6 Laundry Services for The Rohr Home Bid SPECIAL CONDITIONS 1. Award will be made based on overall low bid for items 1 through 12 combined. All bid items that are part of the basis of award should be bid at a fair and reasonable price; failure to do so may cause the bid to be non-responsive. The Procurement Director shall be the sole judge of what is fair and reasonable. Bidders must bid on all 12 items for the bid to be considered responsive. The Procurement Director reserves the right to reject any or all bids and/or waive any minor irregularities in the bids received, whichever would be in the best interest of the County. 2. PERFORMANCE OF WORK: The work required under this bid shall be performed by the entity submitting the bid. 3. The period of performance for this bid is from date of award through January 30, The bid will automatically renew for two (2) one (1) year periods, unless otherwise terminated in accordance with General Information Items #12 and # All prices bid shall remain unchanged during the period of performance, as specified herein, and as may be adjusted in accordance with General Information Item # If it becomes necessary to revise or amend any part of this bid, an addendum will be issued and will be posted on the County s website at It is the sole responsibility of the bidders to check the website to ensure that all available information has been received prior to submitting a bid. 6. Vendors must possess a Polk County Local Business Tax Receipt (f/k/a Business License) in order to do business with the County. A copy of such license must be provided to the Procurement Division before award is made to the successful vendor. 7. Prices must include the cost of picking up soiled laundry from The Rohr Home and returning clean laundry to The Rohr Home. 16

7 Laundry Services for The Rohr Home Bid SPECIFICATIONS 1. Provide pickup service Monday, Wednesday and Friday (other days, if necessary) of all soiled laundry (linen) consisting of, but not limited to, the following items: flat and fitted sheets; pillow cases; blankets; spreads; towels; wash cloths; gowns; tablecloths; and cubicle curtains. 2. Provide delivery no later than 3:00 p.m., Monday, Wednesday and Friday (other days, if necessary) of clean laundry consisting of, but not limited to, the following items: flat and fitted sheets; pillow cases; blankets; spreads; towels; wash cloths; gowns; tablecloths; and cubicle curtains. 3. Vendor will furnish all flat and fitted sheets, pillowcases, towels, washcloths and gowns in sufficient quantity, as determined by The Rohr Home, to cover regular usage and emergency usage. Minimum thread count per square inch for sheets and pillowcases will be one hundred thirty (130), commonly known as T-130. Gowns will be 50% cotton and 50% polyester, with thread count at four and one-half (4.5) ounces per square yard. Washcloths will be one hundred percent (100%) cotton, with the minimum specifications as one-half (1/2) pound per dozen. Bath towels will be one hundred percent (100%) cotton, with the minimum specifications as five (5) pounds per dozen. 4. Vendor will furnish all laundry carts to be used for pickup and delivery; laundry carts must be left until Rohr Home personnel have emptied all carts. 5. Vendor must meet all requirements of STATE, CDC, OSHA and JCAHO Standards, which state that all linen will be washed at one hundred seventy (170) degrees Fahrenheit and dried at one hundred eighty (180) degrees Fahrenheit for a minimum of twenty-five (25) minutes, to eliminate all air and blood borne pathogens. The water will be treated with chemicals to control the ph balance. 6. All MSDS sheets shall be supplied to the Rohr Home Administrator, updating as needed. 7. Vendor will deliver adequate linen and pick up soiled linen in accordance with schedule above to and from an alternate site in the event of The Rohr Home s residents' evacuation or relocation. 8. Vendor will charge The Rohr Home on a price per item basis for soiled linen, not for inventory. 9. In the event soiled items not supplied by the vendor are picked up, such as blankets or personal clothing of the residents, the vendor shall ensure that they are cleaned, based on a price per item basis, and returned to The Rohr Home. 17

Is it only for the 60 residents or for you to have extra on hand for emergencies and other reasons? Answer: Specifications, Item 3 states, Vendor will furnish all flat and fitted sheets, pillowcases, towels, washcloths and gowns in sufficient quantity, as determined by The Rohr Home, to cover regular usage and emergency usage. 11 BID # LAUNDRY SERVICES FOR THE ROHR HOME ADDENDUM #2 CLARIFICATIONS Specifications, Item 3. Revise the following: Washcloths will be one hundred percent (100%) cotton, with the minimum specifications as onehalf (1/2) pound per dozen, or eighty-six percent (86%) cotton and fourteen percent (14%) polyester, if necessary for best quality. Add the following: Successful vendor will be required to provide a sample of linens prior to award for approval by the Rohr Home staff for comfort and durability. Specifications, Item 5. Add the following: Linens must meet or exceed 59A-4.122(2), FAC (Florida Administrative Code) regulations for maintaining a sanitary, orderly, and comfortable interior, including linens in good condition. Based on the State survey it means no unraveling, holes, noticeable stains or discoloration. Question 1: Would you please let me know the size of the items listed on the Bid Sheet, such as the bed linens (i.e. are they for single beds, double, or a percentage of various kinds, etc.)?
