3 OVERVIEW OF OTSUKA GRANT MANAGEMENT SYSTEM WHAT IS THE PURPOSE OF THE OTSUKA GRANT MANAGEMENT SYSTEM? The Otsuka Grant Management System (GMS) is a web-based application that supports the Educational Grant and Charitable Contribution Request review process. Once registered in the system you will be able to request either grant type on behalf of your organization. Otsuka will only consider grant requests submitted through the GMS. GRANT REQUEST PROCESS WHAT IS THE GRANT REQUEST SUBMISSION PROCESS? There are four primary stages in the Otsuka request submission process that will serve as primary topics in this user guide. The process includes: Getting Started Request Submission Contract Execution Outcomes & Reconciliation HOW DO I ACCESS THE OTSUKA GRANT MANAGEMENT SYSTEM? STEP 1. Navigate to the following address: STEP 2. Select Click Here at the top of either the Healthcare Related Charitable Contributions or Independent Medical Education Grant Requests banners to be directed to the GMS system. 3 P a g e

4 STEP 3. If you have already registered, login to GMS by entering your address and password and click the double arrows. If you have not registered, please do so by consulting the registration user guide. Click to Register GETTING STARTED WHAT IS MY HOME PAGE? After successfully logging in, you will be brought to your home page. Your home page includes the sections listed in the table below and displayed in the subsequent screen shot. 6 WHAT IS MY INBOX? Your inbox, a section of the homepage, displays previously submitted or initiated requests. The columns in your inboxes may be filtered. STEP 1. From your Requestor Home Page, click Submit New Request, and you will be directed to the Request Type Selection Page. 6 P a g e

7 STEP 2. Click Medical Education to begin the creation of a Medical Education Grant Request. The system will display the Medical Education Instructions page. Read the instructional text carefully in case the educational request submission process has changed. STEP 3. Review the request completion instructions to ensure you are applying for a grant for which you are eligible. Click Proceed to continue with your Education Grant Request. 7 P a g e

9 HOW DO I COMPLETE AN EDUCATIONAL GRANT REQUEST? There will be seven tabs on the Medical Education form that you will complete: General Information Grant Information Delivery Format Budget Document Uploads Accreditation Details Authorized Signer and Payee The following links are available within each tab to help you navigate between tabs. Action Description Saves all entered form information and brings you back to the previous tab/page Validates and saves all entered form information and brings you to the next tab/page Saves all entered form information and brings you back to your home page NOTE: You may continue your request submission at any time by clicking Please Complete Request located in your inbox Cancels the request submission completely STEP 1. Complete the first tab of the request form: General Information. Complete all required fields [marked by red asterisks (*)] and click Save and Proceed to Next Step to go the next tab. Some fields will be required/only visible dependent on your responses to other questions asked during the application process. NOTE: Any required fields not filled out will trigger error messages that will prevent you from moving to the next section. 9 P a g e

10 STEP 2. Complete the second tab of the request form: Grant Information. Complete all the required fields [marked by red asterisks (*)] and click Save and Proceed to Next Step to go to the next tab. 10 P a g e

11 STEP 3. Complete the third tab of the request form: Delivery Format. Complete all required fields [marked by red asterisks (*)] and click Save and Proceed to Next Step to go to the next tab. All activities related to the Medical Education Request can be entered here: 1. Select a Delivery Format Type, fill in all the related required activity fields and the expected audience group to be present at those activities. NOTE: You may add additional audience types by clicking Click here to add another audience. 2. After completing all required fields, click Click here to save Activity to save all entered information. NOTE: You may add additional activities by clicking Click here to add another activity. 11 P a g e

13 Associated Questions for Web Delivery Formats: Associated Questions for Enduring Material Delivery Formats: STEP 4. Complete the fourth tab of the request form: Budget. A summary page will show the Program Budget Amounts and Requested Amounts that you entered on the General Information Tab. NOTE: In order to proceed past the budget tab, the Total Program Budget and Total Requested Amounts from Otsuka entered into the budget must match the amounts as entered earlier in the General information Tab. If there is any discrepancy, the system will display an error message and you will need to update the budget appropriately. You will have the opportunity to upload your program budget in addition to completing the standard template; however, the data entered into the system will be the primary data reviewed. 13 P a g e

16 Tab 7: Fee for Service Tab 8: Other Comments are required for entries on the Other tab STEP 5. Complete the fifth tab of the request form: Document Uploads. Here, you will upload all required documents as specified on this tab. NOTE: If you have uploaded the wrong document, click Clear to remove the selected document and upload again. If your W-9/W-8 is not up to date, select No and provide an updated W-9/W-8 form 16 P a g e

17 STEP 6. Complete the sixth tab of the request form: Accreditation Details. If your organization is the accreditor of the program, select the accrediting bodies and upload accreditation documents for each. If your organization is accredited but is not the accreditor of the program, enter the accredited organization associated with the accredited program. You must click Add Program Accreditor to save the information entered. Note: The accreditor entered here will become a signatory to the Letter of Agreement. They will be sent an upon submission to set up their own accounts in the GMS system. 17 P a g e

18 Similar organization information will need to be entered for third party implementer/educational partners. Lastly, you will need to agree to the certification by checking the box at the bottom of the tab. STEP 7. Complete the seventh tab of the request form: Authorized Signer and Payee. The first section will require you to confirm the proper authorized signer information. The name and address will be pulled from registration so, if the information is not correct for this request, click Add Authorized Signer and enter the appropriate information. 18 P a g e

19 After entering the Authorized Signer Information, click Save and the new information will be included in the dropdown menu. After confirming the authorized signer information, ensure the Payee Information is correct and click Save and Proceed to Next Step to complete the final tab of the request form. STEP 8. A read-only view of the entire request form will display with the information that you previously entered. Review the request summary and if any item needs to be edited, click Edit in the associated section. This will bring you back to the appropriate tab to update the proper fields. If your application is correct and complete, read and accept the compliance commitment certification, and click Proceed to complete the request submission process. 19 P a g e

20 After you successfully submit the request form, a confirmation page will display. A confirmation will also be sent to you. Click Proceed to return to your Inbox. CREATING AND SUBMITTING A NEW CHARITABLE CONTRIBUTION REQUEST HOW DO I CREATE A CHARITBALE CONTRIBUTION REQUEST? STEP 1. From your Requestor Home Page, click Submit New Request, and you will be directed to the request Type Selection Page. STEP 2. Click Charitable Contribution to begin the creation of a Charitable Request. The system will display the Charitable Instructions page. Read the instructional text carefully in case the contribution request submission process has changed. 20 P a g e

21 STEP 3. Review the request completion instructions to ensure you are applying for a grant for which you are eligible. Click Proceed to continue with your Education Grant Request. HOW DO I COMPLETE A CHARITABLE CONTRIBUTION REQUEST? There will be three tabs on the Medical Education form that you will complete: 21 P a g e

25 STEP 4. A read-only view of the entire request form will display with the information that you previously entered. Review the request summary and if any item needs to be edited, click Edit in the associated section. This will bring you back to the appropriate tab to update the proper fields. If your application is correct and complete, read and accept the compliance commitment certification, and click Proceed to complete the request submission process. After you successfully submit the request form, a confirmation page will display. A confirmation will also be sent to you. Click Proceed to return to your Inbox. HOW DO I RESPOND TO A REQUEST FOR FURTHER INFORMATION? 25 P a g e

26 In the event that Otsuka requests additional information or clarification during the review process, you will be notified by and your request will be available for you to edit. STEP 1. Login to the Otsuka GMS and navigate to the request in your inbox. A request that requires your updates will have a status of Requestor RFI. STEP 2. Click Please Submit Additional Information. The request form will display. You will only be able to update the fields and questions where Otsuka requests additional information. NOTE: No other fields will be editable. Update Field Uneditable Field STEP 3. Update all enabled fields selected for RFI, navigate to the request review page and after you confirm that all fields have been updated as needed, click Proceed to re-submit the updated request. GRANT PROCESS FOLLOWING APPROVAL HOW DO I SIGN A LETTER OF AGREEMENT? 26 P a g e

27 Once a request has received final approval, the grant coordinator will send a notification to you informing you to sign a Letter of Agreement ( LOA ). STEP 1. Login to GMS and locate the Action Required Column of the request and click Please Submit Letter of Agreement. STEP 2. After you click Please Submit Letter of Agreement, the LOA will display. Click to sign the LOA STEP 3. Read the Letter of Agreement. STEP 4. Click Approve or Revision Needed after reviewing the LOA. Approve: Clicking this button will capture your esignature which consists of your IP address, address and the time/date stamp. You will be navigated back to the inbox and the request will enter status Pending LOA Review. Revision Needed: Clicking this button will navigate you back to the inbox and the request will enter status Pending LOA Revision. The OAPI Grants Office will then initiate a manual Letter of Agreement process offline. HOW DO I REQUEST A SCOPE CHANGE? Once your request has been fully approved and funding has been released, you have the ability to request a scope change if aspects of your program have changed. This is done through a link in your inbox. STEP 1. Login to the Otsuka GMS and navigate to request for which a scope change is necessary. 27 P a g e

28 STEP 2. Click Submit Scope Change Request. You will be navigated to the Scope Change Completion Instructions page. STEP 3. Read the completion instructions carefully to ensure you are completely informed of all the stages of the scope change request process. Click Proceed. HOW DO I COMPLETE A SCOPE CHANGE REQUEST? There will be three tabs on the Scope Change form that you will complete: General Information Delivery Format Budget The following links are available within each tab to help you navigate between tabs. Action Description Saves all entered form information and brings you back to the previous tab/page Validates and saves all entered form information and brings you to the next tab/page Saves all entered form information and brings you back to your home page NOTE: You may continue your request submission at any time by clicking Submit Scope Change Request located in your inbox Cancels the request submission completely STEP 1. Complete the first tab of the request form: General Information. Update any field necessary and click Save and Proceed to Next Step to go the next tab. If you do not wish to update any field, you may simply click Save and Proceed to Next Step to move to the next section of the request form. NOTE: The fields of the scope change request form will be prepopulated with the approved information from your original request/previous scope changes. Any required fields not filled out will trigger error messages that will prevent you from moving to the next section. 28 P a g e

29 STEP 2. Complete the second tab of the request form: Delivery Format. Update any field necessary and click Save and Proceed to Next Step to go the next tab. If you do not wish to update any field, you may simply click Save and Proceed to Next Step to move to the next section of the request form. NOTE: For a detailed, step-by-step breakdown of the different elements of the delivery format page and how to complete the tab, please review the How do I complete an educational grant request? section. 29 P a g e

31 STEP 3. Complete the third tab of the request form: Budget. Update any field necessary and click Next Section to go the next tab. If you do not wish to update any field, you may simply click Next Section to move to the next section of the request form. NOTE: For a detailed, step-by-step breakdown of the different elements of the budget section and how to complete the tab, please review the How do I complete an educational grant request? section. NOTE: Updates made to the total program fees or the requested amount from Otsuka on the general information tab need to be reflected in the budget section. Any required fields not filled out will trigger error messages that will prevent you from moving to the next section. STEP 4. A read-only view of the entire scope change request form will display with the information that you previously entered. Review the request summary and if any item needs to be edited, click Edit in the associated section. This will bring you back to the appropriate tab to update the proper fields. If your application is correct and complete click Submit to complete the request submission process. After you have successfully submitted the request form a confirmation will also be sent to you and you will be returned to your Inbox. You will be notified of a decision upon review by the Grant Review Committee. If approved, you may or may not be required to sign a new Letter of Agreement depending upon the requested changes. This will be communicated with the decision. 31 P a g e

32 HOW DO I SUBMIT OUTCOMES? Once your request has been fully approved and funding has been released, you will be able to submit Outcomes for the activities outlined in your request submission. This is done through a link in your inbox. STEP 1. Login to the Otsuka GMS and navigate to request for which outcomes will be submitted. STEP 2. Click Please Submit Outcomes. STEP 3. Complete the Outcomes Level 1 form. NOTE: Outcomes Level forms available depends on the level of expected outcomes originally entered at the time of request submission (e.g. If you only indicated Outcomes Level 3 in your request submission, then only Outcome Levels 1,2, and 3 will be available for submission). 32 P a g e

33 STEP 4. Complete the Outcomes Level 2 and associated Outcomes Level 2 Results form (if applicable). If outcome level 2 was achieved select Yes and example survey questions will populate at the bottom of the page. Select those that most closely matched those asked in your activity and click proceed. 33 P a g e

36 STEP 6. Complete the Competencies field questions and click Proceed to complete the outcomes submission process for Otsuka review. NOTE: The list of competencies displayed will reflect those entered during request submission. 36 P a g e

38 FINANCIAL RECONCILIATION WHAT IS FINANCIAL RECONCILIATION? After a fully approved Medical Education Request has been executed and the program event date has passed, you will be asked to reconcile the request by providing the actual attendance, financial actuals and other figures. WHAT DO I SUBMIT FOR FINANCIAL RECONCILIATION? You will receive an after the program end date asking you to login to GMS and reconcile your grant request. STEP 1. Login into your account and click Please Reconcile Budget and Attendance link found in the actions column of the request to be reconciled. The Reconciliation Form will display starting with the Delivery Format Reconciliation page. 38 P a g e

39 STEP 2. Complete the Delivery Format Reconciliation section. Click pencil icon to enter the actual participation of the program by updating the CE/CME Credit Hours for Category, # of invitations Actually Distributed, # of Learners, and # of Learners Receiving Credit in the audience information table. Click the disk icon when complete and your entries will be saved. Then click Save and Proceed to Next Step to navigate to the next page of the reconciliation. 39 P a g e

40 STEP 3. Complete the Financial Reconciliation Form. This section will allow you to enter in financial information and certify that the funds received from Otsuka were used for the activity detailed in the original grant request. 40 P a g e

41 STEP 4. Complete the Reconciliation Budget section. The budget section will be identical to the budget template in the original grant request form; however you will have to enter your budget actuals for the program. Note that your original budget values will not be displayed. Any line items that you entered in the budget during request submission will need to have an updated value entered for actual program fees and actual total dollar amount used from Otsuka. 41 P a g e

42 NOTE: If no funds were used for these line items, enter a $0.00 value. When the financial reconciliation template is complete, the system will automatically verify that the sum of the Total Actual Program Fees and the sum of the Actual Total Used from Otsuka budget line items are equal to the actual values entered on the Reconciliation Page. Once verified, you will be brought to the Reconciliation Review Page. STEP 5. Complete the Sunshine Act Reconciliation section. Sunshine reconciliation is required to establish whether your program is exempt from reporting according to CMS standards and, if not, to collect information on payments or transfers of value that are reportable to the federal government under the Physician Payment Sunshine Act. Below is a brief guide to completing the different elements of the Sunshine reconciliation; however, the page has been automated in such a way that you will only be required to report payments if your program is found to be non-exempt. 42 P a g e

43 1. Answer these questions to determine if your program is exempt according to CMS standards. 2. Answer these questions to establish what transfers of value took place. 3. If your program is non-exempt and value transfers took place you will be asked to enter each recipient here. 4. For each recipient entered above you will need to enter one or more payment or transfer of value. 43 P a g e

44 STEP 6. Submit the Reconciliation. Upon completion of the Reconciliation Delivery Format, Reconciliation Page, Reconciliation Budget, and Sunshine Reconciliation a final review page will display where you may review all reconciliation information you entered. NOTE: You also may edit any of the sections by clicking Edit on the section you wish to edit. Upon verification that all reconciliation information is complete, click Proceed. This will submit the reconciliation form to Otsuka for review. REFUNDS In the event that there are unused funds remaining after the completion of the activities, a refund process will be triggered by the GMS. You will receive an notifying you of the refund request, wherein which you will be requested to send the refund amount to Otsuka at the address specified in the . No system action will be required.
