An administrator at the agency will need to make a request to Therap s Regional Sales Representative to purchase the module for their agency. Contact a Regional Sales Representative at ext. 4. About Therap Services, LLC Therap Services provides secure, web-based documentation, communication and electronic billing services to over 1400 intellectual & developmental disability providers across the United States as well as for twelve state government I-DD systems of care. This includes a certified EHR, HIPAA compliant medicaid and private billing, service documentation and secure communication and data sharing between all stakeholders including families and self advocates. Therap s software-as-a-service solution is used in HCBS Waiver, ICF/IID and other long term support services to document residential and community based supports, employment supports, vocational rehabilitation, case management, incident reporting, management of staff training records and for electronic billing claim submissions directly to electronic billing. 3 Employer-Job Bank Employment Support staff will have the tools necessary to maintain an accurate history of the individual s work experience and job details. Administrative staff will be able to create, update or delete employers, contact persons and job templates which can in turn be viewed by support staff with assigned privileges. This ensures staff are accessing accurate information on a consistent basis, from any location. New Employer Details such as employer address and contact information are indicated in the New Employer section. Contact Person The key contact persons at the place of employment are listed and can be updated in the Contact Person section. Job Template Details of the job including title, wage and weekly hours, shift / schedule and skills needed are indicated in the Job Template section. 9:00 3

4 Employment / Volunteering History Form The Employment and Volunteering History form provides a comprehensive summary of the services in one place. It details assessments and accomplishments the individual has made throughout their work experience. The attachment feature allows for electronic storage of documents up to 3 MB per section, creating a complete record of the Employment History. The attachment feature allows for electronic storage and ecient viewing of the actual assessment. Career Development Plans The Career Development Plan is summarized in this section, along with electronic storage for ease of access. 7 Demographic Data, Safeguards and Guidelines for Individuals Receiving Employment Services Therap provides detailed demographic data including enrollment and admission notes, diagnosis information as well as guidelines, safeguards and medical and personal contact information for the individual being supported. These forms are accessible across programs to ensure efficient access of critical information. Individual Data Form The Individual Data Form displays detailed demographic information including program enrollment, medical and support guidelines, contact information and more. Emergency Data Form The Emergency Data Form summarizes essential information and replicates the To-Go form once done on paper. This is accessible remotely and easily printable as needed. Individual History Form The Individual History Form includes medical, social and family history information which may be considered critical knowledge across program settings.
