1 COLLEGE APPLICATION PROCEDURES Student/ Parent Responsibilities: Application(s) If applying by mail, the student must make arrangements to secure the applications, complete the application and be aware of deadlines and fees. If applying online, follow the procedures indicated on the college website. Transcript Request: One Transcript release form must be signed by a parent. Subsequent transcript request forms for each college must be submitted to Guidance Services. Forms are available in the Guidance Services office or online. (Two weeks noticed is required) Essay (if required) The student should begin early and is encouraged to utilize connectedu, as well as, their English teacher. SAT Scores Using your collegeboard.com account, send your SAT scores to the college(s) you are applying to. Plan ahead, it could take a few weeks for the scores to reach the college(s). Letters of Recommendation: Teacher Letter Details can be found on specific Point Pleasant Borough High School Guidance Services forms available in the Guidance Services office or web site. Counselor Letter Parents are encouraged to complete the parent questionnaire which is available in the Guidance Services office or on line. School Secondary Reports (if applicable) Some colleges require counselors and or teachers to complete separate forms that are part of the application. The student must make these forms available to teachers and or counselors in a timely manner. Guidance Services Website Click on Schools, High School. Rather than delay the mailing of a transcript and possible adversely affecting you or your child s application, we are asking that this form be completed and returned to the Guidance Services Office. I am aware that this request must be submitted in writing. B. 3 REQUESTING A LETTER OF RECOMMENDATION The following suggestions will assist you and any teacher who may write a letter of recommendation for you: 1) Fill out the STUDENT INFORMATION WORKSHEET as completely as possible - not all sections may apply. 2) On a separate piece of paper, describe yourself. Your statement may include strengths, attitudes and/or skills which may help you be successful in the completion of your post-high school plans. 3) THINK OF A TEACHER WHO KNOWS YOU WELL, preferably an 11th or 12th grade teacher. Ask that teacher to write a letter of recommendation, for you, and supply them with a copy of the STUDENT INFORMATION SHEET and your written description of yourself. 4) Plan to meet your timeline for applying to colleges and allow the teacher(s) days for completion of the letter. 5) Be sure to inform the teacher of any deadline required for submission of the recommendation. 6) Do not assume the letter will be completed if you leave it in the teacher s mailbox; make personal contact with the teacher. 7) Ask the teacher to return the recommendation to the guidance services office for typing, if necessary. It will then be mailed with your application package. If the teacher wishes to mail the letter, provide an addressed, stamped envelope. 8) Write a short thank you to the teacher(s) who has supported you in this way. 9) Indicate which teachers you have requested write a letter, for a particular school, (on the application checklist) when you submit your application for processing.
