4 Making the right choices about what to eat and when to eat are two of the biggest success factors in managing your weight. Program Overview Upon enrollment in the program, you will be scheduled for: An bariatric nutrition assessment with our program s dietitian. A class that reviews the important role exercise plays in a successful weight loss management program. A behavioral evaluation by our psychologist to determine whether there are any underlying eating behaviors contributing to your weight problem. Counseling is available if you re interested in using additional behavioral assistance to achieve your goals. Two additional visits with the dietitian to assess your progress and adjust your diet plan, if needed. These are scheduled during the mid-point of your six-month program. An individualized and group exercise visit with our exercise specialist to assess your progress and help ensure you meet your exercise goals. Three additional visits with our physician to monitor your progress and make any changes needed to your program. Two follow-up phone calls from the dietician three months and six months after completing the program to check in on your progress. The Eating Plan Making the right choices about what to eat and when to eat are two of the biggest success factors in managing your weight. Our program helps you learn how to make good choices while still providing enough variety in your diet so it s easy to stick to the plan. Some things you should know include: We do not use any pre-packaged specialty foods that must be purchased We do not prescribe medications to assist with weight loss Our dietitian has extensive experience working with patients who struggle with weight management. All patients are provided with a structured eating plan, as well as suggestions on how to tailor the eating plan to suit their preferences and lifestyle For those patients wishing to accelerate weight loss, we offer Optifast as a supplemental option to the basic program 6 7
5 The Exercise Plan Regular exercise is a critical part of a successful weight loss program and to the lifelong maintenance of a healthy weight. Our clinical exercise physiologist helps each program participant create a personalized exercise plan that will help them meet their weight loss goals. The exercise plan: Focuses on a combination of aerobic and strength training activities Uses patient-specific activities that are chosen following a group discussion regarding exercise options and preferences Requires all patients to commit to a specific exercise plan during their group education session Is reviewed at each visit during the program to ensure all patients are following the program they chose The Behavior Plan Changing your behavior when it comes to eating and exercise is the key to success in any weight loss plan. The degree to which you change your behavior is directly related to the amount of weight loss you can expect to achieve. Turning these changes into lasting habits is the real key to maintaining a healthy weight. Three important things to know about our behavior plan are: All visits take place in the privacy of the Bariatric Care Center The evaluation with the clinical psychologist is extremely important, as underlying disordered eating behaviors can be at the root of whether success or failure can be expected from weight loss efforts. Both individual and group therapy sessions are available We understand that change is difficult. That s why our team at the Bariatric Care Center includes an exercise specialist and a clinical psychologist. These two experts help our patients transition to a healthier lifestyle through changes in diet and exercise. 8 9
6 Choosing to lose weight is a life-changing decision which impacts not only you, but also your family, job and future. Outcomes Patients who have fully participated in our program have achieved a weight loss of as much as five to eight pounds per month. Success is directly related to how closely you follow the recommended program. Patient Satisfaction All patients are given the opportunity to rate the quality of their experience with Summa s Bariatric Care Center. Summa s weight loss program has consistently achieved a 95 percent satisfaction rating as compared to other hospital-based programs around the country. The Cost Some expenses related to this program are non-covered by all insurance plans. These expenses are the patient s responsibility and include the following costs: Program administration fee, which includes the bariatric nutrition assessment, interim visits with the dietitian, the group and individual education classes, and all program materials. Co-insurances, deductibles and co-payments for physician and psychologist visits as determined by For patients who do not have medical insurance or whose insurance will not cover physician visits for weight loss, we do offer private pay rates.
