1 POSITION DESCRIPTION / PERFORMANCE EVALUATION Job Title: LPN Prepared by: Date: Supervised by: Director of Nursing Approved by: Date: Job Summary: The LPN will assist with coordination of patient flow, telephone screening, and preparation of patients for examination and procedures and giving injections as directed. Plans and provides under supervision of a RN comprehensive nursing care including, but not limited to, executing physician's orders, dispensing and administering medications, and assisting physician in examinations and treatment of patients. Applies nursing and supervisory techniques required in the care, treatment and referrals of patients. Performs procedures such as intramuscular injections, blood pressure and CPR. Obtains and transmits urine, blood and other specimens for lab analysis. DUTIES AND RESPONSIBILITIES: Performance is clearly outstanding;performance is superior it far exceeds standards E = Excellent or expectations;performance is exceptional on a continuous basis. G = Good S = Satisfactory NI = Needs Improvement Performance generally meets or exceeds standards or expectations;attains all or nearly all of position objectives. Demonstrates Competency in the Following Areas: E G S NI U Provides nursing care to patients through use of the nursing process. Provides the assessment planning, implementation and evaluation of patient care. Assists in developing, implementing and evaluating a written care plan, including long and short term goals. Is accountable for all assigned patient care given during shift. Responsible for assisting in managing supplies and equipment. Participates in discharge planning using appropriate available resources. Involves the patient and/or family in developing the nursing care plan and in teaching. Demonstrates ability in patient assessment. Collects patient data as assigned. Makes observations of general physical and mental conditions of patients and signs and symptoms indicative of change or disease process. Keeps patient s age in consideration during the process. Reports/documents observations appropriately. Accurately communicates thru appropriate charting. Able to adapt to change in workload of patient assignments without interference to patient care. 1

Assists with planning and implementing the nursing care plan. Contributes to the development and/or revision of the nursing care plan, and documents such. Performs direct patient care as assigned to provide for the physical and emotional comfort of the patient in accordance with the nursing care plan. Reinforces, assists in patient/family teaching. Contributes to the evaluation of the response of the patient to the care plan and to nursing interventions and communicates this to the charge nurse. Contributes to the revision of the patient care plan according to the changing needs of the patient. Care plan is evaluated for currency at teach shift report. Knowledge of work/duties. Prepares and administers medications accurately. As authorized, hangs, monitors, and discontinues IV fluids and blood products accurately and according to policy and direction. Provides direct care to patients assuring safety and comfort. Performs treatments and procedures with attention to detail, accuracy, patient safety and comfort. Maintains a clean and safe environment for patients and other nursing staff. Assists in maintaining equipment and reports malfunctioning equipment to appropriate personnel. Provides basic life support in emergency situations. Is ACLS, NRP, PCEP trained. Attends and participates regularly in staff meetings and inservice offerings. Attends the minimum number of 8 staff meetings per year and as designated by DON. Reads synopsis of all staff meetings not attended. Actions show awareness of materials covered. Shows interest and willingness in planning and participating in inservices. Is an active member of the nursing team. Demonstrates understanding of emergency procedures, fire drills, evacuation procedures, and internal and external disaster plans and procedures. Functions in a calm manner during emergency and crisis situations. 2

Understands and functions within limits of State Nurse Practice Act and hospital policies and procedures. Maintains an effective working relationship wit health care team, patients, families and visitors. Establishes good rapport with patients, families, and visitors, respecting patient s right to privacy and confidentiality of information, follows HIPPA guidelines. Provides direction to patients, families and visitors in a courteous, precise manner. Responds to requests in a friendly, cooperative manner. Accurately communicates patient information to other nursing staff and medical staff. Is active member of ER team. Professional Requirements: E G S NI U Adheres to dress code, appearance is neat and clean. Completes annual education requirements. Maintains regulatory requirements, including all state and federal regulations. Adheres to the Group I level of HIPAA Minimum Necessary Standard when using, disclosing or requesting Protected Health Information (PHI). Reports to work on time and as scheduled. Wears identification while on duty. Attends annual review and performs departmental inservices. Works at maintaining a good rapport and a cooperative working relationship with physicians, departments and staff. Represents the organization in a positive and professional manner. Attends committee, CQI and management meetings, as appropriate. Resolves personnel concerns at the departmental level, utilizing the grievance process as required. Ensures compliance with policies and procedures regarding department operations, fire, safety and infection control. Effectively and consistently communicates administrative directive to personnel and encourages interactive departmental meetings and discussions. 3

5 Education/Experience Requirements: Graduate of an accredited school of nursing. Current licensure by the State of Oklahoma as a Licensed Practical Nurse. Current CPR, ACLS, & PALS required within 1 year. Ability to work cooperatively with medical staff, patients and families. Language Skills: Able to communicate effectively in English, both verbally and in writing. Skills: Demonstrates abilities in ER areas and OB as needed.
