1 Your Family, Your Way Overview CT Fertility offers diverse IVF, egg donation and surrogacy programs and cost options. The expanded 2013 lineup includes an overall 15% reduction in previous fees, the addition of several new low-cost shared egg donation plans, and our new Unlimited IVF Programs for egg donation and surrogacy which include unlimited access to all medical and laboratory fees for as many tries as you need to create your baby. Clients can now choose between single and unlimited cycle options; using an outside donor (often family or friend) or one who is matched and screened by CT Fertility; fresh or frozen eggs of various guaranteed quantities (5, 10 or 15); and even free embryos that other families donate. Our motto your family, your way boils down to giving our intended parents exactly what they need to create the family that is right for them, while reducing the cost and emotional burden of having many extra eggs or embryos which so often go unused. Unique features across all of the programs While adding new programs, we provide several unique features to help reduce process uncertainties and help control the overall costs the new Freeze & Hold option, Guaranteed Number of Eggs, Automatic Access to Back-up Eggs (at no additional costs), and Automatic Progressive Discounts features which CT Fertility is uniquely positioned to offer due to its highly successful Frozen Donor Egg Bank. The innovative Freeze & Hold option allows prospective parents to fertilize only a portion of the eggs they receive through their egg donation and freeze the rest until they know the results of the IVF treatment with the embryos they created. Later on they may decide to create additional embryos at markedly reduced cost with the same or different sperm source, or return the unused eggs for a refund. With our promise of a guaranteed number of eggs, if your donor does not yield the expected number of eggs (see attached table), and you run out of viable embryos before you get pregnant, CT Fertility will then supplement the difference (at no charge) to create additional embryos. These eggs can come either from our pool of cycling donors or our Donor Egg Bank. Automatic progressive discounts are offered on frozen embryo transfers to people who do not choose the unlimited transfers plans, as an alternative means to assure their peace of mind. Thus if they do not become pregnant on their first or second attempt, they are automatically offered a 50% discount on their next frozen embryo treatment, and subsequently get all their embryo transfers for free until they have a baby or run out of embryos.

2 Our Programs At $44,900, our Unlimited IVF with Egg Donation Package offers maximum certainty and flexibility below the price of last year s standard single-cycle option. With this new package, prospective parents pay a one-time flat fee that includes everything egg donor-related fees, all medications, and unlimited IVF and embryo transfers (fresh or frozen) until they have a baby. Note that there will be no additional restrictions to people who wish to only transfer one embryo at a time, so that parents who choose to be conservative because they wish to avoid twins are not penalized. This program involves the use of fresh eggs from any donor in our program, and therefore the donor pool is wider than the one available in the Egg Sharing or Egg Bank programs; and in this all-inclusive option we guarantee at least 15 viable eggs. In other words, if your donor does not produce the expected 15 eggs, and you run out of viable embryos before you get pregnant, we will supplement the difference (at no charge) with eggs from our pool of cycling donors or our Donor Egg Bank. If the egg donor produces more than 15 eggs, you will receive up to 20 eggs. If there are more than 20 you will have the option to purchase as many more as you like for an additional cost (which you can decide either on the day of the retrieval on which case the additional eggs can be fertilized along with the rest of your eggs, or after you have seen the result of your fresh cycle). With these advantages this package is ideal for two-dad couples or anyone wanting twins, with excellent potential for extra embryos for additional attempts or a sibling journey, and anyone who wants the very highest chances for success with the peace of mind of full cost control. Note that similar multiple cycle packages currently offered by other clinics never guarantee the number of eggs that you receive, and often do not include the full range of even the first round of donor-related costs (such as stipend, matching fees and insurance). Variations: Unlimited IVF Cycles with an Outside Donor: For a total of $34,900 clients can enjoy the benefits of the unlimited IVF and embryo transfers while using the eggs from an agency donor, family member or friend. Fee does not include egg donor stipend, matching fee or other agency fees. For a total of $59,900 the Unlimited IVF Package with a CT Fertility donor can also be supplemented with our Guarantee / Money Back option that appeals to those who are most risk averse because it refunds $30,000 if pregnancy does not occur. At $39,900 the Unshared Egg Donation Program is our most flexible, least restrictive and most empowering single cycle plan. It involves the use of fresh eggs from any donor in our program, and therefore the donor pool is wider than the one available in the Egg Sharing or Egg Bank programs. In this all-inclusive option we guarantee at least 15 viable eggs. If your donor does not yield the expected 15 eggs, and you run out of viable embryos before you get pregnant, we will supplement the difference (at no charge) with eggs from our pool of cycling donors or our Donor Egg Bank. 3 Variation: Unshared cycle with an Outside Donor: For a total of $27,400 clients can enjoy the benefits of the unshared program while using the eggs from an agency donor, family member or friend. Fee does not include egg donor stipend, matching fee or other agency fees. At $29,500 the Egg Sharing Program allows clients to reduce donor-related costs by purchasing 10 viable eggs from a cycling proven CT Fertility donor. Our shared program is unique in that it does not require the availability of another couple being co-matched to your donor because if no other match is made, we can freeze the rest of the eggs for our Egg Bank. This program is less costly than the unshared program and only slightly more expensive than the Egg Bank Program because it gives you more donors to choose from, and it is likely to result in the storage of more high-grade embryos for later use. Includes all donor fees, medications, IVF and one fresh transfer. Progressive discounts are available for subsequent frozen embryo transfers if needed. This program is suitable for budget-conscious clients, single dads or two dad couples where only one is the sperm source, or anyone preferring fresh eggs, wanting one baby, one time, with the potential for extra embryos. Variation: The Limited Share Program: For a total of $19,500 clients can enjoy the benefits of the shared program while receiving only 5 fresh eggs from a proven CT Fertility donor. This may be a good option for a budget-conscious family preferring fresh eggs, wanting one baby, one time. At $25,500 the Donor Egg Bank Program is our least expensive all-inclusive plan, and it involves the use of eggs that have already been harvested and frozen and are now available for immediate use. This program is possible because of a new and safe rapid freeze process called vitrification that enables eggs to be safely frozen. With this option we guarantee you will receive 10 viable eggs from donors that have a proven history of successful donations. It is the quickest plan to a baby because the eggs are immediately available and it eliminates the need to synchronize the donor and carrier cycles. Our Egg Bank program has been extremely successful, with over 50 babies born, and pregnancy rates that closely match our success with fresh eggs. Includes all donor fees, medications, IVF and one fresh transfer. Progressive discounts are available for subsequent frozen embryo transfers if needed.
