Many traditional and nontraditional relationships are being built around the country, and investments are being made to address the social determinants of health through a variety of unexpected sources. How can your agency get involved? How do we find funding? How do we capitalize on a rapidly-changing healthcare environment? By attending this session participants will: Learn how to best secure resources for improving population health, including chronic disease prevention Learn about the latest funding opportunities and non-traditional public health partners Gain a better understanding about opportunities for promoting population health in a reforming health care system Hear stories highlighting the work hospitals around the U.S. are doing to promote population health PLEASE NOTE: REGISTRATION FOR THE PRE-CONFERENCE WORKSHOP IS A SEPARATE FEE.
