2 The health insurance plan that starts right here at home. Louisiana Health Cooperative, Inc. (LAHC) is a non-profit, member-governed health plan that makes insurance accessible to the people of Louisiana, including those with pre-existing conditions. We are an independent company endorsed by the largest, most respected healthcare providers in the state. We offer a wide selection of health insurance benefits, at competitive rates, for individuals, families and small businesses. Louisiana Health Cooperative will offer members: Guaranteed issue We accept any eligible individual who wants health insurance into our plan. Affordable choices We provide four comparable tiers of health plans, all of which limit out-of-pocket expenses. No cancellation We will not cancel or revoke our members health insurance benefits if their health status changes.»» No annual or lifetime limits We do not place annual or lifetime limits on health care, which means our members are always covered.
3 Have you ever been denied coverage or charged more because you have a pre-existing condition? As of January 2014, being sick doesn t keep you from getting health insurance. As a result of the Affordable Care Act, health insurance plans can no longer turn you down or charge you more because of your condition even if you ve been refused coverage in the past. Louisiana Health Cooperative is Louisiana s only statewide health insurance CO-OP. Health insurance CO-OPs, like Louisiana Health Cooperative, are being launched nationwide in accordance with the Affordable Care Act. On May 8, 2013, LAHC obtained licensure from the Louisiana Department of Insurance to operate as an insurer statewide. LAHC continues to contract with a comprehensive network of many of Louisiana s largest, most respected health care providers to offer programs and services to improve the quality of health care delivered to members. These physicians, hospitals and other health care professionals share LAHC s mission of promoting health and well-being to every single individual and providing high quality integrated health care to the communities they serve. LAHC is a local business based in the Greater New Orleans area, giving members the peaceof-mind that we ve got the best interests of Louisiana patients in mind.
4 Louisiana Health Cooperative will provide our members with these essential health benefits ambulatory patient services emergency care hospitalization maternity and newborn care mental health and substance use disorder service, including behavioral health treatment prescription drugs rehabilitative & restorative services and devices laboratory services preventative & wellness services, and chronic disease management pediatric services, including vision care Programs and services provided to members will include: preventative health programs, including health screenings ongoing measurement and assessment according to clinical quality standards opportunities for LAHC members to participate in their own care expanded resources for improved health outcomes
5 OUR MISSION Louisiana Health Cooperative s mission is to promote community health and well being by engaging the members and providers it serves in the valued delivery of high quality, integrated health care services. Important reminders about health insurance coverage If you have health insurance you like, keep it. However, LAHC could potentially provide you with lower premiums and less out-of-pocket costs. As of January 2014, you must have health insurance. If you don t, you will be charged at a minimum fee of $95 per uninsured adult and a minimum fee of $47.50 per uninsured child. Maximum fee for a family is $285. You might not qualify for lower cost LAHC plans. This depends on your annual income and whether your employer offers health insurance. It doesn t mean that you can t get your health insurance from Louisiana Health Cooperative. Health benefits vary from plan to plan even in the same state, there can be small differences. You can see them and compare when you re selecting your particular coverage here in Louisiana.
6 When will Louisiana Health Cooperative insurance plan options become available? Open enrollment for Louisiana Health Cooperative coincides with that of the federal Health Insurance Marketplace, beginning on October 1, 2013 and ending on March 31, You may be eligible for tax deductions and subsidies to help keep out-of-pocket costs down. Through the federal Health Insurance Marketplace (the Exchange ), low and moderate income families may be eligible to receive financial assistance to help reduce monthly out-of-pocket costs, such as premiums, deductibles and coinsurance.
7 Why choose a health insurance option from LAHC? You ll be able to access the health care you need, when you need it. You ll get preventative care, like vaccines, screenings and check-ups at no additional cost. You ll get coverage for prescription drugs, emergency care, doctor visits, and many other health care services. You ll get coverage for pre-existing conditions that may have kept you from getting coverage in the past. If you re a Louisiana resident and are not offered health insurance from your employer have a pre-existing condition and want health insurance work as an independent contractor, a freelancer or a small business owner have recently graduated from college and need to get off of your parent s plan are tired of getting the health insurance runaround and want guaranteed coverage then consider enrolling with Louisiana Health Cooperative today!
