M-F 7:30-5, Sat 9-2, Sun 5PM Check OUT Time before 12:30 PM Any Pet picked up after 12:30 PM will be charged $10.00 for half day. $10.00 charge will apply for Sunday 5:00 o clock PM pickups per pet! Overnight Boarding Included: o Individual Kennel for bedding, food and water. o Multi-Daily Individual walks. ¾ acres of property. o You may bring bedding but no large blankets. o You may bring favorite food and one toy. o Medication Charge may apply. Any pet staying over 3 nights will required a bath at regular rate, to go home! Grooming is also available at regular rate and will be performed on last day of visit. 4 DAYCARE, GROOMING & BOARDING AGREEMENT Client Agreement and Release of Liability I hereby release the Lola Bell s Pet Grooming, LLC hereafter referred to as LBPG, its agents, officers, sub-contractors, employees, animal owners, customers, and potential customers of LBPG from any and all liabilities, financial, and otherwise, for injuries to myself, my dog, or any other property of mine, which arise in any way from services and/or products provided by, or as a consequence of my association with LBPG. I agree to assume all liabilities and responsibilities, financial and otherwise, for the behavior and health of my dog In consideration of the services rendered by LBPG. I waive any and all claims, actions, or demands of any nature, foreseen or unforeseen, that I may have against LBPG relating to the care, control, health, and/or safety of my dog arising during pick-up, transport, drop-off, and stay at LBPG facilities. Further, I agree to indemnify, hold harmless, and defend LBPG from and against all losses, claims, damages, liabilities, costs, attorney s fees and any other expense of every nature whatsoever incurred by and asserted against LBPG by any third party due to any damage, injury or death caused by my dog. I authorize LBPG to do whatever they deem necessary for the safety, health, and wellbeing of my dog while under the care of LBPG, including seeking professional veterinary treatment for my dog. Due to the many outstanding benefits of dog socialization and LBPG s commitment to the safety and wellbeing of my dog, I agree that the benefits of dog socialization outweigh the risks. Furthermore, I request a socialized environment for my dog while under the care of the staff of LBPG.I understand that my dogs immunizations must be kept current and that failure to provide evidence of their currency is basis for refusal of my dogs participation until they are current and updated records have been provided to LBPG. I further understand that LBPG staff temperament tests all dogs prior to admission to daycare and boarding to ensure the safety of each dog in their care. In the unlikely event my dog does not pass this test they will be prohibited from participating in any off-leash events at LBPG as well as prohibited from participating in the daycare and boarding services provided by LBPG until they have been re-tested and pass. LBPG has the right to refuse service to me and/or my dog at any time for any reason. I understand that if my dog has a history of or repeatedly demonstrates aggression or biting of humans or animals, LBPG reserves the right to refuse service. I understand that all bites will be reported to the local authorities as required by law. Both LBPG and the Dog s Owner(s) agree that the laws of the State of Alabama shall govern this Agreement, the construction of its terms and the interpretation of the rights and duties of the parties hereto. Both parties also agree that if any provision of this Agreement, or any portion thereof, is found to be invalid or unenforceable, the remainder of this Agreement shall nevertheless remain in full force and effect. Under no circumstances shall LBPG s liability hereunder be greater than the sum paid to LBPG for the care of my dog(s). This Agreement and any documents or exhibits attached hereto constitute the entire understanding and agreement of the parties, and any and all prior agreements, understandings, and representation are terminated and cancelled in their entirety and are no force and effect. No amendment, change or modification of this Agreement shall be valid unless in writing and signed by the parties hereto. Grooming Details I have completed the New Client Grooming Form and given specific grooming information about my dog to the Lola Bell s Pet Grooming LLC, (LBPG) and understand that if LBPG requires any clarification of my grooming requests they will not groom my dog until they have spoken with me. I understand that the de-matting procedure increases the risk of injury to my dog and will not hold LBPG, its owners or its employees responsible for injury or medical expenses resulting from my dog being de-matted. I understand that my dog will only be de-matted with my approval. Hours of Operation, Boarding and Daycare Pick-up Requirements I understand that the hours of operation of the Lola Bell s Pet Grooming are 7:30 am to 5:30 pm Monday through Friday, 8:30am to 3pm Saturday and that Sundays from 10am to 10:30am and then again from 4pm to 4:30pm and are only for boarding pick-up and drop-off. If pet is not picked up before closing times on any calendar day then at that time my dog becomes an overnight guest at LBPG and will be boarded at my expense. Should this take place, I authorize in advance that LBPG will automatically begin charging me for these services and that they will be paid in full when I, or my agent, picks up my dog. I understand that I am required to provide food, medication and any toys/snuggle blankets my dog needs to be healthy and comfortable while in the care of LBPG. I understand that if I do not provide enough food for my dog LBPG will provide a high-quality food deemed nutritionally sound to sustain my dog until my return. This food will be provided at my expense. I also authorize LBPG to contact my vet should my dog run out of medication while in LBPG s care and I will be wholly responsible for any costs LBPG incurred for acquiring this medication. If I purchase a half-day daycare service and fail to pick up my dog after six (6) hours, I will be charged the full-day fee of $15. I hereby declare to LBPG that I am the legal owner of my dog; that my dog has not been exposed to distemper, rabies, or parvovirus within the past thirty (30) days that my dog has been inoculated as indicated by records which have been presented to LBPG. I understand that videos and pictures may be taken of my dog(s) and are the sole property of LBPG and may be used by LBPG for marketing purposes.
