2 Introduction This registration form records the data necessary for the registration of a company as a provider of telecommunication services. The most important legal foundations are listed in the guide to the present form. In each case, the current versions of the cited laws and decrees are definitive. This form, the corresponding guide, the texts of laws and decrees, technical and administrative regulations and other important information can be found on the website of the Federal Office of Communications (OFCOM) at In so far as no prevailing public or private interests are compromised, OFCOM provides information on the names and address of the telecommunication services providers and on the telecommunication services and interfaces they provide. The telecommunication services provider declares that it consents to OFCOM publishing the information contained in this questionnaire. The telecommunication services provider is obliged immediately to report to OFCOM by means of this form any changes or enhancements to the services and interfaces provided or to the infrastructure it operates. 2/9

4 Part 1: register Note: Please read each question and the possible answers carefully and then cross each answer which is relevant to your company. Further explanations on completing the registration form can be found in the corresponding guide. Questions on basic characteristics of services Questions 1 to 6 serve to clarify whether the services you provide fall within the scope of the Law on Telecommunications and therefore whether these are telecommunication services or not. 1. By means of the services you provide, you enable third parties to transmit information using telecommunications techniques, in that you independently, i.e. under your own name, sell telecommunication services and assume responsibility to your customers for the telecommunication services provided. 6 Part 2: Registering telecommunication services This part of the registration form is used to register telecommunication services. This information is published on the internet at Public telephone service 1. Voice transmission (telephone service) in real time with free origination and termination and single-stage call set-up a) voice transmission via a fixed-network connection (e.g. PSTN, ISDN) b) voice transmission, nomadic (e.g. VoIP via broadband connection) c) voice transmission, mobile (e.g. GSM, UMTS) Other voice transmission 2. 7 Part 3: Notification of subscriber interfaces of telecommunications networks (Network Termination Point or NTP) This part of the registration form must be completed separately for each interface (Network Termination Point) provided.
