1 Invest in your retirement and your yourself today, with help from the St. Luke s Health System Matched 403(b) Plan and 401(a) Matching Plan Enrollment Guide Because we VALUE a healthy retirement

2 Invest some of what you earn today for what you plan to accomplish tomorrow. Dear St. Luke s Health System Employee: St. Luke s Health System offers a generous matching contribution, outstanding convenience, and a variety of investment options. The St. Luke s Health System Matched 403(b) Plan allows employees to actively save for their retirement. When you participate in the Matched 403(b) Plan you are eligible for the St. Luke s Health System 401(a) Matching Plan. Take a look and see what a difference participating in the Plans could make in achieving your goals. Matching contributions. The 401(a) Matched Plan helps your contributions grow through a generous employer match. When you contribute to the 403(b) Plan, you are eligible for employer matching contributions made to the 401(a) Plan. St. Luke s Health System will match 50% of the employee s contribution up to the first 6% contributed with a maximum match of 3% of compensation. Take advantage of this great benefit today! Roth 403(b) contributions. A Roth contribution option lets you contribute to the 403(b) Plan on an after-tax basis and make potentially tax-free withdrawals in retirement. A distribution from a Roth 403(b) is tax free and penalty free, provided the five-year aging requirement has been satisfied and one of the following conditions is met: age 59½, disability, or death. Convenience. Your contributions are automatically deducted per paycheck. Tax savings now. Your pretax contributions are deducted from your pay before income taxes are taken out. This means that you can actually lower the amount of current income taxes you pay each period. It could mean more money in your take-home pay versus saving money in a taxable account. Tax-deferred savings opportunities. You pay no taxes on any earnings until you withdraw them from your account, enabling you to keep more of your money working for you now. Investment options. You have the flexibility to select from investment options that range from more conservative to more aggressive, making it easy for you to develop a well-diversified investment portfolio. Catch-up contributions. If you make the maximum contribution to your plan account, and you are 50 years of age or older during the calendar year, you can make an additional catch-up contribution of $6,000 in To learn more about what your plan offers, see Frequently asked questions about your plan in this guide. Enroll in your plan and invest in yourself today.

When can I enroll in the Matched 403(b) Plan and the 401(a) Matching Plan? There is no waiting period. You can enroll at any time. How do I enroll in the Matched 403(b) Plan and the 401(a) Matching Plan? Log on to Fidelity NetBenefits at or call the Fidelity Retirement Service Center at You will be automatically enrolled in the 401(a) Matching Plan when you make contributions to the Matched 403(b) Plan. A Roth contribution to your retirement savings plan allows you to make after-tax contributions and take any associated earnings completely tax free at retirement - as long as the distribution is a qualified one. A qualified distribution, in this case, is one that is taken at least five tax years after your first contribution and after you have attained age 59½, or become disabled or die. For more information please log on to NetBenefits at and select "Library" from the home page. How much can I contribute? 12 Investment Options Investment Options Before investing in any investment option, consider the investment objectives, risks, charges, and expenses. Contact Fidelity for a mutual fund prospectus or, if available, a summary prospectus. Read them carefully. New York Life Guaranteed Interest Account VRS Code: Fund Objective: Seeks to provide competitive yields and limited volatility with a guarantee of principal and accumulated interest. These guarantees are backed by the full faith and credit of New York Life Insurance Company. Fund Strategy: Contributions to the Guaranteed Interest Account are invested in a group annuity contract issued by New York Life Insurance Company. Contributions to the contract are currently invested in a broadly diversified fixed income portfolio within New York Life Insurance Company s general account. The investments in the general account are intended to provide a stable crediting rate consistent with preservation of principal. The general account is invested primarily in a conservative array of securities and cash-equivalent investments in accordance with the investment restrictions of New York Insurance Law. The primary objective of the general account is to ensure that New York Life Insurance Company can meet its obligations to policyholders and contract holders. Balances may be transferred into the Guaranteed Interest Account at any time. Fund Risk: The fund is backed by a diversified portfolio of fixed-income assets held in the general account of the issuer. Guarantees are subject to the claims paying ability of the issuer. Restrictions or fees may apply to exchanges or withdrawals. The Contracts provide for the payment of certain withdrawals and exchanges at book value during the terms of the Contracts. In order to maintain the Contract issuers promise to pay such withdrawals and exchanges at book value, the Contracts subject the fund and its participants to certain restrictions. For example, withdrawals prompted by certain events (e.g., layoffs, early retirement windows, spin-offs, sale of a division, facility closings, plan terminations, partial plan terminations, changes in laws or regulations) may be paid at the market value of the fund s securities, which may be less than your book value balance or may restrict withdrawals in these events. Additional risk information for this product may be found in the prospectus or other product materials, if available. Certain investment options offered by your plan (e.g., money market funds, short term bond funds, certain asset allocation/ lifecycle funds and brokerage window) may be deemed by the Contract issuers to "compete" with this fund. The terms of the Contracts prohibit you from making a direct exchange from this fund to such competing funds. Instead, you must first exchange to a non-competing fund for 90 days. While these requirements may seem restrictive, they are imposed by the Contract issuers as a condition for the issuer s promise to pay certain withdrawals and exchanges at book value. Someone who seeks a slightly higher yield over the long term than is offered by money market funds, but who is willing to accept slightly more investment risk. Someone who is interested in balancing an aggressive portfolio with an investment that seeks to provide a declared crediting rate that is reset on a periodic basis. The investment option is an annuity. The fund is managed by New York Life Insurance Company. This description is only intended to provide a brief overview of the fund. This option is not insured by the FDIC or the Federal Government. New York Life Insurance Company provided the description for this investment option. Fidelity Brokerage Services LLC and New York Life Insurance Company are not affiliated. This investment option is not a mutual fund. 10
