3 1 The Bachelor of Arts in Nursing Science The Bachelor of Arts in Nursing Science with the focus on Clinical Nursing Expertise and Family Health and Rehabilitation Nursing prepares the students, based on a scientific education, for future occupational fields to face new tasks in nursing practice. Concerning the focus of Clinical Nursing Expertise students are prepared for the exposure to complex tasks like the implementation of nursing diagnostics, assessment instruments, nursing process, care and case management, quality assurance instruments as well as the instruction, training and consultation of patients, their relatives and nurses. Future occupational fields imply the clinical and community based setting. Concerning the focus of Family Health and Rehabilitation Nursing students are prepared for similar tasks like for the clinical nursing expertise. They specialize for fields like public health services, acute and residential care of elderly people, rehabilitation setting and polyclinics of sports medicine. 2 The work placement The studies of the Bachelor of Arts in Nursing Science comprise a work placement regarding the development of a good clinical practice with the duration of 12 weeks. The students are prepared concerning the content and methods of the work placement. During the work placement the students are supposed to work on a research question or a question of interest, which should be based on an agreement between the cooperating institution and the university. It is intended to provide them an insight into the situation and demands of different working places. The students are supposed to get to know the specific problems and tasks of the particular occupational fields based on their technical and methodical skills and knowledge. They are challenged to gather, analyze and interpret the problems and tasks in order to develop solutions, which are to be presented and implemented in the nursing practice. 1
4 2.1 Aims of the work placement The work placement has the following intentions: Obtaining deepened professional knowledge about the organization, structure and functioning of a specific working field, Testing and implementing professional and methodical skills and knowledge, which were acquired during the studies, Obtaining relevant competencies in specific work settings, Developing questions from a research perspective with a high relevance for nursing practice, Fostering and promoting an exchange and transfer between the needs and realities of the facilities and the acquired knowledge of the university, Testing in research and science related context and developing an occupational orientation. 2.2 Two forms of work placements The work placement has the intention that students work independently in form of a small project on a research question, which arises from the practice field. The definition and organization of the project has to be coordinated by the institution, the student and the responsible tutor respectively authorised person of the university. The students should organize and prepare the work placement concerning their focus of interest and necessary methods in cooperation with the institution extensively on their own. They take responsibility for the results of their own defined activities, which are to be achieved in a limited time frame. They may chose between two forms of work placement the implementation project or the research project. In the context of the implementation project students should develop a project, which has to be implemented and evaluated in the practice field in cooperation with the institution. This approach enables the students a closeness to the practice field by inclusion and use of their competencies in the analysis of problems, development, implementation and evaluation of new concepts. For example, the implementation of an existing concept is possible as well as the development, testing and evaluation of an assessment instrument. In the context of the research project students explore for the practice clinically relevant questions with scientific research methods. Goal-oriented investigations, which are requested by practice facilities, are therefore appropriate. Students may also participate in current research projects of the university or other research institutions. 2
5 2.3 Possible institutions and fields of activity The work placement can be accomplished in the national or international context in different fields of activities and institutions of the nursing and health care system. This implies shortterm and long-term care facilities, the community based and acute care setting in the areas of diagnostics, therapy, rehabilitation, prevention and health promotion. Relevant facilities are for example hospitals, rehabilitation clinics, nursing homes, home based care, practices of doctors, physiotherapists, other therapists, counselling, self-help institutions and sport clubs. Other appropriate institutions for the work placement belong to the area of research and teaching and comprise facilities of the secondary and tertiary educational sector as well as occupational unions and associations. To the potential institutions for the work placement belong in the same way public institutions of the nursing, social and health care setting like public departments and authorities, health insurances, health organizations, schools, kindergardens, prisons etc. 2.4 Possible issues to work on Manifold topics and issues of the nursing and health care field are suitable for an implementation and research project. For example it is possible to work on the implementation of an assessment instrument or writing of a nursing expertise in the field of diagnostics. Concerning the field of interventions the development of consulting and training concepts or implementation of case management in nursing can be ranked among the research options. Evidence based nursing projects are also well conceivable, for example the implementation of guidelines. 2.5 Legal issues and duration of the work placement The work placement contains a practical period of 12 weeks respectively 450 hours in total. It should start on August, 1st each year and should be finished by December, 31st in the same year. Within that period of time the work placement has to be completed. The precise temporal organization of the work placement depends on the conditions of the particular institution. 3
6 The work placement corresponds to a short-term training or employment relationship between the student and a practice institution (e.g. enterprise, authority, association, federation etc.). An agreement of the work placement determines the mutual rights and liabilities. The agreement should be executed as triplicate for the institution, the student and the authorised person at the university. A final report about the work placement has to be written by each student. After a concluding discussion with the authorised person for the work placement at the university the final report should be made available to the practice institution. After the completion of the work placement the student receives a confirmation about the participation from the practice institution. Besides, the practice institution hands out a certificate about the work placement to the student. The work placement is acknowledged by the university after the successful participation in the work placement including the preparation and post-processing as well as the proficiency certificate that has to be passed. 3. Realisation of the work placement 3.1 Preparation for the work placement The project preparation starts in the fourth term with a project seminar. Besides the preparation through courses the staff members of the study courses are at the students disposal for additional questions. In the courses the students prepare their work placement as regards content and methodology (amongst others they will be trained in techniques of project management). If the students carry out a research project a study protocol of 3 to 5 pages has to be developed. In this protocol that is valid as a preliminary test the students plan and document their project. The protocol should furthermore contain a systematic literature research, the development of research questions, hypotheses and aims as well as the selection of appropriate methods for the realisation, analysis and evaluation of the project. Besides the study protocol the students have to forward the formal information in a written form to the practical institution (institution, contact partner, etc.). If the students carry out an implementation project they also have to develop a study protocol of 3 to 5 pages. In this protocol that is valid as a preliminary test the students describe the approach of the implementation of an intervention or other. This could be for instance the 4
7 description of the performance of a training course. Besides the study protocol the students have to forward the formal information in a written form to the practical institution (institution, contact partner, etc.). The study protocol and the formal information concerning the practice institution have to be submitted by May, 1st each year to the authorised person at the university for the work placement. The agreement concerning the work placement with the practical institution can only be concluded after the acceptance of the preparation of the work placement by the authorised person at the university. Then the work placement can start. Concerning the work placements abroad individual solutions by agreement are to be found in order to start the work placement. The students have a free choice of the work placement. Normally they should choose one facility or institution, the choice of more than one should be an exception and can only be accepted on the basis of a reasonable proposal. The authorised person at the university, staff members and the office for students and work placement affairs at the university (Studien- und Praxisbüro) will support the students in search of a work placement by providing contacts, addresses and web sites. 3.2 Monitoring of the work placement During the work placement the students will be monitored in order to moderate organizational questions and processes concerning the practice as well as to discuss concepts that could be implemented. The practical experiences should be reflected on the basis of the scientific knowledge acquired during the studies. For arising questions or problems during the work placement contact partners from the university, authorised persons at the university and coordinators stand at the students disposal. The latter are also responsible for the solution of problems and conflicts between the students and the facility or institution Agreement with the institution For a successful graduation of the work placement and the acquisition of a practical qualification the following general conditions have to be guaranteed by the practical institution by contract: The student has to be provided with an appropriate workplace to implement the intended activities. Besides a systematic vocational adjustment and 5
8 integration into the team a qualified staff member should assure a comprehensive supervision of the student. 3.4 Evaluation of the work placement The intention of the evaluation is to reflect the experiences of the students conjointly in order to gain a mutual educative benefit and to transfer questions from practice to the studies and vice versa. The acquired experiences and reflections of the students that are documented in their final reports, presentations and/or publications should be communicated and forwarded to the following students in context of their preparation of the work placement. After the completion of the work placement first an evaluation seminar in form of a plenum will take place aiming at the reflection of the project results and the support of the theorypractice transfer. Within a public presentation involving the participation of fellow students, university staff members and members from practical institutions a forum for the exchange and discussion of ideas should be provided. A continued cooperation between the university and the institutions is intended to be encouraged and further developed. 3.5 Activity confirmation and acknowledgement of the work placement After the students have accomplished the obligatory courses for the preparation and evaluation of the work placement they are expected to write a report of 20 to 25 pages (without appendix). The report must be scientifically sound and has to be based on the study protocol, which was prepared in advance of the work placement. The report should contain the following structural characteristics: - background, literature review - purpose, objective - aim of the project, hypotheses - methods - description of the workings and proceedings of the project - analysis and evaluation of the results 6
9 - conclusion and discussion - reflection of the personal experiences. The acknowledgement of the work placement is to be carried out by the authorised person at the university after the revision and confirmation of the report by the practical institution as well as after the presentation respectively publication of the project. The assessment of the work placement corresponds to a module exam. After a final agreement on the final report at the university the report will be provided for the practice institution. If a work placement should not be accepted due to non-fulfillment or only partial fulfilment of premises by the university or the facility the authorized person at the university imposes additional conditions. On this basis a future acceptance should be made possible. 4 Work placement abroad The aim of the stay abroad is to experience various solutions in the field of health care and to gain insight in vocational training on an international and interdisciplinary basis in nursing. The contact to other cultures will broaden the social and intercultural competence of the students and relativise the own perspective upon their home country. The preparation of the work placement is carried out in close cooperation with the office for students and work placement affairs of the university (Studien- und Praxisbüro). It is advisable to complete the work placement by networking with an institution of health care abroad. By networking for instance with the German Society for Technical Cooperation GmbH (a non-governmental organization), universities or hospitals it can be ensured that the students will contribute to the sustainability of projects in their work placement. The students have the opportunity to complete their work placement in already pre-structured projects in the context of existing cooperations or to choose their own facilities or institutions in their preferred countries. The regulations, that are relevant for the national work placement, are transferred to the work placement abroad. The students have to proceed a project respectively to start the work placement with a special problem field and the related approach. The processed problem depends on the corresponding context and is coordinated and acknowledged with the authorized person at the university in the forefront of the work placement. 7
