2 Objectives Identify potential causes of the A1C goal not being achieved Define diabetes self management training (DSME) List the benefits of DSME Discuss the EBP literature that supports self care education in diabetes management

3 Why is the A1C not at goal Are medication and doses adequate? Is patient taking medications correctly? Has the patient participated in diabetes self management education? Has the patient recently seen a dietitian? Is the patient depressed? Low health literacy?

4 Depression and Diabetes Higher incidence of depression in persons with diabetes What came first depression or diabetes? Often overlooked as a cause of poorly controlled diabetes Dealing with depression and emotions are a part of the diabetes education program

5 Diabetes Self Management Education/ Training DSME Diabetes is a chronic disease that does not fit into the acute care medical model 90 to 95 % of diabetes management is self care A knowledge deficit may be the cause of poorly controlled diabetes Many patients have not participated in a diabetes education program as a result of lack of a referral ( BMC Family Practice, 2011)

6 DSME National Standards for Diabetes Self Management 10 standards have been establish by a task force Task force members are representatives of the: AADE- American Association of Diabetes Educators ADA- American Diabetes Association ADA- American Dietetic Association Veteran s Health Administration CDC, IHS

7 DSME Defined by the task force as the ongoing process of facilitating the knowledge, skill and ability necessary for diabetes self care.

8 AADE Seven Self-Care Behaviors Healthy Eating Being Active Monitoring Taking Medication Problem Solving Reducing Risk Healthy Coping

9 Diabetes Self Management Education (DSME) ADA and AADE are 2 agencies the certify or recognize education programs Recognition is necessary for insurance reimbursement Certification is for a period of 4 years A requirement by these agencies is the education is conducted by a multi discipline group of educators at least one a CDE

10 Reimbursement for Education Medicare (CMS) will cover the cost of DSME 10 hours comprehensive education once in a lifetime 2 hours each year for Medical Nutrition Therapy (MNT) Most insurance companies will cover DSME

11 Limited number of Educators Certified Diabetes Educator NCBDE National Certification Board for Diabetes Educators As of 1/26/2011 there are 17,073 healthcare professionals hold the CDE credential 54% registered nurses (this includes APNs) 39% registered dietitians 4.7% registered pharmacists 2.3% others (PAs, MDs etc)

12 Group vs. Individual Education Individual One on one allows for individualized care Need to know information only Comfort zone not being in a group setting Appropriate for Type 1 pediatric patients and gestational diabetes Group education Peer support Least costly Increased number of individuals an educator can meet with Group dynamics have the potential to increase participation

13 strategies in people with type 2 diabetes mellitus (review) The Cochrane Collaboration 2009 A systematic review by Deakin, Mc Shane, Cade and Williams 11 studies of a total of 1532 participants Meta-analysis in favor of group DSME improved A1C Other positive findings were improved fasting glucose, diabetes knowledge, reduced systolic bp, body weight and medication requirements

14 Short comings of DSME Lack of sustainability with current education methods Not all participants improve self care after attending DSME Patients and providers do not appreciate the value

15 Call for additional research for EBP Other venues to deliver diabetes education-shared medical appointments Tele educations Health Coaching Individual vs. group education Utilization of lay or community educators vs. professional educators Cost effectiveness of diabetes education
