1 REQUIREMENTS FOR INDIVIDUALS REQUESTING TO BE RECOGNIZED BY THE CITY OF MESA AS A CERTIFIED BACKFLOW PREVENTION ASSEMBLY TESTER All items are required with initial application and with every backflow prevention certification renewal: 1. Complete a certified backflow prevention assembly tester application. 2. Enclose a copy of your certificate of competency issued by ABPA, ASETT, AWWA, or PIPE. 3. Enclose copies of your gauge calibration report. 4. Enclose copies of any contractor s licenses and proof of insurance. 5. Enclose a signed tester agreement. 6. 2 TO: All Businesses Employing Certified Backflow Prevention Assembly General Testers Recognized by the City of Mesa This letter is to keep testers informed on the City of Mesa requirements regarding cross-connection control and the testing of backflow prevention assemblies. The Water Quality Services Office administers Mesa s Cross-Connection Control program. This is performed in co-operation with the Building Inspections, Engineering Construction Services, Fire Prevention, and Plan Review Departments. Regulations are enforced using City of Mesa Ordinances, Arizona Department of Environmental Quality regulations, and the Uniform and International Plumbing Code. Certified Testers It is the responsibility of the certified tester to notify Water Quality Services of any changes to keep our records accurate. The gauges used for testing must be checked for accuracy on an annual basis and a copy of the calibration report sent to Water Quality Services. Keep your tester certification current by planning ahead and retesting before your certificate expires. All assemblies shall be tested in accordance with Chapter 9 of the USC Foundation for Cross-Connection Control and Hydraulic Research, Manual of Cross-Connection Control, tenth edition, October New Assembly Installations All new backflow prevention assemblies whether used for service containment or internal isolation must be tested by a certified tester before any final inspection approvals are issued by the City. Return the original test report to the Water Quality Services office, leave a copy of the report with a responsible party onsite if possible, and keep a copy for your records. Do not leave the original test report attached to the assembly. Do not submit the original test report to the City s Building Inspection, Engineering Inspection, or Plan Review departments. The test report(s) must reach the Water Quality Services office directly. Automatic Fire Sprinkler Systems In accordance with City of Mesa Code 7-2-2, Uniform Fire Code, Article 10, Section , the annual backflow prevention assembly testing shall be performed at the same time as the annual automatic fire sprinkler system inspection, by a certified backflow prevention assembly tester recognized by the City of Mesa. Individuals or businesses possessing an Arizona State Board of Registrars L-16 contractor s license shall conduct the inspection, testing, repair and maintenance of the fire sprinkler system. After completion of the inspection and testing, the L-16 contractor will forward the original backflow test report to Water Quality Services. These requirements also apply to automatic fire sprinkler systems that contain a testable single check valve per Mesa Standard Detail M31.6. The testable single check valve installation is required on all new automatic fire sprinkler systems constructed after October 30, This UL approved valve is tested for proper operation in the same manner as a Double Check Valve assembly. If you have not received training on this valve and need a copy of the test procedures, contact the Water Quality Services Office. L-16 contractors should note that any existing alarm check or swing check that becomes unserviceable is required to be replaced with the M31.6 design.

3 Pressure Vacuum Breakers After completing the standard field test on a pressure vacuum breaker, the City of Mesa requires you to perform a backpressure check on the assembly. A section is provided on the report form to record your findings. If you are unsure of the procedure to check for backpressure you may contact this office for instructions. Assembly Test Reports All assembly test report forms must be filled out completely and accurately, and be submitted by the tester to Water Quality Services within ten business days of the test date. Reports that omit test information, assembly information, water meter number, serial number, address, owner, your employer contact information header or have been prepared in a sloppy unprofessional manner will be rejected. The customer will be advised and the report(s) returned to the tester s employer. Negligence by any tester in performing their duties, continued careless inaccurate preparation and submittal of test reports, and failure to follow the instructions presented in this letter will result in the testers employer being dropped from our list. 5 CITY OF MESA BACKFLOW ASSEMBLY TESTER AGREEMENT I have received and reviewed the City of Mesa tester procedures, and as a condition of certification from the City of Mesa, agree to abide by all the procedures, policies, and directives established by the program administrator. I understand that being granted tester certification is a privilege, and that such certification may be revoked at any time, if in the opinion of the program administrator, my actions are detrimental to the successful implementation of the program. I certify that I have complied with all applicable licensing requirements established by the State of Arizona Registrar of Contractors Rules and Statues. I agree to conduct all backflow assembly testing in accordance with Chapter 9 of the USC Foundation for Cross-Connection Control and Hydraulic Research, Manual of Cross-Connection Control, tenth edition, October 2009 testing procedures and submit all reports by the date due, and no later than ten business days of any activity performed on any assembly, regardless of whether payment has been received. I will not represent myself as an agent of the City of Mesa, and will in no way represent to any customer, testing maintenance or repair requirements, which are not expressly defined by the City of Mesa.
