthly payment made by you to us for the Exclusive Lifestyle Package service. Intellectual property All trademarks, copyright and other intellectual property rights inherent, and used in relation to the Exclusive Lifestyle Package services, will continue to be our property. Use of the Exclusive Lifestyle Package services does not entitle you to any rights or entitlements in relation to this intellectual property. Other This Agreement is governed by the laws in force in the State of Victoria, Australia. The courts of Victoria (including appeal Courts) are the jurisdiction for any unresolved disputes. If any part of this Agreement is void, unenforceable or illegal it is severed from the remainder of this Agreement. The remainder of the Agreement will continue to have full force and effect and the validity or enforceability of that part in any other jurisdiction is not affected. We may assign or otherwise deal with our rights and subcontract and novate our obligations under this Agreement by written notice to you without your prior written consent. We reserve the right for third parties to assist us in the provision of the services. You acknowledge and understand Australian Unity utilises call recording for audit quality and training purposes. The Agreement constitutes the entire agreement and understanding between you and us and supersedes any and all prior communications, representations, agreements or understandings between you and us with respect to its subject matter. GLOSSARY OF TERMS Agreement means the initial fixed term period of 12 months commencing on the Commencement Date and any subsequent rolling term for a further 12 month period. Australian Unity means Australian Unity Lifestyle Pty Ltd (ACN ) and any related bodies corporate referenced in the Agreement as us, we, our. Commencement Date means the date of purchase or the date agreed between us and you as the date the Agreement starts. Early Termination Fee means the fee charged to you, which may be debited from your nominated account for ending, by act or omission, the Agreement before the 12 month term has ended. Exclusive Lifestyle Package services refers to the services we provide you with as outlined in the advertised offer, on pages five through nine, and provided in accordance with this Agreement. Licensed Practitioner means any individual certified or licensed by any health regulatory board of a particular jurisdiction for the profession in which they practise. Provider means a supplier or service provider of products and/or services of the type described on pages five through nine, and who has an Australian Business Number (ABN) registration. Qualifying Period means a period, starting at the Commencement Date, and ending at the specified time as detailed in an Exclusive Lifestyle Package. During this period a customer can submit a refund application, and any eligible refund will only be payable after the Qualifying Period has elapsed. Please note that where the Agreement is terminated, or lapses for non-payment prior to the period elapsing, you will not be entitled to receive a refund. You refers to the purchaser of the Exclusive Lifestyle Package or the person for whom the Exclusive Lifestyle Package services have been obtained, depending on the context.

11 20 21 DIRECT DEBIT SERVICE AGREEMENT IF YOU HAVE CHOSEN TO PAY BY DIRECT DEBIT, THESE TERMS WILL APPLY TO YOU. Our commitment to you This document sets out your rights, our commitment to you and your responsibilities to us, together with where you should go for assistance in respect of your direct debit arrangement with Australian Unity. Initial terms of the arrangement In terms of the Direct Debit Request (DDR) arrangement made between us and authorised by you, we undertake to periodically debit your nominated account in accordance with your authority to direct debit. Drawing arrangements if any drawing falls due on a non-business day, it will be debited to your account on the next business day following the scheduled drawing date. we will give you at least 14 days notice when we intend to make changes to the initial terms of the arrangement. Your rights. Changes to the arrangement If you want to make changes to the drawing arrangement, please notify us in writing at least four business days prior to your next scheduled drawing date. These changes may include: deferring the drawing; or altering the schedule; or stopping an individual debit; or suspending the DDR; or cancelling the DDR completely. Enquiries If you have any enquiries, they should be directed to Australian Unity rather than to your financial institution. All information relating to the DDR held by us will remain confidential except for information that may be provided to our financial institution to initiate the drawing to your nominated account or information disclosed to a third party as required by law. Information may also be provided to Australian Unity Limited or any of its wholly-owned subsidiaries to enable this DDR to be effected. Disputes if you believe that a drawing has been initiated incorrectly, you should raise the matter directly with Australian Unity. if you do not receive a satisfactory response from us to your dispute, contact your financial institution who will respond to you with an answer to your claims in accordance with their dispute resolution procedures. Note: Your financial institution will ask you to contact us to resolve your disputed drawing prior to involving them. Your commitment to us It is your responsibility to ensure that: your nominated account can accept direct debits (your financial institution can confirm this); and on the drawing date there are sufficient cleared funds in the nominated account; and you advise us if the nominated account is transferred or closed; and you give us your updated expiry date when you are issued a new credit card if applicable. If your drawing is returned or dishonoured by your financial institution, we will notify you in writing. Any transaction fees payable by us in respect of the above may be passed on to you. Consecutive returns or dishonours may result in the direct debit facility being withdrawn.
