2 Is Hip Replacement Surgery Right For You? If you have gradually stopped doing the things you enjoy or are adapting your life to cope with reduced mobility, hip replacement surgery may be appropriate for you. Fortunately, today s advanced techniques and technologies have revolutionized hip replacements. That means more patients can consider treatments at an earlier stage than they could in the past, potentially allowing them to return to their more active lifestyles. Anatomy of a Healthy Hip The hip joint forms where the top of the thigh bone (femur) meets the socket of the pelvic bone. The top of the femur is ball-shaped and fits snugly in the socket. The bones of the hip joint are covered by cartilage that protects the bones and allows easy motion. Pelvis Acetabulum Femoral Head Femoral Neck Femur
3 If your surgeon determines that your hip joint is not healthy, he or she will help you make the decision about whether a total hip replacement is appropriate for you. Surgeon Considerations: Bone Density Bone Defects Bone Shape Biomechanics Diseased Hip A hip joint may require replacement as a result of osteoarthritis, rheumatoid arthritis, avascular necrosis (AVN), or fracture.
4 traditional HIP REPLACEMENT In a traditional total hip replacement procedure, your surgeon will replace the diseased or unhealthy hip with an implant consisting of a stem, ball, liner and socket. During the surgery, the worn head of the thigh bone (femur) is replaced with a metal ball, or head, mounted on a metal stem inserted into the bone. A matching cup, or socket, is placed into the pelvis. The bearing, or point where the ball and socket rub together, plays a critical role in the durability of an artificial hip. It affects how well the implant will wear over time, how long it will last, and how stable it will be in your body. For many years, a tough medical-grade plastic, or polyethylene, was used in the bearings of artificial joints. Also important is the bearing s size. While traditional hip replacement utilized relatively small bearing diameters, it is now recognized that a larger ball offers greater range of motion and stability because it is more like the natural hip. Your surgeon will choose the optimum bearing surface for you. Bearing Socket Liner (polyethylene) Ball (small diameter) Stem
5 HIGH PERFORMANCE OPTIONS Hip Resurfacing An option for certain patients with strong bone density and structure, hip resurfacing may preserve bone by covering the worn hip joint surfaces with metal caps. The metal caps, placed over reshaped bone, create a metal-on-metal joint. As a result, the joint may experience less friction and less wear than some traditional hip replacement options such as metal-on-plastic. Hip resurfacing is a major surgical procedure that may require a larger incision than traditional hip replacement. For some surgeons, this procedure may also take more time to complete compared to a traditional hip replacement. Hip resurfacing can be more technically demanding than a traditional hip replacement, and your surgeon should have experience with this procedure. Fractures at the top of the femur (femoral neck) are a primary concern after a resurfacing procedure. There are no current data showing that hip resurfacing offers greater functional results compared to other modern hip replacement options. The hip resurfacing procedure has been studied in clinical trials over the past few years in the United States. While the short-term results are encouraging, additional long-term studies regarding safety and effectiveness will be important.
