Doors are tentatively scheduled to open one hour prior to the first performance. NDA Unleashed Master Classes will be held this evening. NOTE: Division times will not be available until 2½ weeks prior to the championship. SUNDAY, FEBRUARY 21 KAY BAILEY HUTCHISON CONVENTION CENTER Final Competition for all divisions. 2 NCA & NDA TEAM PLACEMENT PROGRAM This program was put in place for all traveling teams utilizing hotel rooms while attending the NDA All-Star National Championship. Being recognized as an official citywide event and capturing the room nights derived from this event are crucial for future date selection and venue availability through the Dallas Convention and Visitors Bureau and the. Connections Housing is the official, and only, housing company for NCA & NDA Championship housing. Teams that attend the 2016 NDA All-Star National Championship in February are required to book rooms through Connections Housing in order to participate. NCA & NDA has authorized no other company to book housing for groups attending 2016 Nationals. Numerous hotels around Dallas/Fort Worth have been blocked for your ease and convenience in booking. 4 INFORMATION YOU NEED TO KNOW CREDIT CARD PAYMENT OPTION Visa, MasterCard, Discover and American Express may be used for full payment. The FULL BALANCE will be charged to your credit card. For your convenience, a credit card authorization form has been included in this packet. Note: If you want to add or change a credit card number, you must send the information in writing or fax to NDA. No credit cards will be taken over the phone. Individual credit card numbers will not be accepted for team members. LATE REGISTRATIONS If space is available, late registrations will be accepted after the designated deadlines by sending in the FULL BALANCE due with your registration. If you are registering late, the full payment must be submitted in the form of an official school check, cashier s check, money order or credit card. The standard cancellation policy will apply to all late registrations. CANCELLATIONS AND REFUNDS Notification of cancellations and request for refund must be made in writing, and ed or faxed to NDA. No cancellations will be accepted by phone. Cancellations may be faxed to NDA at NDA is not responsible for faxes not received. All cancellations must be received during business hours: Monday-Friday 8:30 am to 5:00 pm CST. If cancellation is not received during these hours, it will be dated the next business day. NOTE: Due to the complexity of creating and managing the event schedule, NDA will not accept any division changes after January 29th, This effort will help ensure that all gyms know well in advance their performance times, while also limiting any major time shifts. 8 COMBINING/SPLITTING Divisions will be combined first by size when applicable then by gender. Divisions will not be combined if a program is registered in the same category. (Example: Small Jazz and Large Jazz teams from the same program will not compete against each other. Choosing to register two Small Jazz teams from the same program will require said teams to compete against each other.) Small and Large: If there are not at least 3 teams in Small and 3 teams in Large in the Mini, Youth, Junior, Senior and/or Open Divisions within the same Category, Small and Large divisions must be combined (Example: If there are 2 teams in Small Senior Pom and 5 teams in Large Senior Pom, NDA will combine into Senior Pom. NDA will not combine these if it means that a program will compete against itself.) If there are 10 or more teams in the Small or Large division, then NDA may split that division further by team size. Coed and Male Combining: If there is only one team entered in any Junior, Senior or Open Division within the same Category, NDA must combine (in this order): 1. Sr. Coed with Sr. Male and Open Coed with Open Male 2. Sr. with Sr. Coed and Open with Open Coed 3. Sr. with Senior Male and Open with Open Male Prep: NDA must split the Prep category into Jazz, Pom or Hip Hop if there are at least 3 teams in one Division Category and at least 2 teams in the remaining Open category. In the Prep category a program cannot compete against itself; in this instance a mandatory split would occur. (However, programs that choose to register 2 teams in the same division category, will result in said teams competing against each other.) Tiny Division: NDA must split Tiny Divisions within the same category into Small and Large (as outlined in grid) if there are 10 or more teams in the division before the split occurs. There must be at least 3 teams in each division once the split occurs. AGE DIVISIONS Age requirements for the 2016 Dance Worlds may be found online at The Division (age) of a team is determined by the age of the oldest competitor. The age of the competitor as of August 31, 2015 will be the age used for competition purposes throughout the 2015/16 season for all divisions. Participant may compete in more than one division and/or category as long as they abide by the USASF age requirements. 2. Participant may only represent one gym.

10 ALL-STAR CHEER AND DANCE ATHLETE ID FORM Dear All-Star Gym/Studio Owner, In lieu of the Team Eligibility Roster that was previously required by NCA & NDA, all cheer and dance participants must now be registered in the USASF Athlete ID program. Registration is required prior to your first competition of the championship season. If you haven t already registered, please see the quick process below. ATHLETE ID PROCESS STEP 1 Prior to attending your first event in the season, register all cheer and dance athletes by entering information in member profiles at USASF.net. Athlete birth certificates will be uploaded to USASF secure servers for birthdate verification by USASF staff. Once birthdates are verified, birth certificate files are digitally destroyed and not stored on any server. STEP 2 Create and print USASF Official Event Rosters for each team prior to each event and submit at check-in. Please attach the Roster to this form.
