1 Early Intervention in Psychosis (EIP) Information for Carers, Families and Friends

2 Welcome to the Early Intervention Service We understand that having involvement with mental health services can be a confusing time for families and carers so we would like to introduce you to our team and explain how Early Intervention can help. What Is Early Intervention In Psychosis? Early intervention helps people to recover from psychosis by: 1. identifying the illness early on, 2. providing helpful treatment and support at the beginning of the illness and 3. working to help you stay well. We aim to identify possible causes of psychosis as early as possible to stop the illness from getting worse. This should lower the time it takes to recover and get back to a normal routine which in turn lessens the strain on the Service User and their family. Before we explain more about EIP it would help to know more about psychosis.

3 Psychosis What Is Psychosis? Psychosis is a general term used to describe a number of symptoms. What Are These Symptoms? Confused thinking e.g. difficulty concentrating, racing thoughts False beliefs / delusions e.g. paranoia, suspiciousness Hallucinations e.g. experiencing things that are not really there, such as hearing voices, smelling or seeing things Changed feelings e.g. mood swings, depression, anxiety Changed behaviours e.g. sleep pattern, appetite Some find their symptoms difficult and frightening while others are less worried by their experience. Symptoms are sometimes grouped into positive symptoms (hallucinations etc) and negative symptoms (depression). What Can Cause Psychosis? Factors that contribute to psychosis can be environmental, physical, social and psychological. For many people it is not one single factor, it is often a series of events that have eventually triggered the illness. Can Drugs And Alcohol Affect the Illness? Yes. Taking drugs or drinking alcohol could make some people more vulnerable to developing psychosis. It may also worsen symptoms. How Can Psychosis Develop? The pathway through psychosis will vary from person to

4 person. The illness can appear suddenly, over the course of a few days or slowly over a much longer period of time. It is best to seek help as soon as possible after symptoms or changes are first noticed. Medical terms used to describe the possible phases of the illness include: Prodromal the period before psychosis is evident where some changes begin. Acute during this phase symptoms of psychosis emerge. Recovery This phase will vary from person to person. Many people will recover with the appropriate treatment. Not everyone s illness will follow these phases. Are There Different Types of Psychosis? There are a number of different types of psychosis. Some are: Bipolar Schizophrenia Drug Induced Schizoaffective Disorder Psychotic Depression Psychosis can happen to anyone. Like any other illness, it can be treated.

5 Early Intervention How Does Early Intervention Help? We work closely with the individual and other important people in their lives to provide a complete package of care designed to meet personal needs. Services could include: Education regarding signs and symptoms of psychosis Referral to specialist services, i.e. dieticians Social activities, either in a group or individually Help with housing and benefits Family Therapy / Behavioural Family Therapy Inpatient care Planning to help you stay well Appointments with a consultant psychiatrist Medication Help to access work or education Physical health and wellbeing checks There are many other areas of treatment, the use of which depend on your individual situation. Who Is The Early Intervention Service For? The Early Intervention service works with young people aged between 14 and 35 years who might be experiencing psychosis for the first time. The team will offer support for up to 3 years. What are the Benefits of Early Intervention? As a team we encourage full recovery and will work towards reducing the chance of getting unwell again. We aim to reduce the disruptions to relationships and life plans.

6 EIP aims to: Reduce disruption of activities (e.g. school, work) Reduce disruption of family and social relationships Reduce the chance of a hospital admission Support a fast and complete recovery. How Do You Access The Service? Anyone can make a referral to the Early Intervention Service. It could be by a GP, family member, friend, school, other mental health team etc. Someone can even refer themselves. Please see the back page for details how to contact us. What Happens Next? When a person is referred to our team we would firstly decide whether our team is the most appropriate for them. If we feel that the person will benefit from Early Intervention we will designate one member of staff who will then be the main contact for everyone involved throughout their care. This person is called the Care Coordinator. The Care Coordinator will arrange a time to come and introduce themselves and hear more about the difficulties that the individual is experiencing. This appointment can be held at their home or anywhere they feel comfortable. Family members, carers and friends can also be invited. During this meeting the individual and their Care Coordinator will start to create a Plan of how best to support them. The plan will then be reviewed regularly. Help for You We know that it is hard for you as a family member, carer, partner or friend of someone experiencing mental health difficulties. We are here to help and provide Carers Assessments which are designed to assess YOUR needs.

7 The North Essex Partnership NHS Trust The Trust has many different teams working together to provide the best care for people. Below is a flow chart to show you how these teams work along with each other. Community Care Adult Mental Health Services (ages years) Inpatient Care Crisis Resolution and Home Treatment (CRHT / ISS) is a short term community service aiming to help people through a mental health crisis offering treatment at home and preventing hospital admission. There are 3 teams across North Essex, and which team will depend on GP and area of residence. The Assertive Outreach Team (AOT) is a specialised community service which aims to provide intervention and care for those clients who find it difficult to engage and have a long term serious mental health illness. Criminal Justice Mental Health Teams (CJMHT) focus on helping people who are subject to criminal proceedings and/or have a forensic history. Community Mental Health Teams (CMHT) work with people in the community and provide services such as nurses, doctors, social workers, occupational therapists and support workers and also provide access to other services across the Trust such as psychology and family therapy. There are many teams covering North Essex and which team would depend on GP and area of residence. When a person reaches 71 they will be transferred to Old People s Services Admission to Adult Mental Health Wards may be required and there are hospitals covering the whole of North Essex, depending on area of residence and GP. These hospitals can provide around the clock care and access to services such as psychology, day activities, family therapy and others. Following discharge from hospital follow up arrangements will be made by one of the community care teams. Drug and Alcohol Services (CDAT, Changes, NEEDAS) offer help and support to people who are having difficulties because of their drug and alcohol use and who wish to have specialist help to change. Early Intervention in Psychosis (EIP) This is a specialised community service that offers help to people experiencing psychosis for the first time between the ages of There are 3 EIP Teams covering North Essex. Which team would depend on GP and area of residence. Consultant Psychiatrists work with specialist geographical areas of the Trust and can provide input at any stage of involvement with mental health services. The consultant allocated will depend on the age range, GP and area of residence. They will offer outpatient appointments of varying regularities depending on need.

8 The EIP Service in North Essex Staff EIP teams consist of different staff including: A clinical manager Team managers Community mental health nurses Psychiatrists Psychologists Social workers Support workers Occupational therapists Family therapists Administrators The Service also has access to other specialities such as: Day Unit Activities Cognitive Behavioural Therapy Behavioural Family Therapy Area Covered The Service consists of 3 teams together covering the whole of North Essex. West Harlow, Uttlesford, Loughton and Epping Central Chelmsford, Maldon, Witham and Braintree East Colchester and Tendring The team to which a person is referred depends ultimately on which GP practice they are registered with. If in doubt contact any of the teams for advice.

9 Mental Health Jargon When you first come into contact with mental health services you will notice lots of unusual words and abbreviations. 10 PALS SMI SS SW Patient Advice and Liaison Service Serious Mental Illness Social Services Social Worker Unusual Names and Phrases Acute Addiction Advocacy A phase of illness when most symptoms may be present and active. Physical and/or psychological dependence. The process of actively supporting an individual toward getting their needs and wants met as appropriate. Care Coordinator A member of staff who will be the main contact throughout a person s involvement with mental health services. Care Plan CareBase Carer Carers Assessment Clinical Psychologist Cognition Delusion This is a document that details a person s main problems and what they have decided to do to address them. A secure electronic data base which holds information about an individual s care. Term used to describe someone who provides support to someone who is experiencing any kind of illness. A way of assessing what a carer s needs are. A specialist in mental health problems who focuses on talking treatments and assessments. Another word for thought. An abnormal belief.

11 Family Therapy Hallucination Treatment that involves interaction with family members as well as with the individual. Something you can hear, see, touch, taste or smell that is not actually there. Mental Health Act Assessment This process is likely to take place to determine the best course of care when an individual is considered so unwell that they are unable to choose what is best for them. Prodromal Psychiatrist Psychotherapy Psychosis Psychosocial Service User A term used to describe the period before psychosis. A medical doctor specialising in mental health problems. A broad term applied to a variety of psychological approaches to the treatment of mental and emotional disorders. A group of symptoms that affect the mind. A term describing aspects of social and psychological behaviour. This is another term for Patient or Client.
