2 PURPOSE OF THE POSITION To support the development and delivery of clinical quality assurance and pharmacy service improvement projects by identifying and measuring gaps in best practice pharmaceutical care and devising strategies to ensure timely and optimal pharmaceutical care is provided to our patients. To establish and help with the implementation of therapeutic guidelines and clinical KPIs to reduce practice variation. Monitor and improve performance of clinical pharmacy service. Act as link between Pharmacy and the Medication Safety Service. *NB for roles with a joint clinical appointment refer to Clinical Pharmacist position description for the deliverables within that element of the appointment. 4 NATURE AND SCOPE OF RESPONSIBILITIES Key Accountability Clinical Quality Assurance Projects Standards / Achievements Lead and deliver clinical quality improvement projects associated with pharmaceutical care Plan and undertake clinical audits and practice based research to assist the pharmacy service in identification of clinical quality gaps and improvement initiatives relevant to evidence based best practice use of medicines Develop and implement clinical KPIs to enable performance monitoring and improvement Report regularly on progress against KPIs and project deliverables Collaborate with clinical pharmacists and the wider interdisciplinary team in designing and implementing clinical quality projects Facilitate and inform the development of best practice guidelines and support the integration of these into clinical practice Contribute to pharmaceutical costs analysis to inform therapeutic decision making in the context of therapeutic guideline development and review Research and evaluate literature on new therapies to provide a critical evaluation of the evidence to guide therapeutic choice Manage the pharmacy service intervention database, produce reports and monitor themes to inform clinical quality improvement projects Contribute to education and training to support clinical best practice associated with pharmaceutical care Prepare and communicate clinical practice issues as

Support pharmacy service performance improvement by contributing to audit and measurement against Pharmacy Service agreed standards of practice and legislative standards Support the development and establishment of standards and contribute to training and validation of staff Contribute to professional elements of staff orientation Provide feedback to team leaders and managers on deficiencies in staff performance and learning to facilitate performance improvement Contribute to the development and review of pharmacy KPIs and devise systems to measure, monitor and report on these Support the pharmacy management team to enable the appropriate prioritisation of service delivery by contributing to needs assessment Contribute to the development of service plan Effectively plan, develop, implement and manage projects assigned Collaborate with all pharmacy teams in designing improvement strategies Team Function as the link between Pharmacy Service and Medication Safety Service and contribute to development and implementation of medication safety projects that require pharmacy contribution Support the pharmacy roster (dispensary, on-call and weekend service) Promote team work practices and provide support to team to achieve team goals Assist the team leaders and clinical pharmacy manager in prioritising resources during staff

Is conversant with applications required for specific discipline/role. For example, PIMS, Concerto, Outlook, etc. Maintains own professional development by attending relevant IT educational programmes. Develop own IT skills pertinent to project and medication safety roles and share learning with others.
8 COMMUNICATION AND INTERPERSONAL SKILLS The Specialist Pharmacist Clinical Pharmacy Quality will be required to interact on a regular basis with a range of CMH staff members including: Management: Clinical Heads and Service Managers of the relevant service/clinical area, Pharmacy Services Manager, Clinical Pharmacy Manager, Dispensary Manager and Pharmacy Systems Manager. Pharmacy staff Clinical: Staff of the designated service Medication Safety Service Risk Management and Quality Assurance Service Patient Safety Service Externally there will be contact with: Other New Zealand Hospital Services, both primary and secondary care Other DHB hospital pharmacies NZHPA, Pharmacy Council of New Zealand, PSNZ Medsafe Situations may often call for tact and diplomacy and will require information to be handled in a discreet and sensitive manner. In conflict situations will be required to exercise sound judgement, negotiation and persuasiveness skills, toward facilitating a workable outcome. PROBLEM COMPLEXITY The Specialist Pharmacist - Clinical Pharmacy Quality will be regularly challenged by a range of competing issues. There will be a requirement to be able to prioritise issues and negotiate time frames while still providing a quality customer service and expert clinical care. The range of issues will be diverse and will require solutions customised to meet the needs of the client and other key stakeholders. Innovative solutions will be required. Deadlines will be met whilst maintaining accuracy and providing a high quality service. Failure to do so could impact on the organisation in terms of patient safety or cost effectiveness. The Specialist Pharmacist -Clinical Pharmacy Quality will refer to the Clinical Pharmacy Manager or the Pharmacy Services Manager for advice, if needed. SCOPE FOR ACTION Is empowered to make decisions or recommendations relating pharmacy service quality initiatives relevant to best practice pharmaceutical therapy and on projects undertaken relevant to this role. Are encouraged to use initiative and problem solving skills to develop innovative approaches to issues. Discretion is required to be exercised in releasing confidential information to the appropriate parties.
9 DIMENSION OF THE POSITION Will be responsible to the Pharmacy Service Manager for the achievement of agreed objectives and operates within the confines of Counties Manukau Health. Functional lines of reporting will also be to the Clinical Pharmacy Manager. POSITIONS REPORTING Directly Indirectly Nil Nil PERSON SPECIFICATIONS Minimum Qualification Degree qualification in Pharmacy Registered Pharmacist or eligible for registration Working towards a relevant postgraduate qualification Experience 4-5 years of experience in a hospital clinical setting Skills/Knowledge/ Behaviour Previous experience of audit, data analysis and or/ project work Evidence of practice at a senior/specialist level Evidence of ability to mentor and train others The ability to think strategically; develop and implement appropriate plans. Good communication skills (i.e. able to articulate advice and ideas both verbally and in writing) Good interpersonal skills Organised and motivated Preferred Postgraduate Pharmacy Qualification in Clinical Pharmacy Training in quality improvement processes Training in clinical education delivery Experience with project and change management Experience in quality improvement Demonstrated ability to show leadership in innovation Statistical skills Advanced computer skills
