3 For further information, please contact CREEK CLUB AGES 14 TO 17 YEARS YOUTH CLUB Creek Club is an after school recreational program for middle school students (boys and girls in grades 6th, 7th, and 8th) Program participants receive structured activities, quiet time, and a light snack and drink Monday through Friday (school days only) from 4:00 PM to 6:00 PM (hours will be adjusted for early release days). 6 ONGOING ADULTS 18+ PROGRAMS SPECIAL NEEDS PROGRAMMING Programs led by a Certified Therapeutic Recreation Specialist For ages 3 to Senior Citizens For more information, call Complete our Special Needs Survey to have your needs heard Find the survey online at Eligibility: Each special needs individual must meet the following criteria in order to take part in the activities. If an individual is unable to do so, they are still welcome to attend each activity but must provide their own assistant for supervision (such as a family member, companion, etc.). Criteria: Independent bathroom skills, ability to follow basic instructions, ability to participate in a group. FITNESS TRAINING & INSURANCE FUNDED MEMBERSHIPS HEALTHWAYS/SILVER SNEAKERS PROGRAM If you are a Medicare eligible member of a health plan, you may be able to receive a funded basic annual fitness membership through Healthways and the Silver Sneakers.
