3 A Message to Our Community Service to the community is at the very heart of University of Maryland St. Joseph Medical Center s (UM SJMC) mission to provide loving, compassionate care. There are many ways of doing this outside the medical center s walls, and I am proud to present this Community Health Improvement Report, which describes how UM SJMC goes the extra mile or, in our case, many miles to fill what otherwise would be a void in the health needs of our community. Our goal is not just improving the overall health of our community but also addressing the distribution of health services. That means providing access to minority pockets of the population who may be less healthy or have limited access to care. These determinants of population health include access, education and income. Mohan Suntha, MD, MBA For example, UM SJMC s St. Clare Medical Outreach program in Lutherville provides free, high-quality primary care to a large, hard-working, mainly Hispanic population with limited access because they are uninsured. This year, we hired a new medical director, expanding the capacity and hours to see patients, many of whom live nearby in Baltimore County. Achieving population health goals means reaching out to provide preventive screenings, education and primary healthcare for underserved, underinsured and at-risk populations. UM SJMC s Community Health Department not only offers free, innovative health education programs at the hospital but also travels to all 20 Baltimore County Senior Centers to support our aging community with bone density screenings and preventive health education. Susanne DeCrane We provided more than 3,000 free flu immunizations this year at the hospital and six community sites. In addition, the Community Health staff along with UM SJMC medical professionals performed free health screenings for hundreds of people. This convenient access positively impacts the health of our community. We look forward to continuing our mission of community service and enhancing the health and well-being of our community by building on the accomplishments that you ll read about in this report. 4 About University of Maryland St. Joseph Medical Center University of Maryland St. Joseph Medical Center (UM SJMC) is a 247-bed, Catholic acute care, non-profit regional medical center. UM SJMC is part of the University of Maryland Medical System, a multi-hospital system with academic, community and specialty services throughout Maryland. UM SJMC has a rich heritage of compassionate service and superb clinical programs. The medical center s primary goal is to build upon that heritage by enhancing our relationship among our community, employees and physicians. Located in Towson, a northern suburb of Baltimore County, UM SJMC serves patients in Baltimore County and from Franklinville and Westminster in the west, to Essex and Dundalk, to the Pennsylvania line up the I-83 corridor. UM SJMC s Community Health Department is committed to enhancing individuals quality of life at all stages through classes, programs, services and health screenings. A broad variety of health-related topics are offered to the community. Known for innovation, the Community Health staff introduced two new evidence-based multifaceted series programs to the community, Stepping On and Living Well. Stepping On is a seven-week series that is proven to reduce falls and build confidence among older adults. This fall prevention program covers strength and balance exercise weekly, and works with health and community safety experts from other UM SJMC departments. Living Well is a six-week series that encourages and engages participants in different techniques to self-manage their own chronic conditions by discussing topics such as: nutrition and physical activity, goal setting, problem solving, medication and pain management, communication, and evaluating new treatment options. At UM SJMC, we are committed to serving and advocating for those who are poor and marginalized and partnering with others to improve the quality of life and make a real difference in the communities we serve. 5 St. Clare Medical Outreach Program The St. Clare Medical Outreach Program, located in Lutherville and owned, operated and financed entirely by University of Maryland St. Joseph Medical Center (UM SJMC), has stayed true to its roots in Baltimore, where it began as a traveling medical outreach van for needy uninsured patients, then evolved into a primary care clinic within the Esperanza Center in Fells Point before relocating to Baltimore County. This year, the UM SJMC s St. Clare Medical Outreach Program s patient access and visits really grew with the addition of medical director Joanna Saba, MD. Her presence has doubled the hours a doctor is available and has worked with nurse manager Mary Jo Huber, RN, to offer more services. The program s mission is to provide high-quality, comprehensive primary care, serving culturally diverse people who are the working uninsured with no other access to care. About 99% of patients are Hispanic. Good health is very important to our hardworking patients. We want to help them continue to be productive members of the community, said Dr. Saba. Patients come from Baltimore City and across Baltimore County. The program is located on a major bus route. The St. Clare staff, many of whom are native speakers of Spanish, include a nurse manager, registered nurses, certified nurse practitioners, physician assistant, phlebotomist, pharmacy liaison who arranges medications for patients, health care coach who provides nutritional and lifestyle counseling, office staff and volunteers. We provide culturally appropriate care and understand the stresses and financial challenges of this immigrant population, said Dr. Saba. St. Clare utilizes medical students from the University of Maryland School of Medicine, who are enrolled in a Spanish-language medical program for the chance to use their Spanish-speaking skills in a clinical setting. A key service St. Clare provides is to refer patients to specialty care as needed, ranging from surgeries to inpatient care. Thanks to the generous medical specialty community at UM SJMC who donate their time to uninsured St. Clare patients, we are expanding referrals for a wide variety of care, said Dr. Saba. 6 Center of Clinical Excellence: Mentoring Future Health Care Professionals The Center of Clinical Excellence at University of Maryland St. Joseph Medical Center (UM SJMC) serves an important role enriching the education of future health professionals by facilitating exceptional preceptorship and hands-on training and mentoring those studying to become radiology technicians, physical therapists, occupational therapists, respiratory therapists, nurses, future nurse educators and nurse practitioners. Students come from all over the region, numbering more than 400 this year. We are a learning and developmental center; our staff are all master s prepared instructors. We have 59 contracts with universities, community colleges and professional schools in Maryland and Washington, D.C., says Paula Thrasher, RN, director of the Center of Clinical Excellence. Students spend a full clinical rotation of four to six weeks at UM SJMC, facilitated by the Center of Clinical Excellence and Learning Development, with master s students spending up to three months including a one-on-one preceptorship. The opportunity to benefit from the tutelage of UM SJMC clinical practice specialists and clinical developmental specialists is invaluable. For example, students use the skills lab, work closely with UM SJMC s Radiology department, Respiratory Therapy department, or are paired with physical therapy and occupational therapy preceptors. The center offers a unique health sciences experience for underprivileged, inner city children attending summer camp at St. Paul s Preparatory School. Every July, the staff holds a day-long program with lunch in collaboration with UM SJMC s Volunteer Services. This year, 25 campers attended. We want to broaden their horizons and even inspire health science careers, says Thrasher. The day included watching an instructor use a SIM man (patient simulator), with each student taking pulses and listening to heart sounds, practicing microscopic surgery, learning about hand bacteria hygiene and orthopaedics, plus experiencing the adventure of the Hoverjack for safe patient lifts. The center extends education into the community and provided Basic Life Support classes to numerous community residents and first aid classes to the Boy Scouts in Baltimore County this year. 7 Community Health: Prevention, Early Detection and Support Enhancing the community s health through screenings, immunizations and education is the calling of the Community Health department. WITH THE SUPPORT OF UM SJMC SPECIALISTS, COMMUNITY HEALTH HELD THESE FREE SCREENINGS IN 2015: Breast Cancer: 29 screened Prostate Cancer: 16 screened Cervical Cancer: 31 screened Stroke and Abdominal Aortic Aneurysms: 36 screened Varicose Veins: 29 screened Glaucoma and Cataracts: 19 screened Foot Assessments: 24 screened Skin Cancer: 32 screened PAGE 5 The department is well known for its annual free flu shot campaign, immunizing approximately 3,000 individuals this year. To provide easy access, four immunization events were held at University of Maryland St. Joseph Medical Center (UM SJMC) on weekends and evenings, and six events were held in the community, at Kenilworth Mall, White Marsh Mall, Greetings and Readings at Hunt Valley, St. Joseph Parish in Cockeysville, Our Lady of Grace in Parkton and the Towson YMCA. Participants are asked to contribute a canned good, resulting in thousands of canned goods that were donated to Our Daily Bread. Community Health staff traveled to all 20 Baltimore County Senior Centers, providing bone density screenings, bone health education and body composition analysis, which includes education on hydration. Older people tend not to drink enough fluids. Dehydration can put them at risk of imbalance, low blood pressure, fatigue and even passing out, explains Mary Jo Adams, BSN, nurse coordinator of Community Health. Known for innovation, the Community Health staff introduced two new evidence-based programs: Stepping On and Living Well. Stepping On is a seven-week series that is proven to reduce falls and build confidence among older adults. This fall prevention program covers strength and balance exercise weekly, and works with health and community safety experts from other UM SJMC departments. In 2015, Community Health offered one workshop serving 14 participants, who started with an average of three falls per person. After completing the program, though, they had a 100% confidence level in their ability to reduce their own risk of falling. Living Well is a six-week series that encourages and engages participants to self-manage their chronic conditions by discussing topics such as nutrition, physical activity, goal setting, problem solving, medication and pain management, communication and evaluating new treatment options. Community Health worked with several participants with chronic conditions such as diabetes, stroke, COPD, congestive heart failure, arthritis and cancer. After the workshop, participants had a 100% confidence level in their ability to feel more motivated to take care of their own health and work with their health provider COMMUNITY HEALTH IMPROVEMENT REPORT
8 Community Health Needs Assessment Beginning in 2012, University of Maryland St. Joseph Medical Center (UM SJMC), Greater Baltimore Medical Center and Sheppard Pratt Health System partnered to conduct a comprehensive Community Health Needs Assessment (CHNA) to evaluate the health needs of individuals living in the hospital service area within Greater Baltimore. The purpose of the assessment was to gather information about local health needs and behaviors. The assessment examined a variety of indicators including risky health behaviors (alcohol use, tobacco use) and chronic health conditions (diabetes, heart disease). The three hospitals contracted with an independent research and consulting firm to conduct research in support of the Community Health Needs Assessment. Qualitative data was collected and interpreted from reputable sources including the U.S. Census Bureau, Centers for Disease Control and Prevention, National Cancer Institute, and Maryland Department of Health and Mental Hygiene. Key informant interviews via phone, were conducted, analyzed and interpreted. Community engagement and feedback were an integral part of the CHNA process. Community input was collected through a variety of methods, including key informant interviews with community stakeholders and inclusion of community partners in the prioritization and implementation planning process. Public health and health care professionals shared knowledge and expertise about health issues, and leaders and representatives of non-profit and community-based organizations provided insight on the community served by the hospitals, including medically underserved, low income and minority populations. These results were collected via a questionnaire. The questionnaire focused on gathering qualitative feedback regarding perceptions of community needs and strengths across three key domains: Key health issues, health care access and challenges and solutions. Based on the feedback from the key informants, the following issues were identified as areas of opportunity for the local community: Access to health care Mental health Overweight/obesity Chronic health issues (diabetes, heart disease, cancer) PAGE 6 You can find the entire UM SJMC CHNA at stjosephtowson.com/communityhealth UNIVERSITY OF MARYLAND ST. 9 Community Health Needs Assessment Implementation Plan The completion of the CHNA (page 6) enabled University of Maryland St. Joseph Medical Center (UM SJMC) to take an in-depth look at its greater community. UM SJMC used the findings from the assessment to prioritize public health issues and develop a community health implementation plan focused on meeting community needs. The UM SJMC implementation plan consisted of six key priorities identified in the CHNA and the goals it set: Access to Health Care Bring screenings, vaccinations and health education to people who otherwise would not receive any health care interventions. Increase the number of patients receiving free flu shots to more than 3,000 Increase the number of patients receiving health screenings Diabetes Increase the number of patients served by Diabetes Education/Outreach Increase the emphasis in Diabetes Education on obesity education and high blood pressure awareness Maintain and enhance diabetes education for Hispanic patients at St. Clare Medical Outreach Reduce Substance/Tobacco Use and Abuse Increase the number of patients using screening and primary care outreach opportunities. Bring smoking cessation education to high-schoolage students Increase the number of people who participate in one of our multi-week, smoking cessation courses Heart Disease/Hypertension/Stroke Raise awareness of risk factors for stroke, stroke symptoms and appropriate response to symptoms. Maternal/Child Health Educate pregnant women and women of childbearing on of the dangers of poor nutrition, diabetes, high blood pressure, substance/tobacco use during pregnancy and babies born with a low birth weight. 10 Financial Assistance Policy Hospital care is available to all patients regardless of their race, color, national origin, age, gender or ability to pay. We recognize the financial needs of our patients and families who are unable to afford charges associated with medical care. The University of Maryland St. Joseph Medical Center s Financial Assistance Policy follows the federal poverty guidelines and is based on household size and income. We may consider other financial assets and liabilities of the patient and family when determining the ability to pay. The patient is responsible for providing information requested during the qualification process. Bills will continue to arrive until eligibility has been determined.
