7 PROCLAMATION WHEREAS, Title VIII of the Civil Rights Act of 1968, known as the Fair Housing Act, prohibits discrimination in the sale, rental and financing of dwellings based on race, color, religion, sex or national origin; and WHEREAS, the principal of fair housing is not only a national policy, but a fundamental human concept and right for all Americans; and WHEREAS, the forty-eighth anniversary of the Fair Housing Act is an occasion for all Americans to become aware of and support the principle of freedom from housing discrimination. This law guarantees for each citizen, the freedom of choice in the selection of a home. NOW, THEREFORE, We, The Terrebonne Parish Council, along with the Terrebonne Parish President do hereby proclaim the month of April 2016, as: FAIR HOUSING MONTH and do hereby encourage all citizens of Terrebonne Parish to become aware of the Fair Housing Act and commit to fair housing opportunities for all. PROJECT PURPOSE & BENEFITS (150 WORDS OR LESS) The Terrebonne Parish Consolidated Government was awarded application for $7,373,435 of federal assistance under the Hurricane Gustav Hazard Mitigation Grant Program (HMGP # FEMA-1786-DR-LA) for the elevation of (54) flood damaged structures in the Parish. The original application and original Task Order with Solutient, selected after a procurement process, was for 53 units. This resolution will authorize the Parish President to sign a task order increasing the not to exceed contract amount $12,500 to $675,000 not including applications. A RESOLUTION AUTHORIZING THE PARISH PRESIDENT TO EXECUTE THE APPROPRIATE TASK ORDER FOR PROJECT MANAGEMENT SERVICES BETWEEN TERREBONNE PARISH CONSOLIDATED GOVERNMENT AND SOLUTIENT CORPORATION FOR PROJECT MANAGEMENT SERVICES FOR FEMA HAZARD MITIGATION GRANT PROGRAM AND PROGRAM APPLICATIONS DEVELOPMENT AS NEEDED. WHEREAS, the Terrebonne Parish Consolidated Government was awarded funding through the FEMA Hazard Mitigation Grant Program under presidential disasters Gustav and Isaac; and WHEREAS, the Terrebonne Parish Council Authorized the President to enter into an agreement with Solutient Corporation; and WHEREAS, the original Task Order recognized 53 units rather than the awarded 54 units negotiated; and WHEREAS, the compensation in the Task Order is based in part on the number of units; and WHEREAS, the compensation appropriate is $12,500 greater, not to exceed $675,000 as shown in Attachment A; NOW, THEREFORE, BE IT RESOLVED by the Terrebonne Parish Council, (Community Development and Planning Committee) on behalf of the Terrebonne Parish Consolidated Government, that the Parish President is hereby authorized to issue a revised Task Order with Solutient Corporation to provide the needed application development, project management and related services for the FEMA Hazard Mitigation Grant Program, not to exceed $675,000 but for applications.

Dove Parish President FROM: Chris Pulaski, Director Planning and Zoning Department SUBJECT: Request for Agenda Item March 21 st and 23 rd, 2016 Please find the resolution to authorize the President to sign a revised Task Order with Solutient Corporation. The firm will provide project management services for the FEMA Hazard Mitigation Grant Program under this revised task order. The previous application and Task Order included 53 units. Through revisions and negotiations, the Parish has received funding for an additional property. As such, the Task Order will increase the remuneration by $12,500 to include the additional unit. The final NTE contract will then be $675,000 not including applications that may arise from time to time. If everything meets with your approval, it is respectfully requested that you place the resolution on the Community Development & Planning Committee agenda for consideration. 27 Attachment A SUPPLEMENTAL TASK ORDER #2 OF THE PROFESSIONAL SERVICE AGREEMENT BETWEEN TERREBONNE PARISH CONSOLIDATED GOVERNMENT AND THE SOLUTIENT CORPORATION DATED March, 2016 GUSTAV PROGRAM MANAGEMENT SERVICES Solutient Corporation shall provide program management services for the implementation of grant # FIRM shall meet the scope of services for elevation program management as defined in Exhibit "A" and shall be paid no more than Six Hundred Seventy- Five Thousand ($675,000.00) Dollars in accordance with the Agreement. PARISH shall also pay FIRM a fee of no more than $1, per property for each replacement structure added to the grant application amendments as provided in the Agreement. FIRM shall charge these fees to the grant as Pre-Award/Application Development. The Performance Period for services for the implementation of grant # is October 28, 2015 through October 28, 2018 which will be extended without additional compensation to accommodate extensions and closeout activities as necessary to expend the funding. WHEREAS, the Terrebonne Parish Consolidated Government has been formally notified by Kala s Uber Service that a Certificate of Public Convenience and Necessity is desired: NOW, THEREFORE, BE IT RESOLVED, by the Terrebonne Parish Council (Community Development and Planning Committee), on behalf of the Terrebonne Parish Consolidated Government, that Notice of Intent is given for adopting an Ordinance to approve the issuance of a Certificate of Public Convenience and Necessity to the service of Kala s Uber Service. BE IT FURTHER RESOLVED that a public hearing on said Ordinance be called for Wednesday, April 13, 2016 at 5:30 pm.

SECTION I BE IT ORDAINED, that the Terrebonne Parish Council, on behalf of the Terrebonne Parish Consolidated Government, approves the issuance of a Certificate of Public Convenience and Necessity to Kala s Uber Service. SECTION II If any word, clause, phrase, section or other portion of this ordinance shall be declared null, void, invalid, illegal, or unconstitutional, the remaining words, clauses, phrases, sections and other portions of this ordinance shall remain in full force and effect, the provisions of this ordinance hereby being declared to be severable. SECTION III This ordinance shall become effective upon approval by the Parish President or as otherwise provided in Section 2-13(b) of the Home Rule Charter for a Consolidated Government for Terrebonne Parish, whichever occurs sooner.
