The Switch Kit includes everything you need to make a quick change. How does it work? Simply follow the three simple steps below. Fill out the enclosed forms as needed and mail them to the appropriate bank, person or company. Or, stop by one of our full service locations, and we will help you fill out the forms and mail them for you! It s as easy as ! 1 Open your new account Complete the on-line checking account application. If you don t already have a savings account and password, simply click on new member. Or, you can stop by one of our seven convenient full service locations to open your new checking account. 2 Switch your automatic transactions Redirect your direct deposit payroll or retirement check. Fill out the CHNGE PYROLL DIRECT DEPOSIT form, attach a voided check from your new Envista Credit Union account, and give it to your employer s human resources department, the company handling your retirement or pension payments, or Social Security dministration. Fill out and mail or drop off a CHNGE UTOMTIC PYMENTS form for every automatic debit you want to have deducted from your new Envista Credit Union account. Here is a helpful list of the type of companies that may need this form: o Mortgage Company o Homeowner s Insurance o uto Insurance o Life Insurance o Utility Companies o Telephone Companies o Cable Company o Other 3 Close your old account Leave your old account open long enough to allow outstanding checks and automatic withdrawals to clear the account. Make sure there are enough funds in your old account to cover these transactions. When you are sure the old account is inactive, fill out the CLOSE CCOUNT REUEST form and mail or drop it off at your old financial institution requesting any balance from that account. 5 UESTIONS What do I do with my current checks? We will shred and recycle them. Bring them with you when you open your new Envista Credit Union account. Or, drop them by any one of our 6 full service locations. Do I have to buy more checks? Yes. You will need new checks for your new account and we will provide $10.00 towards your first new box of checks. What about my paycheck that is directly deposited into my old account? It s easy to change your direct deposit. Simply fill out the CHNGE PYROLL DIRECT DEPOSIT form and give it to your employer. Won t it be a hassle to switch my automatic withdraws? bsolutely not.
