1 Delaware LLC Original Sheet 1 A. MULTI-LINE PACKAGE BONUS 1. Description The Multi-Line Package Bonus (MLPB) is an optional Residence enrollment billing service offering which provides multi-line Residence Dial Tone Line customers with a monthly discount if they subscribe to one of the following eligible services for one or more of their Verizon Delaware LLC Dial Tone Lines. MLPB Eligible Services: a. The Big Deal b. Verizon Local Package Extra SM or Verizon Local Package SM 2. Regulations a. The MLPB will apply as long as the enrolled customer subscribes to the eligible optional service(s). b. The eligible MLPB services are provided subject to their individual service regulations, rates and charges as specified in this or other applicable Telephone Company tariffs. c. The MLPB discount will apply to the applicable monthly rate for each eligible service, per line equipped. d. To be eligible for the MLPB discount, the enrolled Residence customer must have all the multi-line Dial Tone Lines billed to the same residence customer, at the same residence address and under the same residential main billing number. e. There is no charge for eligible Residence customers to enroll in this offering. f. Enrolled MLPB customers must retain Verizon Delaware LLC as their local service provider for Dial Tone Line and all MLPB eligible services. 3. MLPB Monthly Discount Rate a. MLPB Discount per eligible service, per line 5% NOTE: 1 Availability of Multi-Line Package Bonus for Verizon Local Package is limited to existing subscribed customers of record, at their current location only, as of October 19, February 1, 2011

Local and Toll Package is an optional residential service package that provides residential customers with a combination of offerings for one flat monthly rate. a. The following services are included with Local and Toll Package: 1. Individual Residence Dial Tone Line 2. Touch-Tone service 3. Unlimited Local Usage Package 4. Up to 300 minutes of completed customer-dialed intrastate intralata toll calls in lieu of the regular residence Two-Point Service rate scheduled specified in this Guide. Local and Toll Package also includes customer-dialed interstate intralata calls as specified in the Verizon Telephone Companies Tariff F.C.C. No. 5. b. Customers subscribing to Local and Toll Package cannot have Optional Services Package Plan 1 (marketed as Call Manager), Big Deal, V Local Package or Verizon Local Package Extra, Residence Service Variety Package (RSVP), Maximum Value Plan, Sensible Minute, Choice, Consumer Opportunity Savings Plan, Two-County Calling, Verizon Regional Package or Verizon Regional Package Extra on line as Local and Toll Package. b. Local and Toll Package is not available with Lifeline service or IntelliLinQ BRI service lines. c. For purposes of calculating Volume Discount Plan discounts on Optional Central Office services features not included in the Local and To Package but purchased in conjunction with the Local and Toll Package on the same line, Local and Toll Package will count as four (4) eli features. d. Concession service is not applicable to Local and Toll Package. e. Local and Toll Package is included with the Multiline Package Discount Plan. f. Eligible intralata toll calls are those calls which are customer-dialed and completed without the assistance of a Verizon operator. Calls 900/976/700/555 numbers are not eligible toll calls under the Local and Toll Package Plan. g. Available to individual line residence customers who enroll in Local and Toll Package on an eligible Dial Tone Line(s) basis. Customers affirmatively enrolled in the intrastate intralata Local and Toll Package Plan will automatically be enrolled in the interstate intralata pla 3. Verizon Regional Package offers customers a choice of compatible Custom Calling Service and one (1) voice mail feature. D/B/A Verizon Long Distance Posted Rates, Terms and Conditions. Discontinuance of any one of the required products will result in immediate termination of the discount. The Voice Discount Plan provides a discount for customers who are purchasing an additional line 1, disconnecting service from Verizon or returning to Verizon local service for competitive reasons. Discounts of $20.05(I) and $18.00(I) per month will apply for customers that retain or agree to subscribe to Regional Value or Regional Value along with an unlimited long distance plan for a 12-month term, respectively. A monthly discount of $20.05(I) for twelve (12) months will apply for qualifying customers that retain or agree to subscribe to Regional Value without an unlimited long distance plan. A monthly discount of $18.00(I) for twelve (12) months will apply for qualifying customers that agree to subscribe to Regional Value along with an unlimited long distance plan 3. (Cont d) Customers who (i) call Verizon Delaware LLC to disconnect their Verizon dial tone service and agree to re-establish such service or (ii) call to establish dial tone service with Verizon Delaware LLC after disconnecting their dial tone service from another provider or (iii) or agree to add an additional line(s)1, or (iv) who have certain Verizon non-regulated services as determined by the Company but no local service from Verizon Delaware LLC, and accept this offer in response to a direct mailing, advertising, or other Verizon marketing activity, or (v) who respond to marketing activity or have been offered VDP by a Verizon Representative to negotiate service will be eligible for the Voice Discount. After the initial twelve-month period has expired 1, customers who continue to subscribe to Regional Value or Regional Value along with an unlimited long distance plan will be eligible to receive a credit of $10.05(I) and $15.00(I) per month for a twelvemonth extension period, respectively. Customers who subscribed to Regional Value or Regional Value along with an unlimited long distance plan under an alternative rate treatment will also be eligible for the credit of $10.05(I) and $15.00(I) per month for an additional twelve-month period, respectively. Qualifying unlimited long distance plans must be consistent with the Plan O Service Unlimited plan in the Bell Atlantic Communications, Inc. D/B/A Verizon Long Distance Posted Rates, Terms and Conditions. To receive the additional discount each product must be billed by or on behalf of Discontinuance of any one of the required products will result in termination of the discount. 8. The applicable discount will expire twelve (12) months from the date it is implemented on a customer s account unless extended by Verizon. 9. Price Guarantee 4 Price Guarantee Regional Value is an offering for residential customers in a location that is not eligible for Verizon Online Broadband or Verizon FiOS products who establish new dial tone service or who call to disconnect their local service and agree to subscribe to Regional Value with a qualifying unlimited long distance calling plan. Customers will be eligible to receive Regional Value service for the discounted rate of $17.04 per month for as long as the customer remains at the same service address Qualifying unlimited domestic long distance calling plans must be consistent with the Plan O Service Unlimited as found in the Bell Atlantic Communications Inc. D/B/A Verizon Long Distance Posted Rates, Terms and Conditions. Each product must be purchased through or billed by Discontinuance of any one of the qualifying services will result in termination of the discount. An additional discount ranging from $2.99 to $25.04(I) per month will apply when Regional Essentials is bundled with or without an unlimited long distance plan and certain products offered through Qualifying unlimited long distance plans must be consistent with the Plan O Service - Unlimited plan in the Bell Atlantic Communications, Inc. D/B/A Verizon Long Distance Posted Rates, Terms and Conditions. Discontinuance of any one of the required products will result in immediate termination of the bundle discount. To receive this additional discount, each product must be billed by For customers who have or agree to purchase the products for a 24 month commitment, the additional monthly discount will expire twentyfour (24) months from the date it is implemented on the customer's account unless extended by Verizon. The Voice Discount Plan provides a discount for customers who are purchasing an additional line 1, disconnecting service from Verizon, or returning to Verizon local service for competitive reasons. A monthly discount of $17.05(I) for twelve (12) months will apply for customers that agree to subscribe to Regional Essentials as an additional line 1. A monthly discount of $15.00(I) for twelve (12) months will apply for qualifying customers that agree to subscribe to Regional Essentials along with an unlimited long distance plan 3. A monthly discount of $10.00(I) for twelve (12) months will apply for qualifying customers that agree to subscribe to Regional Essentials along with an unlimited long distance plan 4. Customers who (i) call Verizon Delaware LLC to disconnect their Verizon dial tone service and agree to re-establish such service or (ii) to establish dial tone service with Verizon Delaware LLC after disconnecting their dial tone service from another provider or (iii) agree to add an additional line(s) 1, or (iv) who respond to marketing activity or have been offered VDP by a Verizon Representative to negotiate service will be eligible for the Voice Discount. After the initial twelve-month period has expired 2, customers who continue to subscribe to Regional Essentials or Regional Essentials along with an unlimited long distance plan will be eligible to receive a credit of $7.05 (I) and $17.00(I) per month for a twelve-month extension period, respectively. The Regional Essentials Feature Package provides the following features at no additional charge when Regional Essentials is bundled with an unlimited long distance calling plan: Call Forwarding, Three-Way Calling and Verizon Call Assistant 1, 2 (C) Qualifying unlimited long distance plans must be consistent with the Plan O Service - Unlimited plan in the Bell Atlantic Communications Inc. D/B/A Verizon Long Distance Posted Rates, Terms and Conditions. Each product must be billed by Discontinuance of any one of the services listed above will result in immediate termination of this discount. This offer may not be combined with other offers except as authorized by Verizon Delaware LLC. 11.
