In Conformance? Comments / Action Hot-mix asphalt pavement - TESC - - Proof roll inspection was reported done and addressed yesterday. Prime contractor or developer should include the City inspector when notifying developer s geotechnical inspector regarding subgrade proof-roll inspections. Two lifts were poured and compacted over the parking areas only along 35th Avenue NE. Only the base of pavement along the street was poured and compacted. Exposed ground located downstream of installed silt fence in the northwest corner should be stabilized. Muddy discharge was noted in the 11/17/2015 report developer s CESCL reported no discharge during inspections dated 11/18/2015 and 11/24/2015. Inspection Diary Time 8:50 AM 1:00 PM Discussion Approximate time of arrival on site was 8:50 AM. Building construction work was ongoing. A 4-man crew from Universal Land (UL) continued working on the roadway grading over 35th Court NE, between NE 146th Street and NE 146th Place. The following construction equipment were used: Hot-mix asphalt (HMA) paving by subcontractor: Bomag asphalt paver Bomag DD-28HF big 2-drum roller 1 of 6
2 Bomag BW small 2-drum roller Deere 250 skid steer Miscellaneous grading on site, etc. by prime contractor: Deere 544J loader Deere 50D excavator There were 7± laborers tending to the HMA distribution behind the paving equipment. The following were noted prior to and during the HMA pavement: The edges of cement concrete gutter in contact with HMA were tarred prior to paving. The proposed HMA section was 5 inches deep HMA Class ½ over rock subgrade, as per Section A of the approved Field Change Order No. 1 dated March 18, The subgrade was reportedly proof-roll inspected yesterday. Prime contractor or developer should include the City inspector when notifying developer s geotechnical inspector regarding subgrade proof-roll inspections. There was 1 operator on the asphalt paver; 1 operating each roller (x2); 7± laborers, and; multiple dump trucks that hauled in the asphalt. A flagger was made available to assist traffic for trucks entering and exiting the site. The pavement within the parking areas was done in two lifts to ensure better compaction. Only one lift was done for the HMA pavement along the roadway; final lift was planned to be done at a later date. The compaction test technician arrived around 11:00 AM, conducted asphalt pavement compaction density testing using a nuclear gauge over multiple spots. The following TESC items (since last 11/18 inspection) have been updated: 1. Discharge: No discharge. Approximate amount of precipitation since last inspection: 0.55 inches* Approximate amount of precipitation in the past 24 hours: 0.23 inches* * Based on nearby rain gauge as recorded by King County. 2. Construction Entrance: Accesses to both NE 145 th street and NE 147 th Street were available. The approved construction access on NE 145 th Street was closed, but another access east of the tree protection area was open in its place. Dirt tracks were observed as minimal. Mud starting to build up on the rock pad maintenance recommended on the NE 145 th Street entrance. Rock pad was replaced on NE 147 th Street entrance. 3. Perimeter protection: OK. 4. Tree protection: OK. 5. Silt fence: OK 6. Inlet protection: OK. 7. Stabilization of exposed soils: Contractor should be aware that the areas that will not be worked on for at least 2 days will need to be covered/stabilized. 8. Dust control: Not applicable. 9. Source control: a. Stabilize the exposed roadside landscape (area between silt fence and the asphalt pavement). Only a small area on the northwest corner was stabilized with straw mulch; could need some more to the east. Inspector left at approximately 1:00 PM. 2 of 6

Pictured are the southernmost (a) and northernmost (b) limits along 35 th Avenue NE, and the easternmost limit (c) at the NE 146th Place intersection. 6 Subject: Temporary Sedimentation and Erosion Control Location: On site Comment: (1) Construction accesses to NE 145 th Street had some dirt (top) while NE 147 th Street (bottom) was minimal. Street sweeper was on site during site visit. (1) (2) Maintenance was recommended for the construction accesses. Rock pad for the NE 145 th Street access (top) still looked dirty, but the NE 147 th Street access (bottom) looked clean. Developer s field manager reported that the rock pad was replaced. (2) 6 of 6
