2 FUTURE CARE COST ASSESSMENTS (Life Care Plans) Our Future Care Cost Assessment, also known as a Life Care Plan, is a comprehensive detailed assessment that outlines a disabled person s current and future needs and quantifies the costs related to these needs over the individual s entire life span. All evaluations are completed by Certified Life Care Planners and can be completed for individuals who have experienced catastrophic injury, have chronic health care needs or require confirmation of their service requirements following a personal injury. Our assessments are completed with an emphasis on assisting an injured person to maximum independence, preventing functional deterioration and providing replacement services for those tasks the injured person is no longer able to complete. Our reports provide detailed information regarding: Medical expenses All professional services Attendant care Aids to daily living Home modifications Features of our Plans Medication/pharmaceuticals Accommodation Transportation Education Vocational needs Tailored to the client s unique current and anticipated needs throughout his/her life expectancy. Provide a road map to assist clients and their families to obtain needed goods and services Completed when the individual has reached maximum medical recovery and when all appropriate medical and therapeutic assessments have been completed. Our Approach We provide a balanced and impartial document supported by Medical Specialists and is collaborative involving all members of the client s healthcare team. Our Reports summarize a client s future medical needs and expenses and become a valuable tool used during file negotiations. Based on clinical evaluation in residence as that is medically validated by the Omega Medical Clinical Team, the Report synthesizes valuable medical and expense information from all data sources, including medical documents and quantifies recommendations to establish a comprehensive life care plan. 3 OCF-19 APPLICATION FOR DETERMINATION OF CATASTROPHIC IMPAIRMENT The completion of the Application for Determination of Catastrophic Impairment (OCF-19) is restricted to medical doctors and psychologists. At Omega Medical, we not only have the knowledgeable and experienced assessors representing the required specialties but the entire process is overseen by Dr. Harold Becker, a leader in the field of catastrophic impairment determinations. 6 MINOR INJURY GUIDELINES IMPAIRMENT REVIEW Omega Medical Associates provides a full clinical evaluation to determine if your client s injuries do not fit within the Minor Injury Guideline (MIG). Our evaluations provide the necessary medical information required with any OCF-18 Treatment and Assessment Plan with respect to disability, treatment and attendant care needs. Our clinical assessments identify impairments that are outside of the MIG including: Whiplash associated disorder with neurological signs Fracture and/or dislocation of the spine Complete tear to tendons and ligaments Complete dislocations of a joint Should your client have a pre-existing medical condition that complicates their rehabilitation and functional restoration under the limited treatment offered by the MIG, and prevents them from achieving their maximal recovery, our evaluation can assist with determining the appropriate course of treatment and supplying the compelling evidence required.
