1 Meaningful Use Quick Reference When information is recorded correctly in the structured fields within EMR: Meaningful Use requirements such as safety cross checks are automatically accomplished by the system Established reports can quickly show progress toward and achievement of the Meaningful Use benchmarks. Validation required for incentive payments is accessible Clinic reports show the progress toward achieving the 13 measures that require clinic action to qualify for MU. 3 Core Measure # 1 Use CPOE to enter medication order (Rx) On the CPOE A&P-CCC form On the any tab where an assessment is being entered Use the New Meds button within the Assessment to issue a new prescription. (Medications Prescribed using the New Medications form accessed from any point in the visit also qualify for 2012) Return to beginning of Document

5 1 Or 2 Or 3 Core Measure # 3 Maintain up-to-date problem list (or No Know Problems recorded) 1 of 3 choices for recording (see next page for maintaining) 1. On the Problems-CCC form Use the checkboxes to enter known problem(s) 2. On the New Problem form (available from many forms/buttons) Use the Custom or Reference List to record the coded known problem(s) 3. On the Update Problems form Click the checkbox to indicate NKPROB to attest that you have verified that the patient has no know problems. (Also see next page)

6 Core Measure # 3 Maintain up-to-date problem list (see previous page for 3 choices for recording problems) On the Prob-Meds-Allergies-CCC form Use the Verified Problems button to attest that problems have been added/removed/verified and the problem list reflects current problems. 8 Core Measure #4 Generate Permissible Rx electronically. On the Prescriptions (Refill) form or On the New Medication form Set the Prescribing Method to Electronic Every time a permissible Rx is generated. Controlled substances (Class 2 & 3 drugs) can not be electronically transmitted. NOTE: To be able to E-Rx providers must complete training for E-Rx and be set up. Once training and set up are completed, regular use is required or account will be deactivated by vendor. Return to beginning of Document

10 2 1 Core Measure # 5 Maintain an active medication list (or No Know Meds recorded) On the New Medication form 1 of 2 choices 1. Use the Custom or Reference List to record or prescribe the coded known medication(s) 2. Click the checkbox to indicate NKMED to attest that you have verified that the patient has no know medications. Return to beginning of Document

12 Click the Allergies Verified button to attest that you have reviewed and verified the patient s recorded allergy list. See next page for detail (Also see next page) Core Measure # 6 Allergies recorded as structured data (or No Know Drug Allergy recorded) On the Prob-Meds-Allergies-CCC form On the Prob/Meds/All/Dir tab 1 of 2 things: 1. Enter known allergies using the Edit Allergies button or 2. Click the NKDA button to attest that you have verified that the patient has no know drug allergies

13 Notes about the Update Allergies or Adverse Reactions form 1. When an allergy is entered or amended the Allergies Verified observation term is automatically entered. (It can be cleared using the Clear button on the PMA form.) 2. When allergies are entered, the NKA option is unavailable. 3. NKA and allergy review can be set from the Update Allergies or Adverse Reactions form using the check boxes. a. Using the allergies reviewed check box also enters the Allergies Verified observation term for MU. b. Using the NKA check box also enters the NKDA observation term. Return to beginning of Document

15 Core Measure #7 Demographics (3 fields) Core Measure #9 Smoking On the Vitals Signs -4-CCC form On the VS-1 Additional Information tab Choose a value FROM THE DROPDOWN MENU for ALL 7 of the MU Demographic fields. at least one time during the reporting period. 21 Core Measure # 12 Electronic Access Start a stand alone update using the Encounter Type: Release of Information Electronic On the Patient Authorization form 7 REQUIRED FIELDS TO MEET MU Notes: Requests for copies of information for referrals, insurance, legal and other types of requests do not apply for MU. In order to meet the MU measure, the sent date must be within 3 business days of the date of the request Information to be Disclosed check all that apply 2. Purpose of Disclosure copy for patient must be selected 3. Date Request Received enter date request was received in the clinic 4. Authorized by name the responsible provider (An EP must be listed. ) 5. Sent Via electronic must be selected 6. From-To-Sent by specific dates of the requested information along with the name of the individual sending the information. 7. 23 Core Measure #13 Provide clinical summaries for patients for each office visit. On the Patient Instructions-CCC form Click the Print Pt. Instructions button to print any entered instructions and the attached Clinical Summary document. Provide document to patient at every visit. Return to beginning of Document

25 Menu Measure # 5 Provide patients with electronic access In a stand alone update, use the PIN Generator form, 3 part process: 1. Click the button that indicates the appropriate access. 2. Click the Auto Generate button 3. End update and SIGN the document. Provide document to patient. If patient declines PIN: 1.
