2 Background on CRISP

3 What is CRISP? CRISP is a non-profit health information exchange, or HIE, organization serving Maryland and the District of Columbia. Health Information Exchange allows clinical information to move electronically among disparate health information systems. The goal of HIE is to deliver the right health information to the right place at the right time providing safer, more timely, efficient, effective, equitable, patientcentered care. CRISP s Mission: To advance the health and wellness of our patients by deploying health information technology solutions adopted through cooperation and collaboration. 3

4 CRISP Services Web-Based Query Portal Patient demographics, Labs, Radiology reports, Electronic Reports (discharge summaries, history and physicals, operative notes, consults) Maryland Prescription Drug Monitoring Program Data Part of the web-based portal Encounter Notification Service Real-time notification to provider of patient hospital admission Increases coordination of care, improves follow-up care, helps reduce re-admissions Encounter Reporting Service Inter- and intra-hospital readmission reports

5 CRISP Data CRISP receives inbound data feeds from many provider organizations across the region, including all hospitals in Maryland and most in D.C. This powers CRISP services, putting clinical information in the hands of those with treatment and care coordination responsibilities. 11 Quick Overview of Maryland PDMP DISPENSERS are required by law to electronically report Rx data to the PDMP: For every Schedule II-V CDS drug dispensed Within 3 business days of dispensing Data are confidential, privileged, not subject to discovery or subpoena in civil litigation, not public records Prescribers & dispensers not required to check database; no liability/discipline solely for failing to check Program does not create new clinical practice standards Civil penalty for dispenser failure to report ($500/incident) Criminal penalty for unlawful data access/use (misdemeanor, 1 yr. and/or $10k fine)

12 PDMP Rationale for HIE Integration Start with your PDMP Goals Assist medical, pharmacy, and public health professionals in the identification and prevention of prescription drug abuse Assist law enforcement and regulatory agencies in the identification and investigation of illegal prescription drug diversion Promote a balanced use of prescription data that preserves the professional practice of healthcare providers and legitimate patient access to optimal pharmaceutical-assisted care.

13 PDMP Rationale for HIE Integration Maryland s PDMP Clinical Objectives Give healthcare providers real-time access at the point-of-care to patient CDS Rx history to: Identify aberrant drug use (i.e. doctor/pharmacy shopping ) indicating possible misuse, addiction or diversion Initiate referral to appropriate assessment, treatment & recovery services Increase confidence in prescribing/dispensing decisions

17 Master Patient Indexing The Challenge: Accurately and consistently linking identities across multiple facilities to create a single view of a patient. A near-zero tolerance of a false positive match rate with a low tolerance of a false negative match rate. Accurate cross-entity / cross-source patient identity management is a critical function for an HIE to function with the trust of its participants.

18 Lessons Learned: Patient Matching Strategize way to maximize patient matching Complexities with linking patients from different sources Review way data will display to user Required data fields Fields supplied from sources Determine goals of matching False positives vs false negatives Flexibility with thresholds

20 Lessons Learned: Working Within an HIE HIE Registration policies vs PDMP Law requirements Different user types able to access certain data sources can be complicated MD PDMP Law = only licensed delegates to access PDMP data CRISP = allows ER scribes and other administrative roles Mechanism for controlling user view of data feeds based on role Similar to how one thinks through interstate data sharing access by role Note: issue for both PDMP and HIE

21 Lessons Learned: Working Within an HIE Registration Policies Perform policy analysis between HIE and PDMP CRISP has certain policies in place around credentialing users Matched PDMP requirements, but had certain extra steps per CRISP policies Nuances to various types of users CRISP serves as Maryland and DC HIE portal

22 Lessons Learned: Working Within an HIE Capitalizing on other data sources Determine if other data feeds in HIE are available to the PDMP for trend analyses Using radiography or lab results to confirm diagnoses linked to CDS prescriptions ADT/encounter feed from hospitals to link overdose or other associated diagnoses with PDMP data Accessing registrant data files Additional demographics on users Using for our current PDMP Program Evaluation

23 Lessons Learned: Working Within an HIE Data Transfers and Storage Data Transfer Timeline Establish data transfer schedule Understand how purging or updating records will impact transfer schedule Storing data in the HIE vs pinging PDMP for data views Legal and administrative requirements? Will data persist in the HIE? Ex. Pulling PDMP data into provider notes

24 Lessons Learned: Working Within an HIE Data Translation Determine which vendor is positioned best to do the data translation Some vendors can do it more easily than others Long-term impact / flexibility to make adjustments Ensure that data is mapped and properly displayed in portal Different standards (HL7, ASAP, etc.) do not always map properly Can have vendor limitations on what data elements display discretely May have to be creative with display

25 Lessons Learned: Working Within an HIE Vendor Engagement Define all vendors that need to be engaged Learning to pull in right people at right time Have various groups interact with one another Thoroughly test integration of systems Often working with systems that are inherently different

27 Lesson Learned: Providing Quality User Experience Establish expectations for User Support Clearly define what user issues will be supported by which party PDMP Vendor HIE Vendor PDMP Program Create communication plan to inform other parties of trends in user questions or system down-time

28 Lesson: Providing Quality User Experience Evaluate capacity of HIE Contractor s IT HelpDesk Services Do they have expertise in PDMP data or can you train them? Identify idiosyncrasies in your PDMP data that must be understood by HIE Contractor Date dispensed = date filled? What types of dispensers required to report

29 Lesson Learned: Providing Quality User Experience System Capacity Complete capacity testing of the system Vendors may not be designed to handle thousands of queries per day Important to think about early, before any legislative changes (i.e. mandated registration/use) Portal speed is important to user retention

31 General Thoughts: Opportunities & Constraints Integration with EHR/Pharmacy IS Although HIE and PDMP integrated, still external to EHR/pharmacy system Streamlining user workflow is extremely beneficial Ability to leverage other HIE integration projects Possibility for HIE projects to benefit PDMP and overdose efforts As HIE expands, access to PDMP expands

32 General Thoughts: Opportunities & Constraints PDMP as law enforcement / surveillance tool MD focus for PDMP is as clinical resource Initially, perception was for LE use, clashing with general view of CRISP Marketed PDMP as clinical tool just like rest of CRISP Placing PDMP within CRISP was both blessing and curse in terms of framing PDMP

33 General Thoughts: Opportunities & Constraints Unsolicited Reporting With current structure of HIE-PDMP, cannot do electronic automatic notifications Ex. Would like to be able to mimic Event Notification System (ENS) alerts Limited by statute/regulation language, so in hindsight would have argued for law that allowed this activity TAC review of all data reports Many other states may be able to implement
