The Vanity Number definition has been removed from Part III. Section 15 : Sheet 2 (Service Charges). The Vanity Number definition is now in Part 111, Section 6, Sheet 5 (Directory Listings). The Vanity Number charge has been removed from Part III, Section 15, Sheet 2.1 (Service Charges). The Vanity Number charge is now in Part III. Section 6, Sheet 13 (Directory Listings). The $25.00 per Vanity Number no longer applies and has been removed from the tariff. A monthly vanity number charge of $1.95 now applies and has been added to the tariff. All literature will reflect this new charge. Existing customers will be grandfathered in. Please see one (1) original and two (2) revised copies enclosed for your files. Reconnection (Restoration) Charge a, A Reconnection Charge is applicable for the restoration of service following a disconnect for nonpayment of such service. b. A Reconnection Charge does not apply for: (1) Number change made at the request of the customer. (2) Change of line assignment made at the request of the customer. (3) Change in central office switching features at the request of the customer. (4) Connection or rearrangement of an outside circuit, drop wire and/or protector due to a customer's service request. 5. Vanity Number Charge a. A Vanity Number Charge is applicable when a customer requests a specific number or a number other than the number automatically issued by a sendee order when the customer requests a number change, reconnection of sen-ice, or a connection of new sendee. The usual Sendee Order. Line Connection and Reconnection charges also apply (Part IIL Section 15. Sheet 2 (D) (1) (2) (3)). b. A Vanity Number Charge is also applicable when a seasonal subscriber requests to reconnect a specific previous number which was a full disconnect of seasonal sendee instead of a suspend during off season. This particular number can be reconnected (if available) as a Vanity Number. In this situation. Sendee Order charges and Reconnection charges also apply (Part IIL Section 15, Sheet 2 (D) (1) (3)). 6. On-Premise Visit Charge a. An On-Premise Visit Charge shall apply in those cases where a customer requests the Company to move or otherwise rearrange existing sendee drops due to customer's remodeling or other maintenance.
