1 ELECTION CANDIDATES PRESIDENT: Patty Horvath, C.P.M. Patty Horvath has her C.P.M. and has been a member since 1997 and currently is a board member at ISM-Chicago. Patty has worked in the printing, pharmaceutical and most recently the consumer driven, house-ware s industry. She is trained in process reengineering and change management. This background enables Patty to create best practice environments in purchasing. Just recently Patty s team transitioned the existing purchasing organization from an individual plant purchasing organization to a Category Management/Strategic Sourcing and Procure 2 Pay Transaction Center. Although this new team is still transitioning, it promises to be more efficient and effective in all SGA business processes. Currently Patty works at World Kitchen, LLC where she is the director of North American Sourcing. Patty has an undergraduate degree in business administration from California Coast University and is working on a masters in organization development. EXECUTIVE VICE-PRESIDENT: Daniel B. Martin, CPSM, C.P.M., A.P.P. Daniel B. Martin, NuFarm Americas, Inc., currently serves as Vice President and Board Member. Dan has more than 20 years of Supply Chain and Operations Management experience. "We are in the midst of a changing environment. The professional of tomorrow will need a tool kit of experience, knowledge and leadership. My goal is to continue building onto the strategic direction of supply." Mr. Martin has been active with APICS and ISM for over 20 years. Dan has an MBA and MPM from Keller Graduate School, and a BS in Economics from Northern Illinois University.

2 ISM-CHICAGO ELECTION CANDIDATES 2 SENIOR VICE-PRESIDENT: David Roy David has a finance degree from the University of Central Arkansas and an MBA from North Park University, Chicago, IL. He completed the Managing Effective Supply Chains Program from Penn State University and is currently pursuing CPSM certification. David has 10 years with BP, Naperville IL - positions ranging from credit and risk analysis, vendor inventory management, commercial services (freight payment, detention billing, process improvements), logistics analyst, rail commodity manager. He has experience in relationship management, negotiations, project management, strategy development, sourcing, and supply market analysis. David held previous positions with United States Gypsum, Chicago IL and Volkswagen Credit, Inc Bannockburn, IL. David's special assignments/committees include: ISM Logistics and Supply Chain Committee Chair, United Way Ambassador, Coordinator for American Cancer Society Walk n Roll Chicago, and Member of BP Naperville's Diversity and Inclusion Network. I believe I have a great deal to offer ISM Chicago as a member of its board. Having been the committee Chair the last two years for Logistics and Supply Chain, I have a good understanding of what is needed to continue to make ISM Chicago thrive. Also, by attending one of the board's recent meetings, I definitely feel I have many ideas that could benefit ISM Chicago. Enthusiasm, energy, and dedication are three key areas I would bring to the board and ISM Chicago. I want members to take an active role and participate, I want to see attendance rise at our monthly meetings, I want more feedback from members on what they want from ISM and I want this network to be beneficial both professionally and personally. ISM Chicago has made some great strides over the two years I have been a member, but there is still a lot that can be accomplished. I would like to be an integral part in helping ISM Chicago hit that next level.

3 ISM-CHICAGO ELECTION CANDIDATES 3 BOARD CANDIDATES: Robert J. Ballwanz Robert has over 25 years of broad based Operations and Supply Chain Management experience. He is currently engaged as a Category Manager with World Kitchen a $600 million+ privately held company engaged in the manufacture and distribution of kitchen and household tools. Before joining World Kitchen, Bob was Sr. Director of New Business Integration for PSA, a Print Services Provider, where he was responsible for integrating newly acquired businesses and providing council on continuous improvement projects. Prior experience includes serving as the Operations Manager for Inktel Direct, a Direct Marketing, Fulfillment and e-commerce business; Production Manager for Sourcelink, an Integrated Direct Mail and Marketing firm and Moore/Wallace where he held a number of engineering, quality and supply chain management roles. Bob Holds a BS in Industrial Technology and Engineering from Northern Illinois University, and an MBA from the Stuart School of Business, Illinois Institute of Technology. He also completed the Executive Management Curriculum at Northwestern University's Kellogg Graduate School of Management. He is a Six Sigma Black Belt, and CPIM. Reva Bhatia Reva Bhatia is a graduate from Arizona State University with a degree in Supply Chain Management. In her time at ASU, she was actively involved with her local chapter for SCMA (Supply Chain Management Association) as a Social Chair. Through her time at ASU, she gained internship experience in Warehousing (Target) and Transportation (Knight Transportation). She then moved on to a role in Minneapolis at General Mills, where she worked in Procurement. She was involved with her local chapter of MnCAN (MN Corporate Asian Network) where she worked to recruit and retain members. She moved to the Chicago area 8 months ago for a Procurement role at BP. Most recently, she has taken on the role of managing Public Relations as a committee member for ISM Chicago. Reva hopes to help ISM by focusing organizational efforts on: Recruiting and retention of new members, with a focus on young corporate professionals through: * Offering mentorship opportunities to leverage experience prevalent in ISM already * Building community of participants at the educational levels * Focus on involvement in academic community and spreading word on what "Supply Chain Management" means Focus on developing events with a more robust/ well-rounded appeal to various age groups, helping to prep the newer general of ISM volunteers

Daniels Bio: Scott Daniels is Managing Director of Windan Consulting, focused on providing supply chain management improvement and business development services. Previously, Scott was employed with Vestas Nacelles America as Director, System Leader North America, Power Generation. Prior to his position at Vestas, he held supply chain executive positions within the transportation, aerospace, and other industries. Scott has an MBA from the University of Iowa and a BS Industrial Engineering from Iowa State University. Scott is APICS and Six Sigma Certified, and is Chairman of the ISM-Chicago Alternate Energy Committee and leader of the Wind Energy Supply Chain and Operations Group. Statement: Manufacturing is re-emerging in the United States. After years of competitive efforts to take out cost through implementation of lean and other methods, many American manufacturers we are finding themselves in a position to compete on a global scale. At ISM, our mission is to improve our ability to compete in the global business arena by preparing and educating our current and future members on main stream and cutting edge supply chain processes. As a leader with an extensive supply chain background and experience within a cutting edge industry, should I be elected to the board, my focus will be to encourage programs to drive the supply chain profession as a career path of excitement and growth for our members. Ollerer, CPIM ISM Vision: I would like to serve on the ISM board to advance the integration of Lean and Six Sigma concepts into the supply chain body of knowledge, to promote Supply Chain Management as an attractive profession for college graduates looking to begin their careers, and to promote overall professionalism and integrity in the Supply Chain Management field. Professional Experience: I have been professionally employed in the Operation and Supply Chain Management field for thirty years serving in a variety of management and executive positions in large-scale public corporations, mid-sized privately held organizations, and a start-up company. In all of these organizations I have had responsibility for manufacturing operations, materials management, and purchasing. I am currently employed as Operations Manager at Trelleborg Corporation in Streamwood, IL, and have been previously employed at Peerless Industries, DuKane Corporation, Communication Systems Engineering, and Reuters Corporation. My experience also comes from a variety of industries including electronics, system integration, information systems, precision CNC machining, and plastic injection molding. Sellers, MBA, MPP, CPSD Suzanne Sellers is a uniquely skilled and versatile supply management professional offering over 10 years of procurement, consulting, and finance experience. Ms. Sellers currently consults in business process improvement and professional development training. In supply management, she has consulted for major corporations and also has public sector experience, having managed a number of different categories and provided solutions to complex issues. Ms. Sellers holds a bachelor's degree in Finance from DePaul University, an MBA in Operations Management and Entrepreneurship also from DePaul, and a Master of Public Policy (MPP) from the University of Chicago. In addition, she holds the Certified Professional in Supplier Diversity designation (CPSD) from Institute for Supply Management. A member of ISM-Chicago since 2000, Ms. Sellers has participated on ISM teams at both the local and national levels. She has worked with ISM's national Service Sector Forum, has been a member ISM-Chicago's Service Sector committee since 2008, and she is currently the Chairperson of ISM-Chicago's newly-formed Diversity and Social Responsibility Committee. As Committee Chair, she is responsible for leading the team in developing and implementing various educational programming that will create awareness and strengthen the supply management profession in the areas of Diversity and Social Responsibility. As one of ISM-Chicago's Directors, Ms. Sellers' vision is to increase the standing of ISM among member corporations by promoting membership benefits and by creating partnerships and alliances with other professional associations in various industries. David Sieben CPSM, SCOR-P Mr. Sieben is President of SourceAlign, Inc. and focuses on helping companies implement leading edge SCM practices and providing CPSM certification training. He has supply chain management experience within Fortune 100 companies and has led numerous teams in successfully transitioning to the use of best practice strategic sourcing processes including implementation and execution of strategic supply strategies, supplier development / relationship management as well as early supplier involvement in product design and cost reduction programs. As a member of the AT Kearney / EDS team tasked to transform DaimlerChrysler's indirect spend, Mr. Sieben managed supply strategies, cost reductions, and supplier rationalizations / relationships for 17 spend categories across 37 facilities. Prior to founding SourceAlign Inc., Mr. Sieben was a supply chain manager at Motorola within the fast paced, high volume Cellular phone business where he formed and led global cross functional sourcing teams tasked with executive level supplier relationship management, total cost reduction, on time product launch and six sigma quality. He also held management positions within development engineering, quality and account management. He has led numerous successful product launches also organizational change, process improvements, and developing practices.
