Covering Request Letter: use the sample letter enclosed to produce and submit a formal request to be included in the CIAN group exemption. 2. Application: All questions on the application must be answered and the complete application submitted. 3. By-Laws or Constitution: A copy of the Ministry s Constitution and/or By-Laws should be included. Note: If the Ministry does not have a Constitution or By-Laws, then request the MINIMUM CHURCH CONSTITUTION. Also, we have included text that is recommended to be in your by-laws or constitution. 4. Corporate Resolution: the enclosed resolution should be approved and included in your corporate minutes. Submit a properly notarized copy with this application package. 5. IRS Guidelines: read the letter outlining IRS guidelines to determine if you are required to submit a Form 990 to the IRS each year. For more information consult IRS tax guides. 4 BY-LAWS INSERT 1. NETWORKING: The nature of this ministry is independent and autonomous. However, in view of the command of our Lord to walk in unity and the prayer of our Lord that we all be one, it is the purpose of this ministry to build relationships with various groups and organizations within the Christian community. It is therefore our desire to identify and network with other ministries and groups as the Lord would direct, and the pastor/president and leadership of the ministry would see as beneficial. 2. CHRISTIAN INTERNATIONAL APOSTOLIC NETWORK: Christian International Apostolic Network (CIAN) is an organization or network of ministries that share a common vision regarding apostolic and prophetic oversight and present truth being revealed to the Body of Christ. It is the desire of this ministry to identify with, be a part of, and support financially the CIAN. 3. APOSTOLIC TEAMS: In cooperation with the Apostle and Prophet Ministry of CIAN, the Pastor/President will select a team of three ministers who are also affiliated with CIAN and with which the ministry is related, who will then be appointed to work with this ministry. Their ministry of oversight, counsel and assistance is offered voluntarily and is not in any way binding on this fellowship or the decisions of the Pastor/President and leadership of the ministry. 4. OTHER AFFILIATIONS: This ministry may affiliate with other organizations as the Pastor/President and leadership of the ministry would deem appropriate.

An annual business meeting will be held and accurate records shall be kept of all business meetings and financial matters pertaining to the ministry, by an official member of the ministry.
